Comparative image analysis of cells in voided and catheterized urine.

OBJECTIVE: To objectively evaluate the difference in cytologic findings between specimens of voided and catheterized urine by using a comparative image analysis device, CAS200. STUDY DESIGN: Cells in voided and catheterized urine from 13 patients with transitional cell carcinoma (TCC), including 3 with grade 1, 6 with grade 2 and 4 with grade 3, were compared cytologically. The cellular area, nuclear area, nuclear/cytoplasmic ratio and nuclear density of both types of cytologic specimen were measured using CAS200. RESULTS: Cell area and nuclear area of grade 1 TCCs were significantly greater

[Catheter-related urinary tract infections in the community].

The prevalence of urinary catheterization in the community ranges from 0.02% to 0.07%. Despite the generalized use of closed systems, the risk of bacteriuria in patients with urethral catheters is 3%-10% per day and its presence is universal when the device remains in place for 30 days or longer. Although most of these episodes of bacteriuria are asymptomatic, up to 30% of them lead to clinical symptoms and complications, including severe sepsis and death. The microorganisms infecting the urine of catheterized patients frequently belong to species less susceptible to antibiotics and form biofi…

Evaluation of urinary continence and voiding function: early results in men with neo-urethral modification of the Hautmann orthotopic neobladder.

PURPOSE: At our institution we use the Hautmann orthotopic bladder replacement with a chimney and neo-urethral modification. A neo-urethral tube allows tension-free intestino-urethral anastomosis, thus providing application of this procedure for patients who may otherwise not qualify due to the inability of the small bowel to reach the urethra. However, this neo-urethral tube may also enhance continence by providing significant intra-abdominal urethral length. Conversely, such a modification may be associated with a higher degree of urinary retention. Early evaluation and reporting on the resu

The Mitrofanoff principle in urinary reconstruction.

We report the use of the Mitrofanoff principle to achieve urinary continence in 47 patients. A catheterizable channel can be used as the primary continence mechanism, or as an adjunct to ensure complete bladder emptying if urethral catheterization or voiding is inadequate. Appendico-vesicostomy was performed in 25 of 47 patients (55%). Satisfactory continence was achieved in 45 patients (96%). Bladder emptying by clean intermittent catheterization was performed in all patients. Early complications included peristomal abscess (1 patient) and small bowel obstruction (1). Late complications inclu

Effects of different types of hysterectomies on postoperative urodynamics and lower urinary tract symptoms.

PURPOSE: To evaluate the effects of different hysterectomies-simple hysterectomy (SH) and radical hysterectomy (RH) with or without radiation therapy (RT) on urodynamics and lower urinary tract symptoms (LUTS). MATERIALS AND METHODS: Among patients who underwent urodynamic study between 2009 and 2019, those with RH history due to cervical cancer and SH for uterine myoma were included. Clinical parameters were compared after adjusting clinically significant baseline variables with multivariate regression. RESULTS: A total of 289 patients (RH-only, n=57; RH+RT, n=72; SH, n=160) were included.

Effects of postoperative analgesia on postpartum urinary retention in women undergoing cesarean delivery.

AIM: Various analgesics and administration methods are used to provide women undergoing cesarean delivery pain relief after surgery. We compared three methods of postoperative analgesia regarding the incidence of postpartum urinary retention (PUR) in primiparous women undergoing elective cesarean delivery. METHODS: We estimated post-void residual bladder volume after the first postpartum micturition among 150 parturient women. Risk factors stratified for PUR defined by 150-mL post-void residual bladder volume were analyzed. Obstetric parameters and prevalence of lower urinary tract symptoms a

Ileovesicostomy as an alternative form of bladder management in tetraplegic patients.

OBJECTIVES: Bladder management in tetraplegic patients traditionally has been intermittent catheterization by a caretaker, placement of indwelling suprapubic or urethral catheters, sphincterotomy and external catheter drainage, or supravesical urinary diversion with an ileal conduit. The aim of this study was to examine the ileovesicostomy as an alternative form of bladder management in such patients. METHODS: We report our experience with ileovesicostomy as an incontinent cutaneous urinary diversion not requiring ureteral reimplantation. Six tetraplegic patients who had experienced significa

Urinary catheter management.

The use of urinary catheters should be avoided whenever possible. Clean intermittent catheterization, when practical, is preferable to long-term catheterization. Suprapubic catheters offer some advantages, and condom catheters may be appropriate for some men. While clean handling of catheters is important, routine perineal cleaning and catheter irrigation or changing are ineffective in eliminating bacteriuria. Bacteriuria is inevitable in patients requiring long-term catheterization, but only symptomatic infections should be treated. Infections are usually polymicrobial, and seriously ill pati

The Aquacath hydrophilic coated single-use urinary catheter.

Single-use hydrophilic catheters are now being used widely throughout this country, Europe and America, for intermittent self-catheterization or carer-assisted intermittent catheterization, with considerable success. They are also reported to provide greater comfort for users. The latest catheter to become available on prescription is the Aquacath coated urinary catheter, produced by Seton Continence Care. This hydrophilic catheter performed very well in a trial comparing coating stability after wetting of various coated catheters (Jones, 1997). The teaching of intermittent self-catheterizatio

Living-related kidney transplantation with catheterizable urinary conduit in prune belly syndrome: A case report.

INTRODUCTION: Prune belly syndrome (PBS) presents with both renal dysplasia and urinary tract abnormalities. When performing kidney transplantation in PBS patients with kidney failure, extensive pretransplant urinary tract preparation may be necessary. PRESENTATION OF CASE: We report the case of a 36-year-old man with PBS who underwent living-related kidney transplantation with urinary diversion using the Mitrofanoff principle. The patient had a bilateral loop ureterostomy for a urethral obstruction. Cystourethrography before the operation showed tortuous and dilated ureters with vesicoureter

Monitoring urinary bladder volume and detecting post-operative urinary retention in children with an ultrasound scanner.

BACKGROUND: Post-operative urinary retention (PUR) is associated with a risk of over-distension and permanent detrusor damage. The prevention of PUR by routine catheterization may increase the risk of urinary tract infection. Post-operative monitoring of the bladder volume is a reliable method in adults, but has not been evaluated for reliability in children. METHODS: Forty-eight patients who, according to current clinical guidelines, required urinary bladder emptying were scanned before catheterization using the Bladderscan. Volumes estimated with ultrasound were compared with volumes measur

Kidney transplantation in patients with neurovesical dysfunction.

BACKGROUND: Five renal recipients with neurovesical dysfunction (NVD) were retrospectively reviewed focusing on anatomical and urodynamic abnormalities of the lower urinary tract and their management prior to kidney transplantation. METHODS: The underlying anomalies in these 5 patients were a posterior urethral valve (1 with an imperforate anus; n = 2), meningomyelocele (n = 2) and a congenital short urethra with an imperforate anus (n = 1). Their urinary tracts were evaluated prior to transplantation with voiding cystourethrography, urethrocystoscopy, cystometrography and electromyography of

Urinary Complications and Injuries Related to Vaginal Mesh Surgery for Pelvic Organ Prolapse: Prevention and Management.

Background/Objectives: Pelvic organ prolapse affects up to one in five postmenopausal women. Although transvaginal mesh procedures are restricted to selected cases, they may be followed by transient postoperative urinary retention (POUR) and, rarely, serious urinary tract complications. This study assessed the frequency and risk factors for urinary complications after vaginal mesh prolapse repair and described strategies for their prevention, early detection, and management. Methods: This retrospective, single-center observational study with nested case series included 517 postmenopausal women

Clean intermittent catheterization in Saudi children. Suggestion for a common protocol.

In the last thirty years, clean intermittent catheterization of urinary bladder has proven to be one of the most important advances in Urology. Clean intermittent catheterizationis already utilized in the Kingdom of Saudi Arabia, but the materials and the methods in use are not always the most appropriate. The acceptance of the long-term treatment with clean intermittent catheterization among Saudi families does not seem to be adequate. Moreover, the knowledge and the agreement about clean intermittent catheterization by Saudi Medical Community, and even among Urologists, does not seem to be s

Automatic urine cultures from catheter-obtained samples-time to implement a practice change to improve value.

OBJECTIVES: Urine cultures are often automatically obtained after urinary catheterization in young children, even in the absence of pyuria, to test for urinary tract infections (UTIs). Although this practice conflicts with some newer guidelines, it is frequently followed in busy emergency departments (EDs) to minimize the need for a repeat invasive procedure. To assess the implications of this longstanding practice, we aimed to describe the frequency and characteristics of children with normal urinalysis (UA) and positive urine culture obtained via catheterization, and to describe their clinic…

Upper urothelial tract high-grade carcinoma: comparison of urine cytology and DNA methylation analysis in urinary samples.

Numerous studies showed that bladder urothelial carcinoma and upper urothelial tract carcinoma (UTUC) display clinical and genomic similarities. In order to analyze that the same panel of biomarkers used in the diagnosis of bladder urothelial carcinoma could be suitable for early detection of UTUC, we performed a retrospective study in which we analyzed Bladder EpiCheck scores in the urinary samples obtained by selective ureteral catheterization in a high-grade UTUC cohort, correlating the results with urinary cytology and diagnostic urethral biopsies. The present study represents a retrospect

Long-term intermittent catheterization in the spinal cord injury patient.

Intermittent catheterization was used as a means of long-term bladder management in 85 patients with spinal cord injury. Of these patients followup data revealed that 28 (33 per cent) had reflux and/or hydronephrosis. Treatment in 15 patients consisted of increasing the frequency of catheterization to every 4 hours and avoiding high fluid intake during a relative short interval. Sphincterotomy was done in 3 patients, while 10 were placed on an indwelling catheter because of an inability to adapt to or refusal of other forms of treatment. The upper urinary tract changes noted seemed to be relat

The effect of nurse-led bladder training on urinary tract outcomes after kidney transplantation.

INTRODUCTION: Optimal bladder function is crucial for kidney transplant success. Bladder training could help prevent dysfunction and complications, yet evidence-based guidelines for bladder training methods are limited. This study evaluates two protocols: standard hourly training versus tailored to preoperative urine output, assessing urinary tract outcomes. METHODS: This pre-post study used data from the TransplantLines data and biobank on kidney transplant patients admitted in 2019 (standard bladder training) and 2022 (new protocol) to a Dutch academic hospital.
