Uvulopalatopharyngoplasty (UPPP) is reported successful in treatment of obstructive sleep apnea for approximately 50% of patients. Several modifications of the procedure have been described, including transpalatal advancement pharyngoplasty, which resects a portion of posterior hard palate and advances the soft palate anteriorly. Comparing effectiveness of different techniques based on sleep and respiratory data is confounded by multiple variables including clinical failure at nonsurgical sites and imprecise patient selection techniques. Since pharyngeal surgical procedures prevent collapse an

OBJECTIVE: To determine if there are any pathologic or technical differences between laser-assisted uvulopalatopharyngoplasty (LAUP) and electrocautery-assisted uvulopalatopharyngoplasty (EAUP).
DESIGN: A prospective randomized in vivo trial in an appropriate animal species.
RESULTS: We found no gross difference in healing between the palates treated with laser and those treated with high-frequency electrocautery. However, there was consistently more thermal damage noted histologically in the hemipalates treated with LAUP. On the other hand, EAUP was faster to perform, more hemostatic, assoc

Transpalatal advancement pharyngoplasty is postulated to enlarge the retropalatal airway compared to uvulopalatopharyngoplasty (UPPP) for treatment of the obstructive sleep apnea syndrome (OSAS). Surgical success in treating OSAS has been associated with increasing size and decreasing collapsibility of the retropalatal airway to prevent airway obstruction. To compare transpalatal advancement pharyngoplasty compared to UPPP, upper airway characteristics were measured following each procedure in seven patients using a quantitative endoscopic technique. Following transpalatal advancement pharyngo

It has been proposed that sleep nasendoscopy (SN) will improve the success rate of the uvulopalatopharyngoplasty operation by identifying those patients with palatal snoring. The aim of this study was to test the efficacy of SN in the management of snorers who do not have obstructive sleep apnoea. This study compared a group of 26 snorers managed without SN (group A) to a group of 27 snorers managed with SN (group B). The post-operative results of group A were 61 per cent cured, 27 per cent better and 8 per cent unchanged. Group B results were 76 per cent cured, 19 per cent better and 5 per ce

Isolated uvulitis is a rarely reported disease. Known etiologies include bacterial infection and trauma following the use of instrumentation in the airway. Three cases of isolated uvulitis secondary to heavy marijuana smoke inhalation are reported. Specific recommendations regarding diagnosis and treatment of this disorder are made.
