The effect of truncal vagotomy and pyloroplasty on rat gastric mucosal H,K-ATPase and HCO3-ATPase activities was studied 15 and 30 days past the operation. A significant decrease in gastric body mucosal H,K-ATPase activity occurred 15 days after vagotomy, compared with pyloroplasty (p < 0.05) and non-operated control rats (p < 0.01). A recovery in the enzyme activity on the 30th postoperative day occurred. Ischaemic necrosis of the lesser curve of the stomach is thought to be a specific but rare complication of highly selective vagotomy. While it commonly presents as a free perforation, it may also present as sloughing of the mucosa only, a gastric fistula or a sealed perforation of the lesser curve mimicking a chronic gastric ulcer. We describe a case in which ischaemic necrosis of the lesser curve presented as a chronic gastric ulcer from a sealed perforation.

Pyloric channel ulcers have a significantly higher recurrence rate than duodenal ulcers after proximal gastric vagotomy (PGV) without drainage. The failure of PGV cannot be explained by the pre- and postoperative secretory pattern of pyloric (PU) and prepyloric (PPU) ulcers. 2-Deoxy-D-glucose (2-DG), a potent vagal stimulator, is used for testing the completeness of surgical vagotomies. 2-DG has however not yet been employed routinely for the postoperative testing of bagotomized patients because of possible cardiotoxic effects. The clinical results of truncal vagotomy and drainage for duodenal ulcer in 500 patients are evaluated by a personal 6-8 year follow up. Pyloroplasty was used unless pyloric stenosis made a gastrojejunostomy necessary. Forty-three patients had recurrence (39 were reoperated) and gastric ulcer was seen in five. The rate of dumping was 24% (severe in 3%) and rate of daily-monthly diarrhoea 40% (severe in 8%). The function of residual G cells after selective vagotomy with pyloroplasty (SV + P) in 24 duodenal ulcer patients was assessed by physiological and electron microscopical studies. Nonresective surgery for gastro-duodenal ulcers (GDU) is the operation of choice in 95% of cases of duodenal ulcer (DU) and approximately 50% of cases of gastric ulcer (GU), with a good probability of permanent success if an adequate selective proximal vagotomy (a-SPV) is combined with a pyloroplasty based on form and function (ff-Py).
