Orchiopexy using microvascular surgical technique.

Orchiopexy of high intra-abdominal testes with division of the internal spermatic artery and vein is associated with subsequent testicular atrophy in a significant percentage of cases. We herein describe 2 patients in whom arterial supply and venous drainage to the testis were maintained using microvascular anastomosis. The internal spermatic artery and vein were anastomosed to the deep inferior epigastric artery and vein. Patency of the vascular anastomosis was verified by subsequent radionuclide examinations and selective arteriography in 1 patient.

National trends and outcomes of pedal bypass surgery.

OBJECTIVE: The technical demands associated with pedal bypass (PB) surgery place it at risk of underutilization and may be limiting its widespread adoption as a valuable revascularization modality. This study aims to evaluate trends in PB performance, assess its outcomes, and compare its results between high- and low-volume centers. METHODS: All patients receiving a PB between 2003 and 2023 were identified in the Vascular Quality Initiative (VQI) infrainguinal bypass (IIB) module. The ratio of PB to total IIB performed was calculated for each year and trended over the study period. Centers pe

Supra-Inguinal Inflow for Distal Bypasses Have Acceptable Patency and Limb Salvage Rates.

BACKGROUND: There is a paucity of data evaluating outcomes of lower extremity bypass (LEB) using supra-inguinal inflow for revascularization of infra-inguinal vessels. The purpose of this study is to report outcomes after LEB originating from aortoiliac arteries to infra-femoral targets. METHODS: The Vascular Quality Initiative database (2003-2020) was queried for patients undergoing LEB from the aortoiliac arteries to the popliteal and tibial arteries. Patients were stratified into 3 cohorts based on outflow targets (above-knee [AK] popliteal, below-knee [BK] popliteal, and tibial arteries).

Optimal conduit choice in the absence of single-segment great saphenous vein for below-knee popliteal bypass.

BACKGROUND: Single-segment great saphenous vein (SSGSV) remains the conduit of choice for femoral to below-knee popliteal (F-BK) surgical revascularization. The purpose of this study was to determine the optimal conduit in patients with inadequate SSGSV. METHODS: This was a retrospective review of a prospectively maintained vascular registry. Patients underwent F-BK bypass with alternative vein (AV; arm vein, spliced GSV, or composite vein) or prosthetic conduit (PC). RESULTS: From January 1995 to June 2010, 83 patients had unusable SSGSV for F-BK popliteal reconstruction. Thirty-three patie

Treatment options of crural pseudoaneurysms.

BACKGROUND: Pseudoaneurysms (PAs) of crural arteries represent rare complications of vascular interventions or surgery. Management of crural PAs includes different treatment options, conservative treatment as well as open surgery or endovascular procedures. We reviewed our experience. PATIENTS AND METHODS: We retrospectively analysed all patients who were diagnosed with crural PAs since 2003. We evaluated etiology, treatment and outcome. Endpoints were target vessel patency, vascular re-intervention and limb loss. RESULTS: A total of 30 patients were diagnosed with crural PAs. PA was caused

Histamine H3 receptor activation inhibits neurogenic sympathetic vasoconstriction in porcine nasal mucosa.

Histamine release from mast cells is a primary mediator of rhinorrhea, nasal mucosal swelling, increased secretion, sneezing, pruritus and congestion that occur in allergic rhinitis. It is well known that histamine H(1) receptor antagonists inhibit the itch and rhinorhea, but do not block the allergic nasal congestion. A growing body of evidence shows that in addition to histamine H(1) receptors, activation of H(3) receptors may contribute to the procongestant nasal actions of histamine. Activation of the prejunctional histamine H(3) receptor modulates sympathetic control of nasal vascular ton

[Arteriovenous brachio-basilic fistula as hemodialysis port. Original technique and long term results].

The authors report a prospective series of patients with-long term vascular access for chronic hemodialysis fashioned with a transposed brachio-basilic fistula. Over a three-year period (1998-2000), 14 patients underwent creation of the arteriovenous fistula. Demographic and clinical data were registered and a length of follow up more than 12 months was in all cases obtained. Of the 14 fistulas performed, 7 were successfully used for dialysis without complication. In seven patients late revision of fistulas was performed. The primary and secondary patency actuarial rates at two years were 50%

Femoro-popliteal bypass simplified.

There is increasing emphasis on carrying femoro-popliteal bypass grafts below the knee and most vascular surgeons agree that reversed saphenous vein makes the best graft. However, some other considerations have become important and deserve consideration. P.T.F.E. grafts in this location have almost as good a long term patency rate as autologous vein grafts. Some very experienced vascular surgeons have indicated their preference for this type of graft, citing some of these pertinent reasons: Peripheral vascular disease usually is associated with lesions in other vessels. Since coronary artery b

Drug-coated balloon angioplasty for dialysis access fistula stenosis.

Maintaining vascular access patency represents a tremendous challenge in hemodialysis patients. Although "native" arteriovenous fistula (AVF) is currently recommended as primary vascular access, neointimal hyperplasia stenoses frequently develop, with a risk for AVF thrombosis and vascular access loss. For years, first-line treatment of AVFs stenoses has been percutaneous transluminal angioplasty, generally with high-pressure or cutting uncoated balloons. However, restenosis and reintervention rates remain incredibly high and occur, according to recent studies, in up to 60% and 70% of patients

Free flap survival despite early loss of the vascular pedicle.

BACKGROUND: Early success of microvascular free tissue transfer is dependent upon the patency of the primary vascular pedicle. In time, neovascularization from the recipient bed and surrounding wound margins into the graft may be sufficient to maintain flap viability. The time necessary for successful neovascularization to occur is unclear. Most believe that prior radiation therapy will delay this process. METHODS: This case report describes a patient, status postchemoradiotherapy, who underwent composite resection with anterolateral thigh free flap reconstruction for a new base of tongue squ

Endovascular Salvage of Native Arteriovenous Fistulas: Post-Intervention Primary and Cumulative Patency Outcomes in a Colombian Regional Referral Center.

INTRODUCTION: Arteriovenous fistulas (AVFs) are the preferred vascular access for hemodialysis due to superior patency and lower complication rates. However, access dysfunction caused by stenosis and thrombosis remains frequent, requiring repeated interventions. Endovascular techniques have emerged as the first-line approach for AVF salvage, although real-world data from Latin America are limited. The objective of this study is to characterize clinical, anatomical, and procedural features of patients undergoing endovascular AVF salvage and to evaluate postintervention primary, assisted primary

Artery-to-fistula diameter ratio as a predictor of target lesion primary patency in the juxta-anastomotic segment after percutaneous transluminal angioplasty.

BACKGROUND: Percutaneous Transluminal Angioplasty (PTA) is a frequently performed intervention for repairing dysfunctional arteriovenous fistula (AVF). However, there is few data on the predictive factors for target lesion primary patency (TLPP), especially in the juxta-anastomotic segment. Our study aims to explore the relationship between the artery-to-fistula diameter ratio and TLPP in patients with mature radiocephalic AVF after PTA. METHODS: We conducted a retrospective analysis of 104 CKD patients who underwent PTA due to recurrent stenosis that only affected the juxta-anastomotic area

The upper arm basilic-cephalic loop for distal bypass grafting: technical considerations and follow-up.

PURPOSE: The performance of a graft created from the upper arm basilic and cephalic veins in continuity was investigated. METHODS: Retrospective analysis of 50 patients, who underwent 54 distal reconstructions with an upper arm vein loop graft between February 1989 and October 1993 (male-to-female ratio of 30/20; mean age of 69.2 years, range 39 to 87; 74% had diabetes) was undertaken. Vein grafts were harvested through a near continuous incision, leaving a skin bridge in the cubita. Intraoperative angioscopy was used to exclude endoluminal disease and to directly observe valvulotomy of the n

End-to-end proximal anastomosis is the sole modifiable variable in risk score for loss of patency of aortobifemoral artery bypass after index operative hospitalization.

OBJECTIVE: The purpose of this study was to create a risk score for loss of aortobifemoral artery bypass (ABF) patency using preoperative, perioperative, and long-term follow-up (LTFU) variables in the Vascular Quality Initiative database. METHODS: The Vascular Quality Initiative suprainguinal arterial bypass module was queried from 2009 to 2025, and 4971 patients undergoing ABF had LTFU patency data and thus met inclusion criteria. These patients were then divided into a two-thirds testing cohort (N = 3364) and a one-third validation cohort (N = 1607), in which the risk score was evaluated.

Percutaneous transhepatic venous access for hemodialysis: a single-center experience with a rescue access.

INTRODUCTION: Central venous catheters (CVC) are often the only option for hemodialysis, particularly when arteriovenous fistulas cannot be created or in urgent situations. However, the exhaustion of traditional access sites necessitates alternative approaches. This study aims to describe our center's experience with transhepatic venous access for hemodialysis, focusing on infection rates, catheter patency, and dialysis adequacy, to evaluate the feasibility of this option in patients with limited vascular access options. METHODS: We conducted a retrospective study at Pro-Rim Foundation (Janua
