[Retinal vasculitis in systemic lupus erythematosus].

The authors report three patients with systemic lupus erythematosus (S.L.E.) who developed primary retinal vasculitis. All patients showed evidence of four American Rheumatism Association diagnostic criteria for S.L.E. The first case consisted of a woman with retinal central vein occlusion. Auto-immunization against retinal S antigen was present. She improved under heparin therapy. In the second case, a young girl had chronic optic disk vasculitis for seven years. In the third case, retinal arterial occlusive disease was present with optic nerve involvement. The differential diagnosis with mul…

Rheumatoid vasculitis: diagnostic and therapeutic decisions.

Rheumatoid vasculitis is an uncommon but potentially catastrophic complication of RA. There are few extensive experiences recorded in the current literature and there is no consensus regarding the clinical features, laboratory findings, histologic pattern, prognosis, or appropriate management of this syndrome. We therefore surveyed 1,947 North American ARA members for their perceptions of rheumatoid vasculitis. Four hundred twenty-eight surveys were returned, of which 290 were suitable for analysis. The majority of respondents were within 10 years of fellowships and were evenly distributed amo…

Angiographic findings in systemic necrotizing vasculitis.

Angiographic studies were carried out on 21 patients with systemic necrotizing vasculitis. Four basic arterial anomalies were found: (1) saccular microaneurysms appeared in 62% of the patients (2) arterial thrombosis was seen in 81% of patients; (3) arterial stenosis occurred in 81%; and (4) lumen irregularities occurred in 90%. Alterations in the renal vascular flow were also observed in accordance with changes in the cortical medullary differentiation, heterogeneous nephrogram, and prolonged washout. Two patients showed regression of microaneurysms after immunosuppressive therapy. We found a…

[Vasculitis in children's dermatomyositis (author's transl)].

Vascular lesions have been discovered by a number of authors in children's dermatomyositis, when they are not ordinary found in adults. Affecting capillaries, arterioles and veinlets, vasculitis appears as an hyperplasia of intima, then as an endothelial necrosis and formation of thrombi. Usually seen with light microscope, these lesions are sometimes only revealed by electron microscope. These vascular lesions are observed in the muscle and mainly in the perimysial interstitial tissue, in the skin, and in the various visceral localisations of the disease: pulmonary, ocular, renal, nervous and…

Cutaneous necrotizing vasculitis and related disorders.

Leukocytoclastic vasculitis of the skin comprises several distinct clinical syndromes, each with immune complex deposition and some form of complement activation. Necrotizing vasculitides of larger blood vessels also produce skin lesions. These can be distinguished morphologically from those of small vessel vasculitis. Analysis of the features of these lesions is discussed.

Fluorescein angiography in syphilitic retinal vasculitis.

A case of ocular syphilis with severe ischemic retinopathy, rubeosis iridis and secondary glaucoma is described. Fluorescein angiography revealed a complete blockage of all the vessels around the macula, with dye leakage from numerous bifurcations of veins and an infiltrative mass in the macular area of both eyes. Combined treatment with steroids and penicillin brought no improvement in this condition of severe, ischemic ophthalmia.

A case of pathologically confirmed streptococcal infection-related IgA vasculitis with associated glomerulonephritis and leukocytoclastic cutaneous vasculitis.

We report the case of an 80 year-old woman who developed bilateral lower extremity purpura and renal impairment with proteinuria a few days after a transient fever (day 0). High levels of both anti-streptolysin-O antibody (ASO) and anti-streptokinase antibody (ASK), as well as low levels of coagulation factor XIII in serum were noted. Skin biopsy was performed and showed a leukocytoclastic vasculitis with deposition of IgA and C3 in the cutaneous small vessels, indicating IgA vasculitis in the skin. After initiation of oral prednisolone, the skin lesions showed significant improvement. However…

Vanishing vasculitis: a case of acute necrotic skin findings without pathologic features of vasculitis from adulterated cocaine.

While the usage of illicit drugs in itself carries significant health risks and associated toxicities, drugs that are adulterated to give th … e adulterated with levamisole syndrome, this can range from the classic finding of leukocytoclastic vasculitis of small vessels to occlusive vascular disease without true vasculitis. Our case's biopsy showed no vasculitis, and this is why it is important to highlight that cocaine can also cause a pseudo-vasculitic picture. The other possibility that we enter …

Therapy of resistant systemic necrotizing vasculitis. Polyarteritis, Churg-Strauss syndrome, Wegener's granulomatosis, and hypersensitivity vasculitis group disorders.

The systemic necrotizing vasculitides represent a difficult group of diseases from a therapeutic perspective. Even with successful therapy patients often experience a high degree of disease and treatment morbidity. Refractory disease, or that which fails to respond to standard therapy, is particularly challenging. A clinical approach to several vasculitic syndromes is outlined in this article with emphasis given to therapies supported by controlled investigations and those supported by consensus opinion where available.

Relapsing Hepatitis B-associated Vasculitis with Features of Polyarteritis Nodosa and c-Antineutrophilic Cytoplasmic Antibodies-associated Vasculitis.

Hepatitis B virus (HBV)-associated vasculitis manifests as vasculitis of the medium vessel and polyarteritis nodosa (PAN). HBV-associated PAN (HBV-PAN) is negative for antineutrophilic cytoplasmic antibodies (ANCA), without glomerulonephritis (GN) or pulmonary involvement; relapse does not occur after successful HBV seroconversion. We describe a case of HBV-associated vasculitis characterized by small and medium overlapping vasculitis and an atypical long-term course in a 60-year-old male with hypertension and HBV who initially presented with abdominal pain and scrotal swelling. The abdominal …

2021 American College of Rheumatology/Vasculitis Foundation Guideline for the Management of Antineutrophil Cytoplasmic Antibody-Associated Vasculitis.

OBJECTIVE: To provide evidence-based recommendations and expert guidance for the management of antineutrophil cytoplasmic antibody-associated vasculitis (AAV), including granulomatosis with polyangiitis (GPA), microscopic polyangiitis (MPA), and eosinophilic granulomatosis with polyangiitis (EGPA). METHODS: Clinical questions regarding the treatment and management of AAV were developed in the population, intervention, comparator, and outcome (PICO) format (47 for GPA/MPA, 34 for EGPA). Systematic literature reviews were conducted for each PICO question. The Grading of Recommendations Assessme…
