Back to the wildtype: SARS-CoV-2 evolution in critically ill patients with severe lung failure.

OBJECTIVE: To investigate intra-host evolution of SARS-CoV-2 in critically ill patients with severe lung failure. METHODS: Between November 2020 to December 2022, respiratory samples were collected from 41 mechanically ventilated patients at the intensive care unit of University Hospital Regensburg, Germany, including 16 on extracorporeal membrane oxygenation (ECMO). Paired initial and follow-up samples were obtained at a median interval of 15 days (range: 6–42) and analyzed using next-generation sequencing. Amino acid substitutions in the viral genome were correlated with clinical, virologica

A single-center, single-arm, prospective, open-label, and comparative trial to evaluate the safety and tolerability profile of a 90-day oral L-arginine hydrochloride intervention for patients with amyotrophic lateral sclerosis.

Weight loss, a key indicator of malnutrition in amyotrophic lateral sclerosis (ALS) patients, negatively impacts patient prognosis. However, effective nutritional interventions have not been adequately established. Research in ALS model mice has shown that L-arginine can prolong survival; however, no human intervention studies have been conducted. We conducted a single-center, single-arm, prospective, open-label, and comparative trial to assess the safety and tolerability of L-arginine hydrochloride in ALS patients. ALS patients were administered 15 g/day L-arginine hydrochloride for 90 days.

Cefiderocol Resistance in Pseudomonas aeruginosa ST175: A Case Report with Genomic Analysis.

BACKGROUND: Cefiderocol, a novel siderophore cephalosporin, has shown promising activity against multidrug-resistant (MDR) Gram-negative pathogens, including metallo-β-lactamase (MBL) producers. However, emerging reports suggest rapid resistance development under selective pressure, particularly in epidemic Pseudomonas aeruginosa lineages. CASE PRESENTATION: We describe a 50-year-old man with a history of aortic dissection who developed ventilator-associated pneumonia due to P. aeruginosa ST175 carrying an IMP-type carbapenemase. Initial treatment with ceftazidime/avibactam plus aztreonam was

Pathological diagnosis of transposition of the great arteries as a contributor to neonatal mortality.

BACKGROUND: Transposition of the great arteries (TGA) is a rare cyanotic congenital heart defect, classified into complete TGA and corrected TGA. Late presentations are common in the majority of complete TGA patients alongside ventricular septal defect (VSD) or atrial septal defect (ASD). Complete TGA without accompanying cardiac malformations exhibits low morbidity and high mortality rates. CASE PRESENTATION: A 30-year-old woman, primigravida presented with a negative general and obstetric history. The patient was admitted at 38 weeks and 6 days of gestation due to persistent lower abdominal

Incidence of bloodstream infections in patients with COVID-19: a retrospective cohort study of risk factors and outcomes.

INTRODUCTION: Prior evidence found that bloodstream infections (BSIs) are common in viral respiratory infections and can lead to heightened morbidity and mortality. We described the incidence, risk factors, and outcomes of BSIs in patients with COVID-19. METHODS: This was a single-center retrospective cohort study of adults consecutively admitted from March to June 2020 for COVID-19 with BSIs. Data were collected by electronic medical record review. BSIs were defined as positive blood cultures (BCs) with a known pathogen in one or more BCs or the same commensal organism in two or more BCs. R

The effect of PEEP on oxygenating capacity in acute respiratory failure with sepsis.

We report an evaluation of the effect of postive-end-expiratory-pressure (PEEP) on improving pulmonary oxygenating capacity in the adult respiratory distress syndrome (ARDS), when the latter is associated with generalized gram-negative sepsis. Fifty-seven cases treated in our RICU with PEEP ventilation (April 1972 to January 1975) were retrospectively reviewed. Oxygenating capacity improvement was evaluated in terms of the changes in PaO2/FIO2 and AaDO2 (FIO2 = 1.0). Both the short term (2-3 hours from the initiation of PEEP) and the overall effects of PEEP were evaluated. A mean PEEP of 5.6 c

Pneumatocele With Perforation of the Residual Lung Immediately After Thoracoscopic Left Lower Lobectomy: A Case Report.

Cases of pneumatocele formation following pulmonary resection are increasingly reported, yet its underlying pathogenesis and optimal treatment remain unclear. Here, we present a case of pneumatocele perforation occurring immediately after thoracoscopic left lower lobectomy. A 66-year-old man was found to have a suspicious nodule in the left lower lung zone during a routine chest X-ray as part of a medical checkup. CT suggested lung cancer in the left lower lobe, prompting referral to our hospital for further evaluation and treatment. Contrasted CT imaging revealed a ground-glass nodule with a

Short-term effects of bracing on exercise performance in mild idiopathic thoracic scoliosis.

In adolescent idiopathic thoracic scoliosis (ITS) working capacity may be reduced during exercise. Despite concern about its usefulness, bracing is still being used in ITS. Thus the effects of bracing on exercise performance need to be examined. We studied six females, ages 12-15 years who had mild ITS (Cobb angle range 20-35 degrees). Pulmonary volumes, maximal voluntary ventilation (MVV), breathing pattern, the lowest (most negative in sign) pleural pressure during sniff maneuver (Pplsn), and pleural pressure swings (Pplsw) were measured first. Then, Pplsw, O2 uptake (VO2), CO2 output (VCO2)

Echocardiographic predictors of adverse short-term outcomes after heart surgery in patients with mitral regurgitation and pulmonary hypertension.

BACKGROUND: Pulmonary hypertension (PH) is a frequent occurrence and a negative prognostic indicator in patients with mitral regurgitation. Preoperative PH causes higher early and late mortality rates after heart surgery, adverse cardiac events, and postoperative systolic dysfunction in the left ventricle (LV). METHODS: The research consisted of a retrospective study of a group of 171 consecutive patients with mitral regurgitation and preoperative PH who had undergone mitral valve surgery between January 2008 and October 2011. The PH diagnosis was based on echocardiographic evidence (systolic

[Recommendations of the Infectious Diseases Work Group (GTEI) of the Spanish Society of Intensive and Critical Care Medicine and Coronary Units (SEMICYUC) and the Infections in Critically Ill Patients Study Group (GEIPC) of the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC) for the diagnosis and treatment of influenza A/H1N1 in seriously ill adults admitted to the Intensive Care Unit].

The diagnosis of influenza A/H1N1 is mainly clinical, particularly during peak or seasonal flu outbreaks. A diagnostic test should be performed in all patients with fever and flu symptoms that require hospitalization. The respiratory sample (nasal or pharyngeal exudate or deeper sample in intubated patients) should be obtained as soon as possible, with the immediate start of empirical antiviral treatment. Molecular methods based on nucleic acid amplification techniques (RT-PCR) are the gold standard for the diagnosis of influenza A/H1N1. Immunochromatographic methods have low sensitivity; a ne

Mechanical Ventilation Redistributes Blood to Poorly Ventilated Areas in Experimental Lung Injury.

OBJECTIVES: Determine the intra-tidal regional gas and blood volume distributions at different levels of atelectasis in experimental lung injury. Test the hypotheses that pulmonary aeration and blood volume matching is reduced during inspiration in the setting of minimal tidal recruitment/derecruitment and that this mismatching is an important determinant of hypoxemia. DESIGN: Preclinical study. SETTING: Research laboratory. SUBJECTS: Seven anesthetized pigs 28.7 kg (SD, 2.1 kg). INTERVENTIONS: All animals received a saline-lavage surfactant depletion lung injury model. Positive end-expira

Correlates of Spreading Depolarization, Spreading Depression, and Negative Ultraslow Potential in Epidural Versus Subdural Electrocorticography.

Spreading depolarizations (SDs) are characterized by near-complete breakdown of the transmembrane ion gradients, neuronal oedema and activity loss (=depression). The SD extreme in ischemic tissue, termed 'terminal SD,' shows prolonged depolarization, in addition to a slow baseline variation called 'negative ultraslow potential' (NUP). The NUP is the largest bioelectrical signal ever recorded from the human brain and is thought to reflect the progressive recruitment of neurons into death in the wake of SD. However, it is unclear whether the NUP is a field potential or results from contaminating

Impact of cervical spine management brain injury on functional survival outcomes in comatose, blunt trauma patients with extremity movement and negative cervical spine CT: application of the Monte Carlo simulation.

Cervical spine (CS) magnetic resonance imaging (MRI) and collar use may prevent quadriplegia, yet create brain injury. We developed a computer model to assess the effect of CS management strategies on outcomes in comatose, blunt trauma patients with extremity movement and a negative CS CT scan. Strategies include early collar removal (ECR), ECR & MRI, late collar removal (LCR), and LCR & MRI. MRI risks include hypoxia, hypotension, increased intracranial pressure (↑ICP), and ventilator-associated pneumonia (VAP). LCR risks include ↑ICP, VAP, and delirium. Model elements include Quadriplegia an

Changes in microcirculation variables in an acute endotoxaemic equine model.

BACKGROUND: Microcirculation is the essential link between macrocirculation and cellular metabolism. OBJECTIVES: To test our hypotheses that microcirculation variables will show a heterogeneous flow pattern during experimental endotoxaemia, and that fluid therapy and noradrenaline (NA) infusion will normalise altered microcirculation variables. STUDY DESIGN: In vivo experiments. METHODS: Six healthy adult horses were anaesthetised with dexmedetomidine, ketamine, and diazepam and were mechanically ventilated under isoflurane anaesthesia. Endotoxaemia was induced with 30 ng kg-1 Escherichia c

The value of cardiopulmonary exercise testing in the diagnosis of pulmonary hypertension.

BACKGROUND: Cardiopulmonary exercise testing (CPET) continuously analyzes the gas exchange of patients during rest, exercise, recovery, and simultaneously records the response of the cardiopulmonary system. This study aimed to observe the characteristics of CPET in patients with pulmonary hypertension (PH) and to explore the cutoff value of CPET variables in detecting PH. The diagnostic value of CPET was also investigated in a subgroup of patients who had an incorrect or missed diagnosis of PH by echocardiography. METHODS: Treatment-naïve patients with suspected PH who were admitted to Fuwai

Beyond mean and maximum CO₂: introducing hypercapnic burden as a marker of sleep-related hypoventilation.

OBJECTIVES: In obesity hypoventilation syndrome (OHS), nocturnal hypoventilation can precede daytime respiratory failure. Therefore, the hypercapnic burden index was introduced to quantify sleep-related CO₂ retention. METHODS: Thirty-one patients who underwent polysomnography with transcutaneous capnography and met OHS criteria were retrospectively included in this single-center cross-sectional study. The area above the predetermined transcutaneous partial pressure of CO2 thresholds of 45 mmHg (thresh45) and 50 mmHg (thresh50) from the extracted capnography graphs was computed and statistical
