[Phobic postural vertigo: clinical aspects and course of illness].

Phobic postural vertigo (PPV) is the second most common diagnosis on a dizziness unit. It is a somatoform syndrome characterized as a chronic and incapacitating condition with subjective imbalance and short attacks of dizziness. During a period of 18 months, PPV was observed in 41 patients among 251 of a dizziness unit. Twenty-six had primary PPV, among whom 65% had depressive or anxiety disorders, and 15 patients were diagnosed at secondary PPV. Normal neurological examination and diagnostic tests were observed in most cases. A favorable response to treatment (antidepressants, benzodiazepines…

Visual vertigo: an observational case series of eleven patients.

BACKGROUND: Dizziness is a common symptom which is frequently due to either peripheral or central vestibular dysfunction. However, some patients may lack typical signs suggesting a vestibular or cerebellar lesion and they mostly complain of vertigo or posture imbalance induced by visual stimulation. The symptoms immediately improve either on cessation of the visual input or upon closure of the eyes. Such a presentation is typical of visual vertigo. PATIENTS AND METHODS: From 1993 to 2003, 242 patients were examined for either "vertigo" or "dizziness". The diagnosis of visual vertigo was based…

[Severe vertigo after a scuba-dive to 29 meters].

A 27-year-old flight instructor experienced 5 to 10 minutes after a scuba-dive to 29 m, which lasted totally 50 minutes, dizziness, nausea and severe vertigo. The symptoms lasted about an hour. The patient vomited several times and noted sudden onset headache and vertigo lasting the following three days. Hyperbaric oxygen therapy was started 30 hours after the event because decompression sickness was suspected. Transthoracic echocardiographic evaluation showed a patent foramen ovale. Diving accidents may be caused by decompression sickness, the formation of a free intravascular gas phase (bubb…

Phobic postural vertigo: body sway during vibratory proprioceptive stimulation.

Phobic postural vertigo patients might rely more on proprioceptive than visual cues to regulate stance. We tested 14 phobic postural vertigo patients and 24 healthy subjects with posturography during quiet stance and periods of vibratory proprioceptive calf muscle stimulation, both with eyes open and closed. During quiet stance phobic postural vertigo patients showed higher torque variance than healthy subjects, especially above 0.1 Hz. Vibratory proprioceptive stimulation increased the differences between healthy subjects and phobic postural vertigo patients. The patients were less able to us…

Benign positional paroxysmal vertigo: videonystagmographic study using rotatory test.

Benign Peripheral Paroxysmal Vertigo is a disease of the posterior labyrinth caused by endolymphatic debris, provoking vertigo with some movements of the head. Diagnosis is usually made by finding the positional nystagmus with appropriate manoeuvres. Spontaneous resolution is frequent and in these cases diagnosis is only probable and suspected from anamnesis. Aim of the present investigation was to establish more evaluation parameters in the study of Benign Peripheral Paroxysmal Vertigo. A series of 97 selected patients presenting Benign Peripheral Paroxysmal Vertigo, have been submitted to si…

Treating vertigo with vestibular rehabilitation: results in 155 patients.

Balance is fundamental to our daily activities and the vestibular system, together with vision and proprioceptive functions, are the main structures involved in this process. Dizziness is the main clinical manifestation of malfunction of these systems. The mechanisms of vestibular compensation are one of the most studied aspects since they play an important role in the patient's everyday activities. In this retrospective description of a series of cases the authors present their results in 155 patients that underwent a program of vestibular rehabilitation (VR). The program, first described by

[Central or vestibular vertigo? Diagnostic look through Frenzel glasses].

Vertigo is not a uniform symptom. The basis for a differentiated diagnosis is the history, which should record the frequency and duration of attacks. Further diagnostic investigations serve to differentiate between non-vestibular and vestibular (peripheral) forms of vertigo. Of essential importance is the determination of nystagmus with the aid of Frenzel lenses, with distinction being made between voluntary nystagmus and provoked nystagmus. These orientating examinations, which can be carried out by the family doctor, set the points for the further course. On account of the need for special e…

Spontaneous vertebral artery dissection mimicking acute vertigo. Case report.

A patient with acute vertigo, and normal findings on neurologic examination, was found to have vertebral artery dissection (VAD). This case shows that the clinical picture of VAD can mimic vertigo of labyrinthine (i.e, peripheral) origin.

Clinical features of paroxysmal positional vertigo presenting combined lesions.

Benign paroxysmal positional vertigo (BPPV) is one of the common vestibular disorders. Canalolithiasis is thought to be a likely lesion. A canalith repositioning procedure (CRP by Epley) generally yields good resolution of vertigo and nystagmus. The authors confirmed the efficacy of this procedure on typical BPPV of the posterior semicircular canal type. We designed a new procedure for BPPV of the lateral canal type, which also yielded satisfactory results. BPPV sometimes presents a nystagmus pattern, which suggests multiple lesions. We have seen eight cases of BPPV showing nystagmus that comb…

Superior semicircular canal dehiscence: a new cause of vertigo.

There are many known causes of vertigo, but many cases remain unexplained. Sound-induced, pressure-induced, or positional vertigo caused by bony dehiscence of the superior semicircular canal into the middle cranial fossa is a newly described etiology of vertigo. Three case studies of patients with CT evidence and surgical confirmation of bony dehiscence of the superior semicircular canal with variable presentations are described. The history, symptoms, CT findings, vestibular studies, and method of surgical repair are presented. Two patients had disabling vertigo and one had no vestibular symp…

Recognition and management of horizontal canal benign positional vertigo.

OBJECTIVE: We reviewed the features of nystagmus in 24 patients with horizontal canal benign positional vertigo (BPV). Patients were treated with canalith repositioning maneuvers. Our goal was to develop a framework for distinguishing horizontal from posterior canal BPV and to further develop a mechanistic model explaining the horizontal canal variant of BPV. STUDY DESIGN: The study design was a retrospective case review with prospective treatment outcome and follow-up. SETTING: The study was performed at a secondary and tertiary referral center for vertigo and dizziness. PATIENTS: The diag…

Treatment of horizontal semicircular canal benign paroxysmal positional vertigo.

We evaluated a new therapeutic maneuver-Prolonged Position on the healthy side, for Benign Paroxysmal Positional Vertigo (BPPV) of the horizontal semicircular canal. We devised this type of physical treatment in accordance with the "canalolithiasis" theory of BPPV, in order to try to free the horizontal semicircular canal of otoconial debris. We compared the results obtained by Prolonged Position with two other physical therapies by dividing our horizontal canal BPPV patients into three therapeutic groups: 1) 35 patients treated with Prolonged Position; 2) 24 patients treated with head shaking…

[Benign paroxysmal positional vertigo. Diagnosis, course, physiopathology and treatment].

The benign paroxystic positional vertigo (BPPV) is defined by brief attacks of rotational vertigo and concomitant rotatory-linear nystagmus, triggered by rapid extension and by lateral head tilt toward the affected ear. Cupulolithiasis of the posterior semi-circular canal from one inner ear is responsible of this peculiar type of positional vertigo. Definition diagnostic criteria are based upon the time history of the burst of rotational vertigo associated with the typical positioning nystagmus induced by rapid position changes from the setting to the head hanging right or left position during…

Evaluation of acute vertigo: unusual lesions imitating vestibular neuritis.

The acute onset of vertigo is a common clinical problem presenting to primary care physicians or otologists for evaluation. Usually the underlying disease process is benign and self-limited in nature. In the absence of hearing loss or additional neurologic findings, a common initial diagnosis is vestibular neuritis. The patient is treated symptomatically and observed for spontaneous resolution. However, other more serious disease processes may mimic the presentation of vestibular neuritis and be misdiagnosed. Five cases of serious central nervous system disorders that were similar to vestibula…

Epidemiological studies on benign paroxysmal positional vertigo in Japan.

In 1980, the Research Committee of Peripheral Vestibular Disorders in Japan, which is supported by the Ministry of Health and Welfare, was founded by 24 members, from several districts in Japan. The Committee's first task was to draft the diagnostic criteria for several peripheral vestibular disorders, such as benign paroxysmal positional vertigo (BPPV), and vestibular neuronitis. For the national epidemiological survey, they then collected the data on 101 cases of BPPV out of some 783 vestibular disorder patients. In addition, data on 103 patients of BPPV out of 559 vestibular disorder patien…

Transdermal scopolamine for peripheral vertigo (a double-blind study).

Transdermal scopolamine medication has been evaluated in 30 cases of acute peripheral vertigo. The double blind study revealed favourable effects, e.g. in Menière's disease. The best compromise between effect and side-effects was one active medication patch. The side-effects were those described for scopolamine, such as blurred vision and dryness of the mouth. Transdermal scopolamine seems to offer an alternative form of medication in acute peripheral vertigo.
