Transcanal Transvestibular Endoscopic Neurectomy: First Experience.

OBJECTIVE: Ménière's disease is an idiopathic inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, and persistent tinnitus. In 10% to 30% of cases, conservative therapy fails, and Ménière's disease is defined as intractable. In these patients, ablative techniques with unilateral vestibular deafferentation are mandatory. Several approaches for vestibular neurectomy, which preserve hearing capability, are described. In patients presenting severe dizziness associated with high-grade sensorineural hearing loss, surgical labyrinthectomy, or selected vestibu

Routine Vestibular Tests May Point Out Vestibular Subtype of Seasickness Only.

BACKGROUND: The vestibular system is important in the pathogenesis of seasickness. Our objective is to investigate whether routine vestibular tests detect seasickness.METHODS: Included were 17 professional naval personnel (mean age of 29.76 4.73 yr) diagnosed as having seasickness and 29 healthy age- and gender-matched controls. Cervical (c) vestibular evoked myogenic potentials (VEMP) and ocular (o) VEMP and bithermal caloric tests were performed after ear, nose, and throat examination, pure tone audiometry, and magnetic resonance imaging. Severity of seasickness was evaluated based on the Gr

Vestibular, locomotor, and vestibulo-autonomic research: 50 years of collaboration with Bernard Cohen.

My collaboration on the vestibulo-ocular reflex with Bernard Cohen began in 1972. Until 2017, this collaboration included studies of saccades, quick phases of nystagmus, the introduction of the concept of velocity storage, the relationship of velocity storage to motion sickness, primate and human locomotion, and studies of vasovagal syncope. These studies have elucidated the functioning of the vestibuloocular reflex, the locomotor system, the functioning of the vestibulo-sympathetic reflex, and how blood pressure and heart rate are controlled by the vestibular system. Although it is virtually

Vestibular compensation: the neuro-otologist's best friend.

Why vestibular compensation (VC) after an acute unilateral vestibular loss is the neuro-otologist's best friend is the question at the heart of this paper. The different plasticity mechanisms underlying VC are first reviewed, and the authors present thereafter the dual concept of vestibulo-centric versus distributed learning processes to explain the compensation of deficits resulting from the static versus dynamic vestibular imbalance. The main challenges for the plastic events occurring in the vestibular nuclei (VN) during a post-lesion critical period are neural protection, structural reorga

Vestibular evoked myogenic potentials and motion sickness medications.

OBJECTIVE: Seasickness is a widespread problem among naval crew, and has a major impact on their performance at sea. The three pharmacological agents most commonly employed in the treatment of seasickness are dimenhydrinate, cinnarizine, and scopolamine. At present, the effectiveness of anti-seasickness drugs is tested by a process of "trial and error", while sailing and exposed to sea conditions. A physiological test to evaluate the action of a drug might save crew members long periods of suffering, as well as simplifying the procedure of selecting the appropriate treatment for each individua

Application of Vestibular Spontaneous Response as a Diagnostic Aid for Meniere's Disease.

In this paper, we report on a new method for assisting in Meniere's disease diagnosis. An accurate diagnosis of Meniere's is challenging, and requires an expert opinion after observing several clinical assessments and tests over a period of time. Our proposed method is based on the analysis of the spontaneous and driven ear evoked responses recorded using Electrovestibulography (EVestG). We used the EVestG signals of 35 individuals suspected of Meniere's and 26 age-matched healthy controls, out of which data of 14 patients with Meniere's and 16 healthy controls were used for developing the dia

Is vestibular self-motion perception controlled by the velocity storage? Insights from patients with chronic degeneration of the vestibulo-cerebellum.

BACKGROUND: The rotational vestibulo-ocular reflex (rVOR) generates compensatory eye movements in response to rotational head accelerations. The velocity-storage mechanism (VSM), which is controlled by the vestibulo-cerebellar nodulus and uvula, determines the rVOR time constant. In healthy subjects, it has been suggested that self-motion perception in response to earth-vertical axis rotations depends on the VSM in a similar way as reflexive eye movements. We aimed at further investigating this hypothesis and speculated that if the rVOR and rotational self-motion perception share a common VSM,

[Motion sickness in lower vertebrates: studies under conditions of weightlessness and under land conditions].

The review presents literature data and results of the author's studies with the goal to find out whether lower animals are susceptible to motion sickness. In our studies, fish and amphibians were tested for 2 h and more by using a rotating device (f = 0.24Nz, a(centrifugal) = 0.144 g) and a parallel swing (f = 0.2 Nz, a(horizontal) = 0.059 g). The performed studies did not reveal in 4 fish species and in toads any characteristic reactions of motion sickness (sopite syndrome, prodromal preparatory behavior, vomiting). At the same time, in toads characteristic stress reactions appeared (escape

Estimating distance during self-motion: a role for visual-vestibular interactions.

A fundamental challenge for the visual system is to extract the 3D spatial structure of the environment. When an observer translates without moving the eyes, the retinal speed of a stationary object is related to its distance by a scale factor that depends on the velocity of the observer's self-motion. Here, we aim to test whether the brain uses vestibular cues to self-motion to estimate distance to stationary surfaces in the environment. This relationship was systematically probed using a two-alternative forced-choice task in which distance perceived from monocular image motion during passive

Evaluation of the chemical model of vestibular lesions induced by arsanilate in rats.

Several animal models of vestibular deficits that mimic the human pathology phenotype have previously been developed to correlate the degree of vestibular injury to cognate vestibular deficits in a time-dependent manner. Sodium arsanilate is one of the most commonly used substances for chemical vestibular lesioning, but it is not well described in the literature. In the present study, we used histological and functional approaches to conduct a detailed exploration of the model of vestibular lesions induced by transtympanic injection of sodium arsanilate in rats. The arsanilate-induced damage w

Vestibular sensitivity in vestibular migraine: VEMPs and motion sickness susceptibility.

BACKGROUND: Vestibular migraine (VM) has gained recognition as a distinct clinical entity in recent years. The pathophysiology is unclear. Vestibular evoked myogenic potential (VEMP) is a validated method to test the vestibulocollic reflex and peripheral vestibular hypersensitivity to noise. The aim of our study was to evaluate the vestibular function and sensitivity in a series of VM patients in comparison to ordinary migraine (M) patient and healthy controls. METHODS: Thirty-seven patients diagnosed with VM according to the Neuhauser criteria, 32 migraineurs, and 30 healthy persons underwen

Effects of vestibular training on motion sickness, nystagmus, and subjective vertical.

Pitch head-and-trunk movements during constant velocity rotation are a provocative vestibular stimulus that produces vertigo and nausea. When exposed to this stimulus repeatedly, motion sickness symptoms diminish as the subjects habituate. Acetylleucine is a drug that is used to treat acute vestibular vertigo. In this study, we wanted to ascertain whether this drug (a) lessened motion sickness or delayed habituation; (b) accelerated the recovery following habituation; and (c) whether changes in the subjective vertical accompanied habituation. Twenty subjects were administered acetylleucine or

Bilateral otosyphilis in a patient with HIV infection.

Otosyphilis is a rare but important cause of sensorineural hearing loss and dizziness, because this hearing loss can be reversed by early diagnosis and aggressive treatment. Moreover, HIV may alter the course of otosyphilis and hasten the development of otosyphilis by reducing host cellular immunity. We report the case of a 35-year-old HIV-infected patient with bilateral fluctuating sensorineural hearing loss and bilateral total vestibular loss caused by otosyphilis. We include a discussion of the relationship between otosyphilis and HIV infection.
