A "new pneumonia" is named legionnaires' disease after a gramnegative bacillus - Legionella pneumophila, unknown until 1977. Sofar it has been observed mostly in older male patients, rarely in children and never before in Germany. The legionnaires' disease in our patient, a five and half years old boy with cystic fibrosis caused a severe dyspnea and multilobular pulmonary densifications on the chest X-ray film. The diagnosis was verified by the rise of the titer in the indirect immune fluorescence test and the success of the therapy with erythromycin.

Four patients (aged 5, 7, 18, and 42) suffering from spinal cord involvement due to an aneurysmal bone cyst are reported. In two cases only a preoperative radiological diagnosis was made, one of them by a CT scan. Combined surgical and radiological treatments were done in three cases with a follow-up of 16 years in one patient and 9 years in the other two. In the fourth case, presenting with a C1 lesion, after radical excision and curettage X-ray therapy was not instituted. Pathological and radiological manifestations are discussed. Therapeutic measures are reviewed.

A 4-year-old boy had had progressive central lumbar pain and hamstring spasm. He had a normal lumbar-spine x-ray except for minimal L-5, S1 spondylolysis, but gave an abnormal gallium-67 scan in the region of the low lumbar spine. Eight weeks following intensive antibiotic therapy, confirmation of the diagnosis of disc-space infection was established by roentgenographic studies that demonstrated narrowing of the L 4-5 intervertebral disc space. A technetium-99m diphosphonate bone scan, performed concurrently with the gallium-67 study, was normal.

Twenty-five children treated for ovarian neoplasms at the Royal Hospital for Sick Children, Yorkhill,Glasgow, between 1947 and 1974 are reported. Eight of the patients had malignant tumours and 17 benign. The patients with benign tumours were treated by either salpingo-oophorectomy or oophorectomy. The malignant cases were treated with either surgery alone or in combination with deep X-ray therapy and/or chemotherapy. Two patients died of malignant teratoma, but the remaining 6 with malignant teratoma, dysgerminoma or endodermal sinus are well. The management of these patients is discussed.

We describe the case of a malignant arthritis of the knee. This is the first description with an oesophageal adenocarcinoma being the primary site of the malignancy. Diagnosis of this rare condition can be made by cytological examination of joint fluid or by synovial biopsy. X-ray of the joint can, as in our case, be negative. Case report on the successful gastric ulcer treatment of a patient with primary hyperparathyroidism with the H2-receptor antagonist cimetidine. After four weeks of therapy with 0.8 g/d cimetidine stomal ulcerations were seen by x-rays and endoscopy to be healed. The patient was discharged on a regimen of 400 mg daily of oral cimetidine. Peptic ulcer did not recur after 1/2 year of treatment with this H2-receptor antagonist, although the signs of primary hyperparathyroidism were still present.

Acute dilatation of the colon with signs of systemic toxicity developed in an elderly man with malignant lymphoma. Although barium contrast X-rays taken previously were interpreted as consistent with granulomatous colitis, rectal biopsy had revealed features characteristic of malignant lymphoma, poorly differentiated lymphocytic type. Bone marrow and lymph nodes were also involved. The onset of dilatation was temporally related to therapy with opiates and anticholinergics, and the acute episode was resolved with nasogastric suction, intravenous fluids, albumin, and antibiotics.

A 47-year-old man presented with fever, weight loss and pulmonary consolidations and cavitation in the x-ray of the thorax. The comprehensive diagnostics resulted pulmonary epitholoid cell granulomas, therefore an immunosuppressive therapy was applied on suspicion of sarcoidosis.
