Physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities: a necessity? A prospective descriptive study in 21 patients.

PURPOSE: The authors postulated that physiotherapy as an adjuvant to the surgical treatment of anterior chest wall deformities is only indicated if specific abnormalities can be found that could be corrected by physiotherapy. The purpose of this study is to investigate whether such abnormalities can be found and to evaluate their course during a postoperative period of 18 months. METHODS: Twenty-one patients, 16 with pectus excavatum and 5 with pectus carinatum, were evaluated 6 weeks before and 6 weeks, 6 months, and 18 months after surgical correction. Postural impairments, spinal mobility

Deep inferior epigastric perforator (DIEP) flap for vulvar reconstruction after radical vulvectomy: a less invasive and simple procedure utilizing an abdominal incision wound.

For vulvar reconstruction following radical vulvectomy in a 71-year-old woman with a large vulvar cancer, we applied the deep inferior epigastric perforator flap (DIEP flap), a typical perforator flap, which could be performed by utilizing an abdominal incision wound without producing another surgical scar and had less donor site morbidity because of a minimal sacrifice of muscles. The surgical procedures were less invasive and simple, and morphologically and functionally satisfactory results were obtained: no recurrence of cancer, a well-preserved vulvar morphology with less donor site scarri

Myosteatosis is associated with adiposity, metabolic derangements and mortality in patients with chronic kidney disease.

BACKGROUND/OBJECTIVES: Myosteatosis has been associated with sarcopenia, and increased mortality risk in patients on hemodialysis. We aimed to explore the associations between myosteatosis, as assessed by computed tomography (CT), with demographic parameters, body composition metrics, muscle strength, metabolic parameters and mortality in patients with chronic kidney disease (CKD). SUBJECTS/METHODS: We enrolled 216 patients (age 60.3 ± 10.6 years, 63% men) with CKD stages 3-5. Abdominal CT scans at the third lumbar vertebra (L3) were used to assess body composition. Abdominal obesity was dete

[A Case of T1a Early Gastric Cancer That Metastasized to the Right Femoral Muscles Six Years and Seven Months after Radical Surgery].

A 49-year-old woman presented to our hospital complaining of abdominal distension and right thigh edema 6 years and 7 months after undergoing total gastrectomy for early gastric cancer in December 2008. The histopathological type of the tumor was poorly differentiated adenocarcinoma. The pathological findings led to a diagnosis of T1aN2M0, Stage II A disease. In August 2015, abdominal CT and MRI revealed para-aortic lymph node swelling, ascites, and a tumor on the right femoral muscles. We performed a needle biopsy of the femoral muscle, and the final diagnosis was intramuscular metastasis fro

Voluntary hyperventilation changes recruitment order of parasternal intercostal motor units.

The order of recruitment of single-motor units in parasternal intercostal muscles during inspiration was studied in normal human subjects during quiet breathing and voluntary hyperventilation. Electromyograms were recorded from the second and third intercostal spaces by means of bipolar fine wire electrodes. Flow at the mouth, volume, end-expired CO2, and rib cage and abdominal anterior-posterior diameters were monitored. Single-motor units were identified using criteria of amplitude and shape, and the time of first appearance of each unit in each inspiration was noted. Hyperventilation was pe

Abdominal Wall Leiomyoma: A Case Report.

BACKGROUND: Fibroma or leiomyoma is the most common benign tumor of the female reproductive system, which is usually found in the uterus, but may also occur in other places, such as the ovary, the broad ligament, and in rare cases in the abdominal wall. The formation of the abdominal wall leiomyoma may result from the implantation of myometrium tissue following surgical removal of the uterine leiomyoma, but sometimes these masses occur in a person who has no history of myomectomy. CASE PRESENTATION: This case was a patient who became a candidate for laparoscopy due to abnormal uterine bleedin

Interaction of tracheobronchial cough and expiration reflex in the cat.

Cough and expiration reflex (ER) are two fundamental airway defense mechanisms traditionally considered distinct motor behaviors. This study examined their temporal and functional interaction using an anesthetized cat model to elucidate the integrative neural mechanisms that coordinate airway protection. Electromyographic recordings of diaphragm and abdominal muscles were obtained during mechanically induced tracheobronchial cough and laryngeal ER. The ER was evoked in three specific phases of the cough cycle: inspiratory, active expiratory phase, and passive expiratory phase. While ER provoca…

Purulent infectious myositis of deep core muscles, a MRSA infection in India.

BACKGROUND: Purulent Infectious Myositis is a common bacterial infection in peripheral limb muscles, but its involvement in deep core muscles is rare. Staphylococcus aureus is one of the most common organisms causing purulent infectious myositis. With an increase in antibiotic resistance in India, many cases are now being reported caused by Methicillin-resistant Staphylococcus aureus (MRSA). CASE PRESENTATION: A male farmer in his early 50 s presented with lower back pain, diffuse abdominal pain, significant weight loss, fatigue for one month, and high-grade fever for one week. Clinical examin

Morphological and Functional Changes of Core Muscles in Postpartum Women With Low Back Pain Based on Musculoskeletal Ultrasound.

INTRODUCTION: Postpartum low back pain (PLBP) is a prevalent condition linked to core muscle dysfunction. This study aimed to evaluate morph … groups (p < 0.05), though contraction capacity only differed from non-pregnant controls (p < 0.05). Abdominal muscles (internal oblique, upper rectus abdominis, transversus abdominis) exhibited significantly decreased thickness and relaxation capacity compared to non-pregnant controls (p < 0.05), but not versus postpartum non-PLBP group (p > 0.05). Core stability w …

Quality Matters as Much as Quantity of Skeletal Muscle: Clinical Implications of Myosteatosis in Cardiometabolic Health.

Although age-related changes in skeletal muscles are closely associated with decreases in muscle strength and functional decline, their asso … omography (CT) images have been introduced and utilized in many studies, as the mass and quality of abdominal muscles could be measured opportunistically on abdominal CT scans obtained during routine clinical care. Yet, the mechanisms by which myosteatosis affect metabolic and cardiovascular health remain largely unknown. In this review, we explore the recent advan …

Muscle-specific knockout of PKC-lambda impairs glucose transport and induces metabolic and diabetic syndromes.

Obesity, the metabolic syndrome, and type 2 diabetes mellitus (T2DM) are major global health problems. Insulin resistance is frequently present in these disorders, but the causes and effects of such resistance are unknown. Here, we generated mice with muscle-specific knockout of the major murine atypical PKC (aPKC), PKC-lambda, a postulated mediator for insulin-stimulated glucose transport. Glucose transport and translocation of glucose transporter 4 (GLUT4) to the plasma membrane were diminished in muscles of both homozygous and heterozygous PKC-lambda knockout mice and were accompanied by sy

Features of trunk muscle wasting during acute care and physical function recovery with aortic disease.

BACKGROUND: Low skeletal muscle area or density, such as myosteatosis, identified on computed tomography (CT) is associated with poor prognosis in patients with cardiovascular diseases. However, there is a lack of evidence regarding the clinical process of skeletal muscle decline as a short-term change during acute care settings. This study focused on the use of routine CT imaging for aortic disease management and investigated the changes in skeletal muscle before and after acute care. METHODS: This prospective study included 123 patients who underwent abdominal CT before and after acute care

Prevalence of Sarcopenia in Children Undergoing Total Pancreatectomy With Islet Autotransplantation (TPIAT) and Implications for Short-Term Surgical Outcomes.

INTRODUCTION: Sarcopenia, the loss of muscle mass and function, is a measure of frailty that is correlated with perioperative morbidity in patients with chronic illness and transplant. We sought to understand the prevalence of sarcopenia in our pediatric population undergoing total pancreatectomy with islet cell autotransplantation (TPIAT) and how sarcopenia impacts short-term postsurgical outcomes. METHODS: This was a single-center, IRB-approved study of children (1-18.99 y) who underwent TPIAT at our institution. Patients were identified from a prospective registry where clinical data (pain
