Enhancing cancer therapy via acoustics: chemotherapy-enhanced tunable acoustofluidic permeabilization (ChemoTAP).

Mechano-chemo cancer treatment is an emerging therapeutic strategy that enhances chemotherapy efficacy by combining chemical agents with mechanical forces to improve drug uptake and overcome resistance. However, current approaches for delivering mechanical forces, including magnetic stress, hydrodynamic shear, and ultrasonic cavitation, suffer from limited tunability, poor spatial precision, and off-target effects, restricting their clinical potential. Here, we introduce ChemoTAP (chemotherapy-enhanced tunable acoustofluidic permeabilization), an acoustofluidic system that utilizes standing su

Recording the human brainstem frequency-following-response in the free-field.

BACKGROUND: The human auditory brainstem frequency-following response (FFR) is an objective measure used to investigate the brainstem's encoding ability of sounds. Traditionally, FFRs are recorded under close-field conditions (earphones), but free-field stimulations (loudspeaker) have yet to be attempted, which would increase the applications of FFRs by making this technique accessible to those who cannot wear inserted transducers. Here we test the feasibility and reliability of measuring speech ABRs across free and close-field. NEW METHOD: The FFR was evoked by a 40-ms consonant-vowel (cv) /

Comparison of vestibular autorotation and caloric testing.

The two most common stimuli of the vestibular system for diagnostic purposes are caloric and rotational head movements. Caloric stimulation, by delivering thermal energy to the lateral semicircular canal, is a well-studied method of vestibular testing, and its clinical usefulness has been established. Vestibular autorotation testing uses high-frequency (2 to 6 Hz), active head movements to stimulate the horizontal and vertical vestibulo-ocular reflex to produce measurable eye movements that can be used to calculate gain and phase. We compared the alternate bilateral bithermal caloric results w

PSpice modeling of cervical and site-focused vagus nerve ultrasonic stimulation for reduced tumor necrosis factor-α production.

Clinical ultrasound is widely used as a diagnostic and therapeutic tool. Recently, it has been used to perform neuromodulation to treat diverse effects, including inflammation reduction through the vagus nerve. Although the mechanism by which ultrasound propagates through tissue for diagnostic purposes has been established, there is not a complete understanding of how it interacts with neurons to elicit excitation and inhibit inflammation. This work presents a novel technique based on a well-established electrical engineering tool, PSpice, to model cervical and site-focused vagus nerve ultraso

Ultrasound stimulation for non-invasive visual prostheses.

Globally, it is estimated there are more than 2.2 billion visually impaired people. Visual diseases such as retinitis pigmentosa, age-related macular degeneration, glaucoma, and optic neuritis can cause irreversible profound vision loss. Many groups have investigated different approaches such as microelectronic prostheses, optogenetics, stem cell therapy, and gene therapy to restore vision. However, these methods have some limitations such as invasive implantation surgery and unknown long-term risk of genetic manipulation. In addition to the safety of ultrasound as a medical imaging modality,

Application of auditory mismatch negativity in tinnitus patients based on high-resolution electroencephalogram signals.

OBJECTIVE: The purpose of this study was to investigate the significance of mismatch negativity (MMN) by comparing high-resolution electroencephalogram signals from tinnitus patients and healthy controls. METHODS: The study included eight subjects with chronic subjective idiopathic tinnitus and seven healthy controls. Participants with clinical speech (512-2,000 Hz) hearing thresholds less than 25 dB HL and with negative Hospital Anxiety and Depression Scale scores were included in the study. The E-Prime 2.0 software and a 256-electrode EGI Net Station system were used to evoke and record the

Implementation of a direct install 3-pole type EM transducer in round window niche for implantable middle ear hearing aids.

Since the 1980's, various types of implantable hearing aids using unique means for delivering acoustic power to the inner ear have been developed. Recently, implantable hearing aids that stimulate the round window by the middle ear transducer have received great attention because it reduces loading effect at the ossicular chain. In this study, we have implemented a direct install 3-pole type EM transducer in round window niche for implantable middle ear hearing aid. The 3-pole type EM transducer consists of two permanent magnets and three coils and exhibit structural features that minimize lea

Interaural Place-of-Stimulation Mismatch Estimates Using CT Scans and Binaural Perception, But Not Pitch, Are Consistent in Cochlear-Implant Users.

Bilateral cochlear implants (BI-CIs) or a CI for single-sided deafness (SSD-CI; one normally functioning acoustic ear) can partially restore spatial-hearing abilities, including sound localization and speech understanding in noise. For these populations, however, interaural place-of-stimulation mismatch can occur and thus diminish binaural sensitivity that relies on interaurally frequency-matched neurons. This study examined whether plasticity-reorganization of central neural pathways over time-can compensate for peripheral interaural place mismatch. We hypothesized differential plasticity acr

Evaluation of a Photopolymerized Gelatin Hydrogel Network with bFGF-Loaded Alginate Microspheres for Tympanic Membrane Perforation Repair in Rats.

Tympanic membrane perforation (TMP) often leads to hearing loss and requires effective repair strategies. However, existing surgical options are invasive and lack ideal biomaterials for scaffold-based healing. Herein, we present a custom-engineered mechano-acoustic responsive hydrogel incorporating bFGF-loaded sodium alginate microspheres, designed for controlled drug release and tissue regeneration under dual stimulation: vibrational simulation and low-frequency acoustic waves (generated by radio devices) - representing its first transition from in vitro characterization to in vivo regenerati

A novel transcranial MR Guided focused ultrasound method based on the ultrashort echo time skull acoustic model and phase retrieval techniques.

Transcranial ultrasound stimulation (TUS) has been clinically applied as a neuromodulation tool. Particularly, the phase array ultrasound can be applied in TUS to non-invasively focus on the cortex or deep brain. However, the vital phase distortion of the ultrasound induced by the skull limits its clinical application. In the current study, we aimed to develop a hybrid method that combines the ultrashort echo time (UTE) magnetic resonance imaging (MRI) sequences with the prDeep technique to achieve focusing ventral intermediate thalamic nucleus (VIM). The time-reversal (TR) approach of the UTE

Temporal quantization deteriorates the discrimination of interaural time differences.

Cochlear implants (CIs) often replace acoustic temporal fine structure by a fixed-rate pulse train. If the pulse timing is arbitrary (that is, not based on the phase information of the acoustic signal), temporal information is quantized by the pulse period. This temporal quantization is probably imperceptible with current clinical devices. However, it could result in large temporal jitter for strategies that aim to improve bilateral and bimodal CI users' perception of interaural time differences (ITDs), such as envelope enhancement. In an experiment with 16 normal-hearing listeners, it is show

Electrical Stimulation of the Ear, Head, Cranial Nerve, or Cortex for the Treatment of Tinnitus: A Scoping Review.

Tinnitus is defined as the perception of sound in the absence of an external source. It is often associated with hearing loss and is thought to result from abnormal neural activity at some point or points in the auditory pathway, which is incorrectly interpreted by the brain as an actual sound. Neurostimulation therapies therefore, which interfere on some level with that abnormal activity, are a logical approach to treatment. For tinnitus, where the pathological neuronal activity might be associated with auditory and other areas of the brain, interventions using electromagnetic, electrical, or

Auditory brainstem implant: I. Issues in surgical implantation.

Most patients with neurofibromatosis type 2 (NF2) are totally deaf after removal of their bilateral acoustic neuromas. Twenty-five patients with neurofibromatosis type 2 have been implanted with a brainstem electrode during surgery to remove an acoustic neuroma. The electrode is positioned in the lateral recess of the fourth ventricle, adjacent to the cochlear nuclei. The present electrode consists of three platinum plates mounted on a Dacron mesh backing, a design that has been demonstrated to be biocompatible and positionally stable in an animal model. Correct electrode placement depends on

[Inverted acoustic reflex: an analysis of its morphological characteristics in different physiological and pathological situations].

INTRODUCTION AND OBJECTIVES: Inverted acoustic reflex (IAR) or inverted acoustic effect is the name given to the positive deflexion observed during the carrying out of an admittancemetry when the ear is subjected to a high intensity sound, rather than the typical negative deflexion due to the contraction of the middle ear musculature. The objective of the paper is to analyze the morphology of the IAR by relating it to the situations (physiological or pathological) in which this effect appears with the aim of establishing a pathogenic hypothesis. METHODS: We study the admittance traces obtaine

A Novel Water Method for Reducing Air Conduction in Soft Tissue Conduction.

Background: Bone vibrator (BV) stimulation applied to skin sites on the body elicits hearing by soft tissue conduction (STC). However, BV stimulation to sites far from the ear requires the delivery of higher-intensity stimulus vibrations to achieve threshold, which can then induce hearing by air conduction (AC) contamination. This problem limits the study of STC thresholds at sites more distant from the ear. Objective: To overcome this problem, we evaluated the possibility of delivering STC vibratory stimuli to body sites in a water bath, based on the different acoustic impedances between air

Comparison of Blood Pressure Modulation by Low-Intensity Focused Ultrasound Stimulation of Carotid Sinus, Nodose Ganglion, and Vagus Nerve.

Low-intensity focused ultrasound stimulation (FUS) of the vagus nerve has proven effective in lowering blood pressure (BP) in our pilot animal study, opening new possibilities for noninvasive physiotherapy for hypertension. To further explore the impact of BP regulation by FUS on other targets, we compared the BP modulation pattern when applying FUS to the left carotid sinus, nodose ganglion, and vagus nerve trunk, respectively. FUS with different acoustic intensities was applied to each target, while continuous BP waveforms were synchronously recorded in the right common carotid artery. Subse

[Effect of contralateral stimulation on otoacoustic evoked emissions in acute deafness].

23 patients with unilateral hearing loss and presence of TEOAE on both ears were examined with regard to the association between contralateral acoustic stimulation and evoked otoacoustic emissions. Contralateral white noise of 40 dB HL, 50 dB HL and 60 dB HL decreases the amplitude of TEOAE both at the ear with and without hearing loss. In comparison with a group of subjects with normal hearing, the decrease in the amplitude of TEOAE was less in the group of patients with hearing loss. Two paradoxical cases were found with an increase in the TEOAE amplitude in the ear with hearing loss during …

Dose-dependent ultrasound neuromodulation of the human brain.

Transcranial ultrasound stimulation (TUS) can noninvasively modulate deep brain structures, but the relationship between delivered dose and neuromodulatory response remains poorly characterized in humans. Here, we show that ultrasound-induced tissue displacement measured in vivo predicts local and downstream functional neuromodulation. In 24 healthy adults, we combined magnetic resonance acoustic radiation force imaging (MR-ARFI) with concurrent TUS-fMRI in a double-blind, counterbalanced, within-subject design. Relative to active control stimulation, targeting the lateral geniculate nucleus (
