Management of the small acoustic neuroma: a decision analysis.

Imaging techniques such as magnetic resonance imaging (MRI) with gadolinium may detect small acoustic neuromas before significant symptoms have developed. In this situation, the patient and surgeon may question whether the tumor requires treatment, especially if the tumor was discovered incidentally. A computerized clinical decision analysis was undertaken to model the decision to observe such tumors or proceed to immediate surgery. The decision tree was constructed with flexibility to account for tumor size, hearing level, and other critical variables such as the individual's aversion to unil

Histopathology and growth pattern of cystic acoustic neuromas.

In a series of 571 acoustic neuromas, 23 were identified as cystic tumors. An immunohistochemical study including nervous system-associated protein (S-100), glial fibrillary acidic protein (GFAP), neurofilament protein (NF), laminin, fibronectin, vimentin, factor VIII (von Willebrand factor)-related antigen, and the nuclear antibody Ki-67 was performed. The cyst walls contained numerous S-100-positive fibrils distributed throughout the whole extent of the walls. Laminin, fibronectin, and vimentin were predominantly located around the vessel walls, factor VIII was found inside the vessels. GFAP

[Functional results of the surgery of unilateral acoustic neuroma].

The authors sent a circumstantial questionnaire to 224 surviving patients out of a 228 unilateral acoustic neurinoma operated on from June 83 to December 90 range of patients in order to assess their complaints. Seventy-two per cent of these neurinomas were Stade III or IV of Koos. Translabyrhintine approach was used for 85% and suprapetrous approach for 15%. The post-operative mortality rate is 1.75%. At the end of the procedure, the removal seemed total in 99% of cases and the anatomical facial nerve continuity was preserved in 94% of cases. Our patients kept or recovered a normal (Grade I o

The assessment of hypertrophy of nasopharyngeal tonsil by acoustical methods.

The possibilities for using acoustical analysis as a noninvasive measure of the degree of obstruction created by an enlarged pharyngeal tonsil (adenoid) are discussed. The method capitalizes on the relationship which exists between a specific obstruction of the respiratory tract and the resulting respiratory sound. Results indicate that as the adenoid increases in size relative to the size of the nasopharynx, the dominant components of the respiratory sound spectrum shift towards a higher frequency range. Furthermore, the correlation found between the assessments of the obstruction due to vari

Management of hydrocephalus resulting from acoustic neuromas.

Patients with large acoustic neuromas may have secondary obstructive hydrocephalus and occasionally significant neurologic deficit develops. At the House Ear Clinic, we have managed patients with hydrocephalus by translabyrinthine tumor removal without preoperative ventriculoperitoneal shunting. Forty-three patients with documented hydrocephalus who underwent acoustic neuroma removal have been reviewed. Six patients had neurologic deficit resulting from raised intracranial pressure before surgery. In each of these six cases, the deficit resolved after tumor removed without requiring shunting.

Comparison of the acoustic properties of six popular stethoscopes.

This study evaluated stethoscope acoustics by using a sound frequency generator and an active artificial ear. Six popular, currently available stethoscopes were compared in their various modes involving bells, diaphragms, etc.: Littmann Classic II, Littmann Cardiology II, Littmann Master Cardiology, Hewlett-Packard Rappaport-Sprague, Tycos Harvey Triple Head, and Allen Medical Series 5A RPS Binaural. The transfer function was measured from 37.5-1000 Hz, the range where nearly all heart and lung sounds are found. Sound in the low-frequency range (37.5-112.5 Hz) was in most cases amplified by th…

Presurgical electrophysiological findings in acoustic nerve tumours.

The clinical involvement of the facial nerve is a rare finding among the initial symptoms of acoustic neurinomas. However, compression of the facial nerve is a common intraoperative finding. Blink reflex was recorded in 20 patients affected by cerebellar-pontine angle tumor confirmed at surgery. Recordings were also made of the M-response of the facial nerve from the naso-labial folds. In 6 cases jaw reflex was also recorded. In summary, these electrophysiological studies revealed a facial nerve damage in 13 and a trigeminal nerve dysfunction in 2 out of 18 clinically unaffected patients. The

Electronystagmographic findings in 147 patients with acoustic neuroma.

We report on the preoperative findings in 147 patients with surgically confirmed acoustic neuroma (AN). Gaze nystagmus, spontaneous nystagmus and positional nystagmus were present in 23.1%, 47.5% and 63.0% of the patient, respectively. Pathological findings in ETT, OKP and bithermal caloric test were found in 44.3%, 45.3% and 77.0%, respectively. The ratio of pathological findings in ETT and OKP was larger in the large tumor group than in the small tumor group. Gaze nystagmus to both directions and direction-changing positional nystagmus were also found in the large tumor group. While the corr

[Acoustic impedance measurement in the diagnosis of hypoacusis in children].

In 1986-1988, we examined 1105 children no older than 15 years with various forms of hypoacusis. It was found that measurement of acoustic impedance can help topical diagnosis at any age, especially in children younger than 3 years. In 1988, we examined 350 children with conductive hypoacusis. The specific features of tympanography and acoustic reflexes allow early and accurate diagnosis of middle ear abnormalities. It is stressed that measurement of acoustic impedance may play an important role in examinations of patients with middle ear pathologies.

[Evoked acoustic oto-emissions: effects of atropine].

The Evoked Otoacoustic Emissions (EOE) are a sinusoidal wave complex coming from the contractile properties of the cochlea and particularly of the outer hair cells. This activity is influenced by the centrifugal pathways whose transmitter is acetylcholine. In order to study the effects of the functional block of the cochlea, ten young normal subjects were examined recording the EOE before and ten mins after the i.v. infusion of 1 mg of atropine. The statistical analysis of the results has shown a significant decrease only in EOE appearance threshold in absence of modifications of the saturatio

The translabyrinthine approach for the removal of large acoustic neuromas.

The results and the sequelae are reported from a series of consecutive 400 translabyrinthine removals of acoustic neuromas comprising almost all such tumors in Denmark during a 12-year period from 1976 to 1988. There were two intrameatal, 137 median-sized, 112 large and 149 advanced tumors, the latter having a diameter exceeding 40 mm. The mortality rate in the overall series was 2%. The incidence of cerebrospinal fluid leak occurred in 11%, and persisted for more than 2 weeks in 4%. Facial nerve function was completely normal in 67% of the patients, while only 5% required a nerve anastomosis.

Respiratory acoustic impedance in left ventricular failure.

The measurement of respiratory acoustic impedance (Zrs) by forced pseudorandom noise provides a simple means of assessing respiratory mechanics in nonintubated intensive care patients. To characterize the lung mechanical alterations induced by acute vascular congestion of the lung, Zrs was measured in 14 spontaneously breathing patients hospitalized for acute left ventricular failure. The Zrs data in the cardiac patients were compared with those of 48 semirecumbent normal subjects and those of 23 sitting asthmatic patients during allergen-induced bronchospasm. In the patients with acute left v

[Tumors of the cerebellopontile angle, with the exception of acoustic neuromas].

Based on a review of 38 cases of tumors of pontocerebellar angle, excluding acoustic neurinoma, and a literature review, clinical, radiologic and therapeutic characteristics of these lesions are described. Diagnosis depends on accurate pre-operative identification of the type of tumor. Based on frequency data it can be estimated that tumors of pontocerebellar angle are nearly always acoustic neurinomas : this infers that these other tumors of the angle represent a differential diagnosis where major orientation signs are radiologic. This is fundamental, since when considering all tumors of pont

The caloric response in the contralateral ear in acoustic neuroma.

The caloric test response from the unaffected ear in a case of acoustic neuroma may be normal, hypoactive or hyperactive; each has a different pathophysiological connotation: a normal response is the anticipated finding; a hyperactive contralateral response is due to the presence of a large neuroma with brainstem compression; and a hypoactive response may result from several factors, such as lack of mental alertness during the test, drugs like barbiturates, raised intracranial tension, or the presence of bilateral neuromas.
