Absolute Risk of Adverse Obstetric Outcomes Among Twin Pregnancies After In Vitro Fertilization by Maternal Age.

IMPORTANCE: Twin pregnancy is a common occurrence in pregnancies conceived with in vitro fertilization (IVF), but the absolute risk of adver … al age in each subgroup. CONCLUSIONS AND RELEVANCE: In this cohort study, twin pregnancy, IVF, and advanced maternal age were independently associated with adverse obstetric outcomes. Given these findings, promotion of the elective single embryo transfer strategy is needed to reduce multiple pregnancies following IVF technologies. Unnecessary cesarean delivery shouldh …

A Novel Paradigm for Non-Invasive Prenatal Genetic Screening: Trophoblast Retrieval and Isolation from the Cervix (TRIC).

As the prevalence of pregnancies with advanced maternal age increases, the risk of fetal chromosomal abnormalities is on the rise. Therefore, prenatal genetic screening and diagnosis have become essential elements in contemporary obstetrical care. Trophoblast retrieval and isolation from the cervix (TRIC) is a non-invasive procedure that can be utilized for prenatal genetic diagnosis. The method involves the isolation of fetal cells (extravillous trophoblasts) by transcervical sampling; along with its non-invasiveness, TRIC exhibits many other advantages such as its usefulness in early pregnan…

Does an ideal prototype of a twin mother exist?

Objective To evaluate the association of a set (or the lack thereof) of pre-gravid maternal features with a better outcome in terms of gestational age and birth weight of twins. Methods Analysis of a Slovenian population-based cohort of all dichorionic twin pairs born at ≥22 weeks during an 11-year period from 2002 to 2013. The independent variables included tall stature (≥170 cm), multiparity, and normal pregravid body mass index (BMI). The outcome variables included gestational age and total twin birth weight. Results We studied 3232 dichorionic twins. Being "tall", multiparous, and with a n

[Emergency cervical cerclage during mid-trimester of pregnancy: Experience of Clermont-Ferrand].

OBJECTIVES: The objective of this study was to identify factors associated with favourable perinatal outcome after emergency cervical cerclage during mid-trimester of pregnancy. PATIENTS AND METHODS: This is a retrospective study of all cases who underwent emergency cervical cerclage between 16 to 28 weeks of gestation (WG) over a period of 16 years in a University Hospital. RESULTS: Among the 32 cases, the postnatal survival rate (day 28) was 80%. Delivery occurred at a mean gestational age of 33.1 WG [18-41.3 WG] and after 37 WG in 39% of cases. The perinatal outcome was improved by absenc

Neonatal outcomes after elective delivery management of preterm premature rupture of the membranes before 34 weeks' gestation (DOMINOS study).

OBJECTIVE: The objective was to compare the impact of the reason for delivery (elective delivery versus spontaneous onset of labor) on neonatal outcome after preterm premature rupture of the membranes (PPROMs). STUDY DESIGN: A regional prospective cohort study of all women with PPROM at between 24 and 34 weeks' gestation was conducted. We compared the effects of elective delivery (n=133), spontaneous labor (n=170), and admission for medical complications (n=169) like fetal distress, maternal hyperthermia, or placental abruption, on neonatal outcomes. Primary outcome measures were a composite

First trimester chorionic villus sampling versus mid-trimester genetic amniocentesis--preliminary results of a controlled prospective trial.

This controlled prospective study assesses the relative risks of first trimester chorionic villus sampling (CVS) versus mid-trimester gentic amniocentesis (GA). CVS subjects and amniocentesis controls were comparable with regard to several confounding variables which might influence the risk of pregnancy loss including maternal age, smoking, alcohol consumption, gestational age at study entry, and history of vaginal bleeding or poor prior reproductive outcome. The most common indication for prenatal diagnosis was advanced maternal age (n = 511). In this subgroup, spontaneous abortion (less tha…

Effect of undernutrition on morphine analgesia, haloperidol catalepsy and pentobarbitone sodium hypnosis in developing new born rats.

In the present study, half of the pups of a litter were undernourished by 12 h daily maternal deprivation from day 5 to day 18 postnatal and were subsequently nutritionally rehabilitated. Responses of CNS-acting drugs (morphine analgesia, pentobarbitone sodium hypnosis, haloperidol catalepsy) were studied at the age of day 9, 12 and 18 in maternally deprived and of day 25 in nutritionally rehabilitated new born rats as compared to that of their nourished littermates. The results showed that the response of these CNS-acting drugs was maximum at the age of day 9 postnatal and progressively decre

Maternal age in the epidemiology of common autosomal trisomies.

The birth prevalence rate of each common autosomal trisomy generally increases with advancing maternal age and there is a substantial fetal loss rate between late first trimester and term. The literature is reviewed in order to provide the best estimates of these rates, taking account where possible of biases due to prenatal diagnosis and selective termination of pregnancy. There is an almost exponential increase in Down syndrome birth prevalence between ages 15 and 45 but at older ages the curve flattens. There is no evidence of the claimed relatively high birth prevalence at extremely low ag

The influence of maternal age on the outcomes of pregnancies complicated by bleeding at less than 12 weeks.

The effect of maternal age on the outcome of threatened miscarriage after ultrasound has confirmed fetal heart (FH) pulsation was assessed. At a university teaching hospital, 138 women presenting with bleeding before 12 weeks' gestation were followed up until delivery or pregnancy loss. Those with multiple or ectopic pregnancy, social termination of pregnancy, assisted conception and temporary residence were excluded. Outcome measures were pregnancy loss, fetal abnormalities, preterm delivery, low birthweight and cesarean delivery. Age over 35 years was significantly associated with reduced li

Associations of Maternal Nutritional Status and Supplementation with Fetal, Newborn, and Infant Outcomes in Low-Income and Middle-Income Settings: An Overview of Reviews.

BACKGROUND/OBJECTIVES: Despite advances in maternal nutritional knowledge, the effect of maternal diet, micronutrient status and undernutrition, and the effect of maternal supplementation on fetal, neonatal and infant outcomes still have gaps in the literature. This overview of reviews is intended to assess the available information on these issues and identify the main maternal nutritional factors associated with offspring outcomes in low- and middle-income countries as possible targets for public health interventions. METHODS: The literature search was performed in Medline (PubMed) and Coch

The role of temperament by family environment interactions in child maladjustment.

In order to advance our understanding of the etiology of individual differences in child maladjustment (i.e., conduct and emotional problems), we tested hypotheses about the statistical interactions between child temperament and two aspects of the family environment: maternal negativity and positivity, and household chaos (e.g., crowding, noise, lack of routines). Mothers (n = 149) reported on their child's effortful control, negative affect, surgency, and behavioral/emotional problems. The age range of the children was 3 to 7 years old and half of the sample was girls. Observers rated materna

Endocrine disorders in pregnancy: physiological and hormonal aspects of pregnancy.

The endocrinology of pregnancy involves endocrine and metabolic changes as a consequence of physiological alterations at the foetoplacental boundary between mother and foetus. The vast changes in maternal hormones and their binding proteins complicate assessment of the normal level of most hormones during gestation. The neuroendocrine events and their timing in the placental, foetal and maternal compartments are critical for initiation and maintenance of pregnancy, for foetal growth and development, and for parturition. As pregnancy advances, the relative number of trophoblasts increase and th

Neither Metabolic Dysfunction Associated Steatotic Liver Disease Presence Nor Fibrosis Severity, By FIB4, Predicts Adverse Pregnancy Outcomes.

BACKGROUND: Metabolic dysfunction associated steatotic liver disease (MASLD) has been associated with adverse pregnancy outcomes. Data are limited with regard to whether this association is solely related to MASLD versus comorbid metabolic conditions. This study aims to evaluate the independent contribution of MASLD to maternal and fetal outcomes. METHODS: This single center retrospective study included adult pregnant patients who received care between 2000 and 2021. Patients with MASLD before pregnancy were age-matched with controls without chronic liver disease. Baseline FIB-4 score, within

The influence of biological, social, and developmental factors on language acquisition in pre-term born children.

The objective of this study is to evaluate language outcome in pre-term children, considering multiple factors. The hypothesis is that early communicative capacity (pre-verbal communicative utterances) is affected mainly by biological (prematurity, birth weight, and gender) and social factors (maternal education), while more advanced linguistic abilities (i.e., combinatorial and syntactic abilities) are mostly influenced by previously acquired communicative abilities. Eighteen monolingual Italian pre-term children (birth weight between 750 and 1600 grams, gestational age <37 weeks; 13 males an

[Seizures caused by subarachnoid haemorrhage in a pregnant woman].

This case report describes a pregnant woman of gestational week 37 + 2 days who was admitted to the hospital with first-time seizures. The patient was stabilized, and an acute caesarian section was performed due to the possible aetiology of eclampsia and the advanced gestational age. Because of the atypical clinical history and normal maternal blood samples a computed tomography of the cerebrum was performed demonstrating a subarachnoid haemorrhage. A computed tomography-angiography revealed an aneurism at the anterior communicating artery. The aneurism was coiled the following day to reduce t
