Six cases of alcoholic myopathy (two acute and four chronic) are reported. Histologically, cases of acute alcoholic myopathy showed dystrophic changes of the sarcoplasm (especially in the muscles of lower extremities), ending with total rhabdomyolysis. In chronic cases, histology revealed selective atrophy of muscle fibers of the 2nd type. Both direct and indirect effects of alcohol are discussed in the pathogenesis as well as the metabolic disturbances caused by acetaldehyde, a metabolic product of ethanol. Although the correlation between alcohol intake and heart disease has been known for a century, the information tends to be dominated by historical and anecdotal reports. The more recent literature shows that alcoholic damage leads to metabolic and structural changes in all tissues of the heart, with the result that the contractile element, coronary vessel or specific conduction tissue may fail. The resultant symptoms are heart failure, angina pectoris and arrhythmias. These correlations are discussed.

Acute alcohol ingestion can lead to alterations of either mechanical function or electrophysiologic properties of the heart, whereas chronic consumption can lead to progressive cardiac dysfunction and congestive cardiomyopathy. On the other hand, alcohol appears to have a protective effect for coronary artery disease when consumed in low amounts, although prophylactic use of alcohol is not recommended.

The physician-patient relationship plays a major part in the management. The consultations should be extended and fruitful for the patient, but the physician should also provide pharmacodynamic help and to achieve this tiapride is of value. Tiapride is effective against disorders of sleep, of affect and of behaviour. Twenty patients were given 300 mg tiapride daily (1 tablet three times a day) for one month. Results were as follows: in disturbances of sleep: 83,3% good results; in disorders of affect: 79% good results; in behavioural disorders: 77% good results; in alcoholic tremor: 100% good

It is well established that alcoholics have a very high suicide rate. The evidence is of two kinds: a) Follow up studies of alcoholics consistently reveal high suicide rates. The proportion dying by suicide varies from 8% (Kessel & Grossman, 1961) to 21% (Gabriel, 1935) depending on the length of follow up.
