To trim or replant: a matter of cost.

Many factors are involved in the decision whether to complete the amputation of an injured digit and trim the stump or to replant the amputated member. This paper compares the cost of these two treatments so that this financial aspect might be included among the many factors which influence the surgeon's decision. The cost of these treatments was separated into two aspects. First, the overall cost of the medical and surgical expenses was calculated. Second, the cost which the Israeli social security granted the injured worker due to the disability was calculated. By comparing the total surgica

[Sculptor of The Cripple of the Geneva Museum of Art and History. An ancient Greek portrayal of hemimelia?].

An archaic Greek terracotta vase in the Art and History Museum at Geneva depicts a man deprived of his left arm and with two legs ending in a stump below the knees. Did he suffer from a traumatism (amputation), a mutilating disease or congenital malformation (hemimelia)? A survey of written and iconographic sources throws light on the methods and limits of ancient surgery, and on the chances of survival of abnormal children in archaic and classical Greece.

[Indications for replantation of lower limbs after their traumatic amputation at the shin level].

From analysis of the results of replantation of 37 lower limbs at the level of the leg, which were amputated as the result of injury in 33 patients, the authors determine the tactical approach to the choice of indications for replantation. The authors believe that replantation at the level of the leg is always indicated in children if it is executable technically and the patient's somatic status presents no general contraindications. In other cases the formation of a stump and subsequent prosthetics should be considered more advisable.

A device to facilitate donning and cleaning of a roll-on socket in a trans-tibial amputee with a non-functional arm.

Rehabilitation of a person with a non-functional arm and a trans-tibial amputation with a short stump with fragile skin, multiple scars and a limited knee function presents a considerable problem. The best prosthesis was considered to be one with a roll-on socket. Donning and cleaning of the socket however could not be done by the patient himself and it was therefore necessary to develop a device to permit the patient to perform these tasks independently. A case history is presented and the new device described.

Rudimentary polydactyly: report of five cases.

Rudimentary polydactyly was found in five patients. Surgical exploration of the papules revealed findings that have not been previously published. A cord-like structure arises from the bottom of the papule, with its proximal portion joining the neurovascular bundle of the digit. Histologic examination of the cord showed strong similarity to the neurovascular bundle of a finger. From these findings, we assume that rudimentary polydactyly may represent the amputation stump of pedunculated fingers.

Polymicrobial infection presenting as non-clostridial gas gangrene in a non-diabetic trauma patient. A case report and review of literature.

BACKGROUND: Clostridium species are known to be the primary causative organism of gas gangrene. Non-clostridial gas gangrene (NCGG) is another rare necrotizing entity often associated with an underlying disease, particularly diabetes mellitus, and has a high mortality rate. CASE REPORT: A 16-year-old, immunocompetent male was referred to us after four days, following a roadside accident, with a degloving injury over the thigh and knee along with fractures around the knee. Although clinico-radiologically suspicious of gas gangrene, the initial smear report was negative for any Gram-positive ba

Re-emergence of hand-muscle representations in human motor cortex after hand allograft.

The human primary motor cortex (M1) undergoes considerable reorganization in response to traumatic upper limb amputation. The representations of the preserved arm muscles expand, invading portions of M1 previously dedicated to the hand, suggesting that former hand neurons are reassigned to the control of remaining proximal upper limb muscles. Hand allograft offers a unique opportunity to study the reversibility of such long-term cortical changes. We used transcranial magnetic stimulation in patient LB, who underwent bilateral hand transplantation 3 years after a traumatic amputation, to longit

Tibial turn-up for long distal femoral bone loss.

Since 1990, seven patients have been treated by the authors with the tibial turn-up procedure. They have ranged in age from 8 to 37 years; four were skeletally mature adults and three were children. All seven patients were faced with a clinical situation that required surgical removal of a long portion of their affected distal femur. Three of the adult patients initially were treated for osteosarcoma with long distal femoral resections and allograft arthrodesis of the knee. The allografts ultimately failed, two because of aseptic failure and one because of infection. One patient required dista

Placement of Peripheral Nerve Stimulation Device in Single Limb Controls Long-Term Pain in Pediatric Patient With 4-Limb Amputation: A Case Report.

Septic shock caused by Group A Streptococcus (GAS) can cause multiorgan failure and limb ischemia, necessitating amputations. We present a 17-year-old girl who survived GAS-induced septic shock but required quadruple amputations. Postoperatively, she experienced significant stump and phantom pain primarily in her left leg, posing challenges for pain management. Two percutaneous peripheral nerve stimulation (PNS) leads were implanted, targeting the left femoral and posterior sciatic nerves. This resulted in excellent long-term pain control, allowing for rapid weaning off systemic opioids and ot

Frequency and Localization of Neuromas in Transtibial Residual Limbs.

OBJECTIVE: To describe the frequency and localization of neuromas in residual limbs of individuals with transtibial amputation using ultrasound imaging. DESIGN: Cross-sectional study. SETTING: Rehabilitation center research laboratory. PARTICIPANTS: Adults who have lived with a transtibial amputation for >12 months were recruited for this study. Participants were included regardless of the presence or absence of residual limb neuropathic pain. Twenty-three participants (24 transtibial residual limbs) with and without residual limb neuropathic pain were enrolled. The etiology of amputation o

Purpura fulminans skin loss: surgical management protocols at a regional burns centre.

People with significant skin loss after sepsis frequently require skin grafting; as such, they are often referred to regional burns units for definitive soft-tissue cover. Such individuals have complex requirements after severe sepsis, and usually have complicated wounds. We have developed a protocol for managing these patients and their wounds within a high dependency, intensive-care unit, using allograft as an adjunct in preparing the tissue bed before autografting. This protocol includes optimisation of nutritional needs, microbiological surveillance and definitive soft tissue cover. We ret

Distraction lengthening by callotasis in the hand.

The clinical results of 26 digits (18 patients) lengthened by distraction callotasis were evaluated and the factors which influenced healing were analysed. There were 14 men and four women, with a mean age of 39 years. All digits had suffered traumatic amputation. There were eight thumbs and 18 fingers. The level of the site of the osteotomy was at the proximal metaphysis in ten, the middle diaphysis in 13 and the distal metaphysis in three. Although the proposed length was achieved in 23 of the 26 digits, five required additional bone grafts. The rate of healing was 96 days/cm in the digits w

First experience with a distraction apparatus prototype (SM-fix-phalanx distractor) for phalanges lengthening.

Since 1991 seven children aged between 5-17 years have been treated at the University Children's Hospital in Basel for 10 traumatically partially amputated phalanges with a newly developed external fixator and distractor. Through continual distraction of the osteotomy a phalanx lengthening of 3-10 mm has been achieved (on average 55% of the rest phalanx). The results of this method in distal phalanx amputation with resulting nail dystrophy (claw nail) has shown functional and cosmetic improvements. The average distraction and consolidation phases lasted 5 weeks. The after care lasted 2-12 mont

Salvaging the ischemic transmetatarsal amputation through distal arterial reconstruction.

From 1982 to 1991, 17 patients underwent a lower extremity arterial bypass to salvage an ischemic transmetatarsal amputation at the New England Deaconess Hospital. Eleven patients were male, and 16 had diabetes for an average of 29 years. The mean age was 71 years. Twelve patients presented with an ischemic ulcer, one had rest pain, and four underwent bypass for failure to heal a transmetatarsal amputation. Twelve patients presented with findings of secondary infection. All 17 patients underwent successful lower extremity bypass procedures to a variety of outflow vessels. Thirteen bypasses wer

Skewflap versus long posterior flap in below-knee amputations: multicenter trial.

A multicenter trial of alternative techniques for below-knee amputation is described in which surgeons in 11 centers randomized 191 patients with end-stage occlusive vascular disease to two different methods of stump construction. The skewflap technique was performed in 98 and the long posterior flap was performed in 93. The two groups were well matched in respect to age, sex, smoking, diabetes, and indications for amputation. Early outcome was compared in terms of 30-day mortality rate: skew 11 (11%) deaths versus long posterior flap 16 (17%); the state of the wound at 1 week (primary healing

Intense Focused Ultrasound Preferentially Stimulates Transected Nerves Within Residual Limbs: Pilot Study.

OBJECTIVE: Identifying pain generators in tissue deep in the skin can require uncomfortable, complicated, and invasive tests. We describe pilot studies testing the hypothesis that ultrasound image-guided, intense focused ultrasound (ig-iFU) can noninvasively and differentially stimulate the end of transected nerves in the residual limbs of amputee patients. DESIGN: We applied iFU to the transected nerve ending as individual pulses with a length of 0.1 seconds using a carrier frequency of 2.0 MHz. After targeting, we gradually increased the iFU intensity to reach consistent patient-reported st

Northwestern University Flexible Subischial Vacuum Socket for persons with transfemoral amputation-Part 1: Description of technique.

BACKGROUND: Current transfemoral prosthetic sockets restrict function, lack comfort, and cause residual limb problems. Lower proximal trim lines are an appealing way to address this problem. Development of a more comfortable and possibly functional subischial socket may contribute to improving quality of life of persons with transfemoral amputation. OBJECTIVES: The purpose of this study was to (1) describe the design and fabrication of a new subischial socket and (2) describe efforts to teach this technique. STUDY DESIGN: Development project. METHODS: Socket development involved defining th

Quantification of rectifications for the Northwestern University Flexible Sub-Ischial Vacuum Socket.

BACKGROUND: The fit and function of a prosthetic socket depend on the prosthetist's ability to design the socket's shape to distribute load comfortably over the residual limb. We recently developed a sub-ischial socket for persons with transfemoral amputation: the Northwestern University Flexible Sub-Ischial Vacuum Socket. OBJECTIVE: This study aimed to quantify the rectifications required to fit the Northwestern University Flexible Sub-Ischial Vacuum Socket to teach the technique to prosthetists as well as provide a computer-aided design-computer-aided manufacturing option.
