PURPOSE: To evaluate patient satisfaction with multifocal intraocular lens (MIOL) implants (AcrySof Restor) in relation to the size of angle kappa and precise centration of the MIOL.

An original MIOL-23 multifocal elastic intraocular lens (IOL) was used to operate 5 eyes with acquired lens dislocation and traumatic cataract. By making self-sealing tunnel incision, ILO was implanted into the capsular sac and sutured to the iris. MIOL-23 implantation caused an increase in mean visual acuity. The IOL took up a correct position. Elastic IOL implantation with iris suture fixation is an efficient and safe mode of additional ILO support.

The safety and efficacy of a modified technique for transscleral fixation of a foldable posterior chamber intraocular lens (IOL) for intracapsular aphakia (secondary IOL) or after complicated phacoemulsification was evaluated. All eyes had inadequate or no posterior capsular support. Follow-up was between 4 and 26 months (mean, 12 months). Uncorrected visual acuity improved (> 2 lines) in all patients. PURPOSE: To report and illustrate a modified and simple technique for silicone oil removal from aphakic eyes.
DESIGN: Interventional case series.
METHODS: Eighteen eyes of 18 patients who once had pars plana vitrectomy and lensectomy combined with silicone oil tamponade underwent silicone oil removal using a self-sealing corneal incision.
RESULTS: All of the 18 silicone oil removal procedures performed had clear corneal incisions, which did not require suturing.
CONCLUSIONS: Silicone oil can be removed successfully from aphakia using the self-sealing corneal incision under local anesthesia

One of the most important complication in the cataract surgery is the uveitis of the IOL ("toxic lens syndrome"). There are shown the main factors related to the artificial lens, viscoelastic materials and sutures, which may provoke inflammatory reactions. Eighteen patients had a surgical inferior peripheral iridotomy performed to prevent pupil block and silicone oil anterior chamber prolapse. The occlusion of an iridotomy was treated by neodymium: YAG laser therapy and this form of treatment was successful only in 4 cases (22%). The reopening of occluded iridotomies is best performed by surgery as opposed to laser treatment.

To study the risk factors in the development of glaucoma following penetrating keratoplasty, we retrospectively analysed 190 eyes of 185 consecutive patients who had undergone surgery during 1990. The donor button was larger by 0.2 mm and 0.5 mm in phakia and aphakia/pseudophakia patients, respectively. Interlamellar refraction keratoplasty with central keratectomy of superficial laminae of the cornea was used to correct aphakia in 15 patients and the results followed up for 2.5-7 years. This method permits a simultaneous refraction and optic transplantation of the cornea. Corneal refraction was enhanced by 7.2-14.8 diopters (the mean value 11.03 diopters). The technique is an extraocular one and therefore may be widely used for aphakia correction, particularly in cases with superficial opacities of the cornea.

We have implanted 530 Galand's disc lens in the last 15 months. Stability and self-centering of implant were good when peroperative vitreous pressure was normal and in any case when wound was closed.
