Infant sleep apnea profile: preterm vs. term infants.

By means of polygraphic sleep recording, the sleep apnea profile with respect to the number and duration of inactive, obstructive and mixed apneic episodes as well as periodic breathing has been investigated in infants born preterm at 40, 52 and 64 weeks conceptional age and compared to that of term infants. At 40 weeks preterm infants showed significantly more apnea and periodic breathing compared to term infants. The difference was essentially due to obstructive and mixed apnea in non-REM sleep. There was a sharp decrease in all apneic variables--inactive, obstructive and mixed apnea as well…

Resolution of prolonged neonatal apnea with hernia repair.

Three cases of hernia repair and simultaneous resolution of prolonged neonatal apnea are reported. An association has been made between vagal overactivity and apnea. Protrusion of a large hernia at times of vagal stimulation is suggested as a possible mechanism resulting in sustained inhibition of the respiratory control center by increasing vagal afferent traffic to this center. Consideration should be given to repair of hernias in the premature infant with neonatal apnea rather than wait for resolution of the apneic episodes.

Infant apnea: polygraphic studies and follow-up monitoring.

Twenty-six infants with unexplained and apparently life-threatening apnea were evaluated clinically, underwent eight- or 12-hour polygraphic recordings during sleep, and were then observed on home monitors. The one exception was an infant who was treated with theophylline and not monitored at home. Clinical evaluation revealed a history of vomiting in 19 infants, gastroesophageal reflux (GER) on esophagram in 19, and subtle neurologic abnormalities in ten. Polygraphic studies revealed questionable EEG abnormalities in nine infants, abnormal rapid eye movement (REM) time in four, prolonged apne…

Infant apnea syndrome. A prospective evaluation of etiologies.

The frequency of proposed causes for the infant apnea syndrome (IAS) or "near miss" sudden infant death is unknown. Using a protocol, we investigated a small series of 28 consecutive infants with IAS. This in-hospital evaluation included, for all patients, blood chemistries; x-rays of skull, lumbar puncture, chest, and barium swallow; electrocardiogram, echocardiogram, 24-hour Holter cardiac monitor; viral and bacterial cultures of blood, urine, stool, and CSF; EEG; and esophageal pH study for gastroesophageal reflux (GER). A probable cause was found in 17 of the 28 infants including GER 6, en…

Laryngeal reflex apnea in the chemodenervated newborn piglet.

Effects of peripheral arterial chemodernervation on laryngeal reflex-induced apnea were studied in 18 piglets of either sex varying in age from 4 to 63 days. The distal trachea was cannulated with a cuffed endotracheal tube to secure a free airway and permit ventilatory measurements with a pneumotachograph. The proximal trachea was used to introduce fluids into the larynx. Water elicited apnea, bradycardia, and arterial hypertension, whereas saline caused only transient disturbances. Electrical stimulation of the superior laryngeal nerves reproduced, and conduction anesthesia ablated, the effe…

[Prevention of neonatal apnea with theophylline (author's transl)].

To the well known methods for prevention of neonatal apnea like stimulation, CPAP, and mechanical ventilation the treatment with theophylline has been added recently. We studied the efficiency of theophylline therapy in 10 premature infants with apnea. The initial dosage was 6 mg theophylline/kg body-weight, followed by a dosage of 2 mg/kg for the next 12 h. The frequency of apnea thereby was significantly reduced. Only in two cases the heart rate rose above 180/min as a side effect. In no case an additional therapy was necessary.

Apnea, hypoxemia, and aborted sudden infant death syndrome.

To find out whether there is any relationship between the ventilatory response to hypoxia and the sudden infant death syndrome (SIDS), we studied the effects of mild induced hypoxia (PIO2, 120 mm Hg = 17% oxygen) in 16 infants aged 2 weeks to 6 months. Eight had recurrent apneic spells (apnea group) (five had aborted SIDS and three had recurrent apnea in the intensive care nursery) and eight were "well" preterm infants about to fly in a pressurized airplane (PIO2, 120 mm Hg) (control group). Mean birth weights were 2,245 and 1,400 gm and mean gestational ages were 35 and 30 weeks. Postconceptu…

Navigating sleep apnea in Scandinavia - a journey of contrasts.

STUDY OBJECTIVES: Obstructive sleep apnea is a prevalent condition with substantial physical, psychological, and social burdens, yet limited research explores how patients experience sleep apnea-related services in the Scandinavian context. This study aims to examine lived experiences of diagnosis, treatment, and follow-up services in Southeast Norway to identify opportunities for service improvement. METHODS: Using a Co‑Design approach, the study examined assessment and treatment services through participatory workshops and semi‑structured interviews with ten (10) participants. Reflexive the…

An overview of central sleep apnea with heart failure.

INTRODUCTION: Central sleep apnea (CSA) affects up to one-third of patients with heart failure (HF) and is linked to sympathetic activation, arrhythmias, impaired quality of life, and worse prognosis. Understanding epidemiology, mechanisms, and evolving therapies is essential as management recommendations are rapidly changing. AREAS COVERED: This review synthesizes data retrieved from a PubMed database search on CSA prevalence across HF phenotypes, delineates pathophysiology, and appraises therapies. We performed a structured search for randomized trials, large observational studies, meta-ana…

Roles of vitamins and nutrition in obstructive sleep apnea.

INTRODUCTION: Obstructive sleep apnea (OSA) is the most common sleep-related breathing disorder, with a prevalence that rises alongside with the increasing prevalence of obesity. OSA is characterized by a low-inflammatory state and is followed by cardiovascular and metabolic sequelae. Continuous positive airway pressure (CPAP) represents the cornerstone of treatment for severe OSA. However, poor compliance with CPAP treatment renders OSA treatment a challenging venture. Weight loss and exercise are recommended as adjunctive treatment options for OSA. Several diets have proven to facilitate wei…

Incretins for management of obesity and obstructive sleep apnea.

INTRODUCTION: Recent trials have demonstrated the efficacy of incretin-based pharmacotherapy in obstructive sleep apnea (OSA) in obese patients. In addition to improving sleep indices, these agents also led to sustained weight loss and improvements in cardiometabolic profile. Given cardiovascular mortality remains the most common cause of death among patients with OSA, these agents represent a potential paradigm shift toward more holist management approach toward patients with OSA. AREAS COVERED: This article provides an overview of OSA pathophysiology, in particular, its overlapping risk fac…

Phenotypes of headache in patients with obstructive sleep apnea.

Obstructive sleep apnea (OSA), characterized by repeatedly collapse of upper airway while sleeping, is a common disease; estimated to have a prevalence of 57% in adults. Headache is a common neurological symptom and can be bothersome. A meta-analysis found that 33% of patients with OSA may have headaches. There is limited data on risk factors for having headache in patients with OSA. This study aimed to find risk factors or phenotypes of headache in patients with OSA. This was a retrospective analytical study. The inclusion criteria were adult patients with age of 18 years or over and diagnose…

Sleep apnea endotypes and their implications for clinical practise.

Obstructive sleep apnea (OSA) is now recognized to be a multifactorial disorder caused by several endotypes that differ between individuals and patient populations. In order to understand the current clinical implications of OSA endotypes, this review aims to examine the current state of play in endotype measurements, endotype modification and their impact on clinical outcomes/prognosis. In summary, the field has developed multiple techniques to measure the endotypes over the last few decades. There is now good proof-of-concept evidence that a priori knowledge of an individual's OSA endotype t…

Approach the Patient With Obstructive Sleep Apnea and Obesity.

Obstructive sleep apnea (OSA) and obesity are highly prevalent and bidirectionally associated. OSA is underrecognized, however, particularly in women. By mechanisms that overlap with those of obesity, OSA increases the risk of developing, or having poor outcomes from, comorbid chronic disorders and impairs quality of life. Using 2 illustrative cases, we discuss the relationships between OSA and obesity with type 2 diabetes, dyslipidemia, cardiovascular disease, cognitive disturbance, mood disorders, lower urinary tract symptoms, sexual function, and reproductive disorders. The differences in O…

The association of lymphocyte with hypothyroidism in obstructive sleep apnea.

PURPOSE: Obstructive sleep apnea (OSA) is a common sleep-breathing disorder. Numerous investigations have found a strong inherent relationship between OSA and hypothyroidism. Studies suggest that lymphocytes may be involved in the development of hypothyroidism in patients with OSA. This study aimed to assess the association between lymphocytes and hypothyroidism in OSA patients. PATIENTS AND METHODS: This study involved 920 patients with OSA who underwent nocturnal sleep monitoring, thyroid function testing, and routine blood tests. In patients with OSA, logistic regression analysis indicated…

Management of Obstructive Sleep Apnea in the Infant and Newborn.

Obstructive sleep apnea in newborns and infants presents a unique challenge with distinct differences in sleep physiology, etiologies, and management compared to older children. The indications for and interpretation of polysomnography are less well defined in infants. There are also no broadly accepted clinical practice guidelines for treating sleep apnea in this age group. Etiologies include general causes of upper airway obstruction in infants such as laryngomalacia, micrognathia, and nasal obstruction in addition to adenotonsillar hypertrophy. Treatment strategies must be tailored to the s…
