Laparoscopic appendectomy for acute and recurrent appendicitis: retrospective analysis of a single-group 5-year experience.

BACKGROUND AND PURPOSE: Twenty years after the first report of laparoscopic appendectomy (LA), its role in the treatment of appendicitis is still under debate. We report herein a retrospective analysis of our cases of LA in the last 5 years, during which we adopted a policy of an almost uniform laparoscopic approach on a rather selected population composed mainly of women with acute or recurrent lower quadrant abdominal pain. PATIENTS AND METHODS: Laparoscopic appendectomy was performed on 33 male and 98 female patients. The mean age at operation was 25.7+/-11.4 years (range 11-59 years). Acu…

Comparison of appendectomy medical expense and clinical outcome between fee for service and prospective payment system.

Since the introduction of national health insurance on March 1st 1995 in Taiwan, another 9 items (including appendectomy) were introduced into the Taiwan/prospective payment system (T/PPS). The modified T/PPS was based on those complicated appendectomy cases with secondary diagnosis or second operation where a total fee over 37,500 New Taiwan (NT) dollars was paid by National Health Insurance Bureau (NHIB) according to the real cost. The T/PPS was implemented in October 1997 due to the continuously increasing financial burden of medical expenses on the NHIB. The purpose of this study is (1) to…

Safety of in-hospital delay of appendectomy in complicated appendicitis - a propensity score-matched analysis of 1027 patients.

INTRODUCTION: Appendectomy is one of the most frequently performed emergency surgical procedures worldwide. Current guidelines recommend surgical intervention within 24 h for uncomplicated appendicitis; however, the optimal timing for patients with complicated appendicitis remains unclear. Recent studies suggest that in-hospital delay may be safe in uncomplicated cases, yet patients with clinical or radiological signs of perforation are usually excluded. This study aims to identify clinical and laboratory risk factors for perforation and to evaluate whether it is safe to delay surgery by 6 to …

Migration of a Hem-O-Lok clip into the cecal lumen following laparoscopic appendectomy: a case report.

BACKGROUND: Hem-O-Lok clips are widely used in laparoscopic appendectomy (LA). Although postoperative clip migration is rare, it is a clinically significant complication. Previous reports have mainly focused on clip migration after cholecystectomy and urologic surgery. To the best of our knowledge, there have been no reports of clip migration into the intestinal lumen after LA. This report suggests that such migration may occur after LA, highlights an important potential device-related complication, and adds to the relevant literature. CASE PRESENTATION: A 44-year-old male experienced persist…

Clinical study on the safety of omitting abdominal drainage in laparoscopic appendectomy for adult acute perforated appendicitis.

This study aimed to investigate the safety and feasibility of avoiding postoperative drainage in adult patients who underwent laparoscopic appendectomy for acute perforated appendicitis. A retrospective analysis was conducted on 98 adult patients with acute perforated appendicitis who underwent laparoscopic appendectomy at Hefei Second People's Hospital from June 2023 to February 2025. Patients were divided into a drainage group (n = 62) and a nondrainage group (n = 36) based on whether abdominal drainage was applied. Preoperative peripheral white blood cell (WBC) count, neutrophil (NEUT) coun…

Balancing Between Negative Appendectomy and Complicated Appendicitis: A Persisting Reality Under the Rule of the Uncertainty Principle.

Uncertainty is inherent in medical practice. False-negative decisions can delay treatments and result in adverse outcomes in children with acute appendicitis (AA). On the other hand, false-positive surgery decisions lead to unnecessary appendectomies. Impressive technological advancements, such as magnetic resonance imaging and laparoscopy, have reduced but failed to eliminate the occurrence of erroneous decisions. Furthermore, there seems to be a fundamental limit to further reduction, especially in eliminating the rates of negative appendectomy or, oppositely, complicated appendicitis. What …

Case Report: A Mirror Within: Open Appendectomy in a Patient With Situs Inversus Totalis and Acute Appendicitis.

In case of situs inversus totalis, acute appendicitis can develop in the left lower quadrant, and delayed diagnosis and complications can occur. Clinicians should be highly suspicious and should use imaging to confirm diagnosis. Detection of anatomical differences leads to intervening steps at an initial stage and optimal surgical success in peculiarities.

Localized large B-cell lymphoma with MYC and BCL6 rearrangements as an unexpected finding in an appendectomy specimen.

Large B-cell lymphomas (LBLs) comprise a genetically heterogeneous group of clinically aggressive non-Hodgkin lymphomas, frequently exhibiting overlapping features with diffuse large B-cell lymphoma and Burkitt's lymphoma. We report a case of a 24-year-old, previously healthy male with a classical clinical presentation of acute appendicitis. The pathologic examination discovered a large B-cell lymphoma with MYC and BCL6 rearrangements in the excised appendix. The patient was assigned Stage IEA, IPI = 0 and received chemotherapy. This case depicts a very uncommon occurrence of fully localized L

Risk-Benefit Comparison Between Endoloop and Endostapler Devices for the Closure of Appendiceal Stumps in Laparoscopic Appendectomy.

Introduction Acute appendicitis is a common cause of acute abdomen and the most frequent surgical emergency in the world. Since the nineteenth century, surgical resolution has been the most accepted treatment worldwide, and laparoscopic appendectomy is currently preferred as the treatment of choice because it has several benefits. The closure of the appendiceal stump is the most crucial step during appendectomy since its inadequate management can cause post-surgical complications. Throughout recent years, several methods have been proposed to perform this closure. This study was performed to c…

Strictly-controlled techniques are required in laparoscopic appendectomy for elemental mercury sequestration in the appendix: a case report.

Elemental mercury impaction in the appendix can cause subsequent local and systemic complications. We present a case of a teenage boy who ingested approximately 10 mL of elemental mercury, resulting in residual mercury sequestration in the appendix after conservative management. We performed laparoscopic appendectomy to remove the residual mercury. The patient made a complete clinical recovery without adverse events related to mercury poisoning over the 6-month follow-up. We highlight the advantages of laparoscopic appendectomy, abdominal computed tomography (CT), negative pressure operating r…

Safety and Cost Savings Using Polymeric Clips in Appendectomies in the Pediatric Population: Single-Center Experience.

INTRODUCTION: Although laparoscopic appendectomy is standard management for appendicitis, management of the appendiceal stump remains debated. Even though most surgeons can agree on the safety and effectiveness of various closure methods for the appendiceal stump, such as the surgical stapler (SS) or the Endoloop, the cost of these methods should also be considered. A relatively new alternative method, the polymeric clips (PC), has been gaining acceptance in the surgical community as it has repeatedly proven to be as safe as other methods, while being significantly cheaper. METHODS: For the p…

Appendectomy and the subsequent risk of cancer: A prospective population-based cohort study with long follow-up.

BACKGROUND: The appendix, an organ of immunological and microbiological importance, could be involved in the pathogenesis of cancers, but results are inconclusive. Our objective was to assess the association between appendectomy and the subsequent risk of cancer. METHODS: Data were obtained from the Rotterdam Study; a long-term prospective population-based study of individuals aged 55 years and older, of which the first cohort started in 1990 and included 7983 participants. Information on appendectomy was obtained through either medical interview at baseline or linkage with the national autom…

Reducing the negative appendectomy rate with the laparoscopic appendicitis score; a multicenter prospective cohort and validation study.

BACKGROUND: Approximately nine percent of all acute appendectomies are unintentionally performed on a normal appendix. Failure of treatment (negative appendectomy or missed appendicitis) is associated with higher morbidity and mortality when compared to appendectomy for uncomplicated appendicitis. The Laparoscopic APPendicitis (LAPP) score was developed in order to systematically evaluate the appendix for the presence of inflammation. This study aims to determine whether the LAPP score reduces the negative appendectomy rate without missing appendicitis. METHODS: From September 2013 through Ma…
