Management of pediatric quadrigeminal arachnoid cysts: a systematic review and illustrative case report.

INTRODUCTION: Quadrigeminal arachnoid cysts (QACs) are often incidental but may present with compressive symptoms on surrounding brain structures. This study evaluates management strategies and outcomes in the literature and highlights the feasibility and importance of neuroendoscopic treatment in resource-limited settings using an illustrative case. METHODS: Our systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines using PubMed, Google Scholar, Embase, and SCOPUS databases to identify and assess primary articles explori

Case Report: Scalpel Sign and Dorsal Arachnoid Cyst-The Importance of an Accurate Diagnosis.

Background and Clinical Significance: Thoracic dorsal arachnoid web (DAW) is a rare intradural extramedullary condition characterized by a thin band of arachnoid tissue compressing the dorsal spinal cord. A hallmark imaging feature is the "scalpel sign", which refers to anterior displacement of the thoracic spinal cord with dorsal cerebrospinal fluid (CSF) accumulation, producing a sagittal profile resembling a surgical scalpel. Although highly specific for DAW, this sign may also appear in other intradural conditions such as idiopathic ventral spinal cord herniation and arachnoid cysts. The c

Absent enhancement interface sign: a novel radiologic indicator of intrasellar arachnoid cysts.

PURPOSE: Intrasellar arachnoid cysts are extremely rare, and their differentiation from other cystic lesions can be challenging because of their similar imaging characteristics. METHODS: This study aimed to describe the imaging characteristics observed in five patients and correlate them with developmental and growth mechanisms. RESULTS: These cysts are characterized by the lack of contrast enhancement in their walls and posterior displacement of the pituitary gland. As a result, no enhancing tissue is present at the anterior portion of the sella where it interfaces with the sphenoid sinus.

[First episode psychosis associated with increased volume of an arachnoid cyst in the posterior fossa].

We describe the case of a 64-year-old woman who experienced a first psychotic episode following an increase in volume of a known arachnoid cyst in the posterior fossa. This was associated with anxiety and panic attacks as well as mild cognitive deficits. A cystoperitoneal shunt was placed three years earlier to treat headaches, poor balance and memory problems associated with the cyst. Increasing the flow rate through the shunt, followed by the initiation of an SSRI, resulted in remission of the psychotic and affective symptoms. This case illustrates the potential association between structura

Arachnoid Cysts in Athletes with Sports-Related Concussion: A Case Series and Literature Review.

BACKGROUND: Arachnoid cysts (AC) are associated with a risk of rupture or haemorrhage following head impact and pose a potential predisposing factor for significant complications of sport-related concussion. Despite a recognised association between ACs and intracranial haemorrhage/cyst rupture, the risk profile of participating in contact sports with AC is not well defined. We report a retrospective case series of players presenting to the Birmingham Sports Concussion Clinic between 2017 and 2023 and underwent MRI head, with a comprehensive review of the prior literature. RESULTS: 432 athlete

Quadrigeminal arachnoid cyst: Factors affecting outcomes and complications in a large series of cases.

PURPOSE: Arachnoid cysts constitute approximately 1 % of intracranial mass lesions, with quadrigeminal cistern arachnoid cysts being 5-18 % of those. This study presents a series of 31 cases of quadrigeminal cistern arachnoid cysts, constituting the most extensive series reported to date. METHODS: A retrospective analysis was conducted on 31 patients diagnosed with quadrigeminal cistern arachnoid cysts, focusing on clinical presentation, demographics, treatment approaches, and outcomes. Patient ages ranged from 6 m to 45 years, with a mean age of 29. The cysts were classified into three subty

Sports Participation and Sports-Related Neurologic Injuries in Pediatric Patients With Arachnoid Cysts.

BACKGROUND: Currently, there is no consensus recommendation regarding the safety of sports participation for pediatric patients with arachnoid cysts (ACs). OBJECTIVE: To prospectively survey patients with ACs to define the risk of sports-associated neurologic injury in untreated and treated patients. METHODS: A prospectively administered survey was given to all patients diagnosed with an AC who presented to a single pediatric neurosurgery clinic between December 2010 and December 2021. Data were recorded on demographic information, imaging characteristics, treatment, sports participation, an

Strangulation of the abducens nerve by an arachnoid band within an epidermoid cyst: illustrative case.

BACKGROUND: Intracranial epidermoid cysts (ECs) are rare benign lesions of ectodermal origin that can be found in the cerebellopontine angle (CPA). If large enough, they compress surrounding structures, causing cranial neuropathies, cerebellar dysfunction, and hydrocephalus. OBSERVATIONS: In this case report, the authors present a patient with headaches and diplopia secondary to a CPA EC. The cyst had wrapped itself around and was strangulating the abducens nerve. This choking of the nerve was believed to have been the cause of the patient's double vision. The arachnoid band was released with

Familial and syndromic forms of arachnoid cyst implicate genetic factors in disease pathogenesis.

Arachnoid cysts (ACs) are the most common space-occupying lesions in the human brain and present significant challenges for clinical management. While most cases of ACs are sporadic, nearly 40 familial forms have been reported. Moreover, ACs are seen with increased frequency in multiple Mendelian syndromes, including Chudley-McCullough syndrome, acrocallosal syndrome, and autosomal recessive primary ciliary dyskinesia. These findings suggest that genetic factors contribute to AC pathogenesis. However, traditional linkage and segregation approaches have been limited in their ability to identify

Thoracic Surgeon Encounters Cerebrospinal Fluid? A Rare Case of Mediastinal Arachnoid Cyst.

Growth of a spinal arachnoid cyst into the posterior mediastinum is a rare phenomenon not described in the literature. We present the case of a 30-year-old patient with chest pain, cough, fatigue, and occasional dysphagia who was found to have an 8 × 6.5 × 9.5-cm cyst located in the posterior mediastinum on imaging. The patient was taken to the operating room, where clear fluid positive for β2-microglobulin consistent with cerebrospinal fluid was encountered. Dural communication with the spinal canal was identified, confirming diagnosis of an arachnoid cyst. This report aims to describe this r

Meningeal CSF transport is primarily mediated by the arachnoid and pia maters during development.

BACKGROUND: The recent characterization of the glymphatic system and meningeal lymphatics has re-emphasized the role of the meninges in facilitating CSF transport and clearance. Here, we characterize small and large CSF solute distribution patterns along the intracranial and surface meninges in neonatal rodents and compare our findings to a rodent model of intraventricular hemorrhage-posthemorrhagic hydrocephalus. We also examine CSF interactions with the tela choroidea and its pial invaginations into the choroid plexuses of the lateral, third, and fourth ventricles. METHODS: 1.9-nm gold nano

Recurrent Meningitis After a Post-Traumatic Intradiploic Arachnoid Cyst: A Case Report.

Intradiploic arachnoid cysts are infrequent but benign lesions of the central nervous system. Etiologically, they can be non-traumatic or post-traumatic in origin. We present an unusual case of a post-traumatic intradiploic arachnoid cyst presented with recurrent meningitis episodes. A 68-year-old female patient was admitted to the emergency department with fever and loss of consciousness, with a history of cranial operation due to a gunshot injury to the left occipital bone 45 years ago. On the patient's initial examination, nuchal rigidity was detected; Kernig's and Brudzinski's signs were p

Surgical management of quadrigeminal cistern arachnoid cysts: case series and literature review.

OBJECTIVE: Quadrigeminal cistern arachnoid cysts (QACs) are congenital lesions that can cause pineal region compression and obstructive hydrocephalus when sufficiently large. Management of these cysts is controversial and rates of reintervention are high. Given the limited data on the management of QACs, the authors retrospectively reviewed 20 years of cases managed at their institution and performed a literature review on this topic. METHODS: The authors performed a retrospective analysis of patients treated for QAC at their institution between 2001 and 2021. They also performed a literature

A rare giant intracranial arachnoid cyst confused the diagnosis and treatment of Wilson disease.

BACKGROUND: Hepatolenticular degeneration (HLD), also known as Wilson disease (WD), is a rare autosomal-recessive hereditary disease, which is often missed and misdiagnosed because of its various clinical manifestations. And WD is even more rare with giant subarachnoid cysts. In this report, we will provide a case of WD with an intracranial arachnoid cyst (IAC). CASE DESCRIPTION: A 27-year-old woman was hospitalized in a traditional Chinese medicine hospital in Guangzhou with the first manifestation of a "slight involuntary tremor of her left upper limb". There was no improvement after acupun

Prenatally Symptomatic Suprasellar Arachnoid Cyst: When to treat? A case-base update.

INTRODUCTION: Suprasellar Arachnoid Cysts (SAC) are rare heterogeneous entities. Though prenatally diagnosed, they are rarely treated pre-birth. Symptomatic cases are mainly seen in infants. CASE DESCRIPTION: We describe a case of a prenatally symptomatic suprasellar arachnoid cyst treated postnatally. The cyst was diagnosed on a routine ultrasound at 22 weeks, was rapidly evolving in the ultrasounds and the MRI of the 29th week. It then became symptomatic at 30 weeks with episodes of fetal bradycardia, independent to the uterine contractions. Antenatal treatment was discussed but delivery de

Endoscopic Management of Lateral Ventricle Arachnoid Cysts-Case Series with Proposed Classification.

BACKGROUND: We present a case series of 7 patients with intraventricular arachnoid cysts of lateral ventricle managed with endoscopic treatment with proposed classification of the cyst based on anatomic extent. METHODS: In all patients, the lateral ventricle arachnoid cyst was primarily located in the trigone and body of the lateral ventricle. Proposed classification is based on extension of the arachnoid cyst. Type 1 is an arachnoid cyst located in the lateral ventricle only, type 2 is a lateral ventricle arachnoid cyst extending to the quadrigeminal cistern, and type 3 is the lateral ventri
