[Surgery of atrial fibrillation with intraoperative radiofrequency].

Radiofrequency ablation is successful in most supraventricular tachyarrhythmias and the first results in percutaneous applications in atrial fibrillation have been published. At present, all the surgical techniques for the treatment of atrial fibrillation use extensive atrial incisions. A case report is presented of a 67 year old woman with chronic atrial fibrillation and rheumatic mitral and tricuspid valve disease. At surgery, pulmonary vein isolation was performed using a continuous linear ablation around the pulmonary veins, instead of using extensive atrial incisions. Concomitantly, a mit…

[Antithrombotic treatment of atrial fibrillation in hospital].

BACKGROUND: In clinical guidelines regarding atrial fibrillation, oral anticoagulation is recommended for patients over the age of 65 or with additional risk factors. The aim of the present study was to investigate how these recommendations are followed in clinical practice. MATERIAL AND METHODS: A retrospective study was conducted among patients hospitalized for atrial fibrillation at Tromsø University Hospital from 1995 to 1998. Data were obtained from hospital files. RESULTS: 362 patients with atrial fibrillation (57% men), mean age 68.4 years (range 24-96), were included. 23.5% had their…

[Maze procedure for a lone atrial fibrillation].

Between 1992. 2 and 1997. 12, the maze procedure for lone fibrillation or flutter (lone af) was performed in 8 patients including 2 patients with a sustained atrial fibrillation, 5 patients with a paroxysmal atrial fibrillation and 1 patient with a sustained atrial flutter. All patients had suffered from a drug resistance lone af which induced sever symptom that is the dyspnea, palpitation and fatigue. Therefore patients requested to receive the maze procedure, and they agreed with the informed concent. After the maze operation, the normal sinus rhythm was recovered in 7 of 8 patients (87.5%).…

Screening for atrial fibrillation in primary care.

OBJECTIVE: To investigate a population of elderly people for atrial fibrillation and to determine how many of the cases identified might benefit from treatment with anticoagulants. METHODS: From a practice of four primary care physicians, 1422 patients aged 65 years and over were identified, of whom 1207 (85% of the total population) underwent electrocardiographic screening to detect the presence of atrial fibrillation. Patients with the arrhythmia were further evaluated by echocardiography and interview, to stratify their risk of stroke based on echocardiographic and clinical risk factors, t…

[Pacemaker therapy in patients with atrial fibrillation].

Pacemaker therapy in patients with atrial fibrillation means the best current pacemaker therapy for patients with bradycardias with the aim to avoid the onset of atrial fibrillation and to establish DDD pacing despite of a history of atrial tachyarrhythmias. The newer application of pacing is the suppression of atrial arrhythmias in patients with medical refractory atrial tachyarrhythmias. Patients with slow ventricular rates and permanent atrial fibrillation should receive a VVI-pacemaker, if the bradycardias causes syncope, dizziness or a decrease of their exercise tolerance. In case of chro…

Factors predisposing to the development of atrial fibrillation.

Atrial fibrillation (AF) is in most patients (approximately 70%) associated with organic heart disease including valvular heart disease, coronary artery disease, hypertension, hypertrophic cardiomyopathy, dilated cardiomyopathy, and congenital heart disease, mostly atrial septal defect in adults. In many chronic conditions, determining whether AF is the result or is unrelated to the underlying heart disease, remains unclear. The list of possible etiologies also include cardiac amyloidosis, hemochromatosis and endomyocardial fibrosis. Other heart diseases, such as mitral valve prolapse (without…

[Prevention of thromboembolism in patients with atrial fibrillation].

Atrial fibrillation is a disorder of the cardiac rhythm associated with an increased risk of thromboembolism. Several studies in the past confirmed that anticoagulation or also antiaggregation therapy is associated in these patients with a reduced incidence of cerebrovascular attacks and peripheral embolism. For a more objective assessment of the risk of individual patients risk factors of thromboembolism in atrial fibrillation were defined. They include cardiac insufficiency, hypertension, a history of embolism, left atrial dilatation, systolic dysfunction of left ventricular hypertrophy and …

Prevention of thromboembolism in patients with atrial fibrillation.

New insights into atrial physiology based on observations using transesophageal echocardiography are presented. The approach to anticoagulation of patients with both acute and chronic atrial fibrillation is reviewed within the context of the results of the major multicenter trials. These have provided some useful risk stratification guidelines for therapy. Presently available data suggest that the role of transesophageal echocardiography as a precardioversion screen is promising but requires further definition through clinical trials.

Paroxysmal atrial fibrillation: a need for classification.

UNLABELLED: Classification of Atrial Fibrillation. INTRODUCTION: Clinical aspects of paroxysmal atrial fibrillation are heterogeneous. The attacks of atrial fibrillation may differ in their duration frequency and presence and severity of symptoms. Therefore, a proposal for a clinical classification of paroxysmal atrial fibrillation may be helpful. We tested a new classification system in a cohort of 51 consecutive hospitalized patients with paroxysmal atrial fibrillation. METHODS AND RESULTS: Paroxysmal atrial fibrillation was subdivided into three classes. Class I included a first attack of…

[Atrial fibrillation and the autonomic nervous system].

The autonomic nervous system plays an important role in the genesis of various cardiac rhythm disorders. Experimental studies have indicated that in atrial fibrillation particularly vagal activity may have a decisive influence on the electrophysiologic properties of atrial myocardium. In patients with paroxysmal atrial fibrillation, a carefully taken history and electrocardiographic findings obtained during Holter monitoring can help to distinguish vagally mediated from adrenergically mediated atrial fibrillation. The former is considered to represent a form of lone atrial fibrillation affecti…

[Clinical aspects and prognosis of atrial fibrillation].

Atrial fibrillation is alleged to be one of the most frequently occurring arrhythmia. Many patients with atrial fibrillation are asymptomatic. Others, however, complain of palpitations and paroxysmal atrial tachycardias. And especially in patients with preexisting heart failure, atrial fibrillation can lead to a clear worsening of cardiac output. Not only rheumatic but nonrheumatic atrial fibrillation as well are associated with an increased risk of thromboembolic complications. Arterial hypertension, a history of thromboembolism as well as signs of heart failure are clinical parameters which …

Pacemaker therapy in patients with atrial fibrillation.

Pacing is indicated in patients with AF when clinical symptoms are readily attributable to slow ventricular rate and prolonged ventricular pauses. When choosing a pacemaker for implantation, one should consider the importance of a reasonable chronotropic response to exercise and provide a rate-responsive system (VVIR) for those patients who show chronotropic incompetence. In the case of paroxysmal AF, an AV synchronized system may provide protection against deterioration to chronic AF and heart failure and is preferred over VVI pacing, providing that paroxysms are infrequent. Drug-pacemaker in

Rationale for antithrombotic therapy in atrial fibrillation.

Antithrombotic therapy is clearly indicated in patients with atrial fibrillation who have associated factors that put them at significant risk for thromboembolism This does not include subjects with lone atrial fibrillation who are less than 60 years of age. High-risk patients include those with valvular heart disease, recent congestive heart failure, severe left ventricular dysfunction by echocardiography, prior thromboembolism, demonstration of a cardiac thrombus by echocardiography, and thyrotoxicosis. Anticoagulant therapy appears to be the most efficacious means of preventing thromboembol…

Atrial fibrillation. Cause and time of onset.

To characterize groups of patients presenting at the emergency room with atrial fibrillation (AF) according to the various causes of AF and the time of its onset, 704 medical files from the Beilinson Medical Center (Petah Tikva, Israel), during an eight-year period, were reviewed. Two thirds of all patients converted to sinus rhythm (SR) within a seven-day in-hospital treatment. The most frequent causes associated with AF were atherosclerotic cardiovascular disease, 55%; rheumatic heart disease, 22.8%; chronic obstructive pulmonary disease, 2.8%; Wolf-Parkinson-White syndrome, 2.6%; and thyrot…

Amiodarone in the management of refractory atrial fibrillation.

Thirty-eight patients were studied to evaluate amiodarone hydrochloride in the treatment of refractory atrial fibrillation. Among them were 25 with sustained atrial fibrillation and 13 with paroxysmal atrial fibrillation. All patients were symptomatic and refractory to therapeutic doses of at least two conventional drug trials, and patients with atrial fibrillation had relapsed from electroversion. Amiodarone hydrochloride was administered in doses of 5 mg/kg intravenously, then 600 to 800 mg/d for seven to ten days, followed by 200 to 400 mg/d. Holter recordings were obtained every one to thr…

The histopathological substratum for atrial fibrillation in man.

Quantitative studies on the histological sections of the atria including sinoatrial (SA) node, SA junction and internodal preferential pathways in 12 hearts with long-term atrial fibrillation (Af group) and in 43 hearts with no arrhythmia (control group) have been carried out. The control group was subdivided into two age-groups, the younger and older over 50 years for the age-matched comparison with the Af group. In the Af group, markedly impaired contiguity of the SA junction, severe fibrosis and lipomatosis of atria were common precipitating factors. Regarding the ratio of SA nodal cells to…
