The Combination of IV and Perineural Dexamethasone Prolongs the Analgesic Duration of Intercostal Nerve Blocks Compared with IV Dexamethasone Alone.

OBJECTIVE: The use of multiple-level, single-injection intercostal nerve blocks for pain control following video-assisted thorascopic surgery (VATS) is limited by the analgesic duration of local anesthetics. This study examines whether the combination of perineural and intravenous (IV) dexamethasone will prolong the duration of intraoperatively placed intercostal nerve blocks following VATS compared with IV dexamethasone and a perineural saline placebo. DESIGN: Prospective, double-blind, randomized placebo-controlled trial. SETTING: Single level-1 academic trauma center. SUBJECTS: Forty pat

The role of sympathetic activity in neuropathic orofacial pain.

Seventeen patients with neuropathic orofacial pain are presented with reference to precipitating events, pain descriptions, response to treatment, and other aspects of their histories and clinical presentation. Stellate ganglion blocks were done on 14 patients. Ten of 14 patients reported temporary relief of pain with stellate ganglion blocks. Five of these patients noted more prolonged improvement in pain, two reported no change, and two experienced a temporary increase in pain. It is argued that sympathetically maintained pain involving orofacial locations does occur and that stellate gangli

Distinguishing Chronic Inflammatory Demyelinating Polyneuropathy From Mimic Disorders: The Role of Statistical Modeling.

BACKGROUND AND AIMS: Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is difficult to distinguish from mimicking disorders, with misdiagnosis resulting in IVIG overutilization. We evaluate a clinical-electrophysiological model to facilitate CIDP versus mimic neuropathy prediction. METHODS: Using the European Academy of Neurology/Peripheral Nerve Society (EAN/PNS) 2021 CIDP guidelines we derived 26 clinical and 144 nerve conduction variables. The model was generated and validated utilizing total CIDP (n = 129) and mimics (n = 309); including (1) IgG4-nodopathies; (2) POEMS (pol

Minocycline: neuroprotective mechanisms in Parkinson's disease.

Parkinson's disease (PD) is a common neurodegenerative disorder characterized by cardinal features of tremor, bradykinesia, rigidity and postural instability. In addition to the motor symptoms patients experience cognitive decline eventually resulting in severe disability. Pathologically PD is characterized by neurodegeneration in the substantia nigra pars compacta (SNc) with intracytoplasmic inclusions known as Lewy bodies. In addition to the SNc there is neurodegeneration in other areas including cerebral cortex, raphe nuclei, locus ceruleus, nucleus basalis of meynert, cranial nerves and au

CIDP/autoimmune nodopathies with nephropathy: a case series study.

OBJECTIVE: The co-morbidity of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP)/autoimmune nodopathies with nephropathy has been gradually known in recent years. This study was intended to explore the clinical, serological and neuropathological features of seven patients with CIDP/autoimmune nodopathies and nephropathy. METHODS: Among 83 CIDP patients, seven were identified with nephropathy. Their clinical, electrophysiological and laboratory examination data were collected. The nodal/paranodal antibodies were tested. The sural biopsies were performed in all the patients, and

Microanatomy of the neural scaffold of the pterygopalatine fossa in humans: trigeminovascular projections and trigeminal-autonomic plexuses.

The pterygopalatine fossa (PPF) is an anatomically-hidden deep extracranial space. The neural scaffold of the PPF remains anatomically understudied in humans. Moreover, there are no anatomical data in humans pointing out the extracranial trigeminovascular distributions, in contrast to the trigeminal supratentorial ones. By anatomical microdissections, the neural scaffold of the PPF and the presence of trigeminovascular projections were evaluated. The anterior and superior approaches of the pterygopalatine fossae in nine dissected blocks of human middle skull base and the frontal cuts of two di

Fascia iliaca block in the emergency department for hip fracture: a randomized, controlled, double-blind trial.

BACKGROUND: Hip fracture causes moderate to severe pain and while fascia iliaca block has been reported to provide analgesic benefit, most previous trials were unblinded, with subsequent high risks of performance, selection and detection biases. In this randomized, control double-blind trial, we tested the hypothesis that a fascia iliaca block provides effective analgesia for patients suffering from hip fracture. METHODS: Thirty ASA I-III hip fracture patients over 70 years old, who received prehospital morphine, were randomized to receive either a fascia iliaca block using 30 ml of bupivacai

Bilateral stellate ganglion blockade for recalcitrant oral pain from Burning Mouth Syndrome: a case report.

Burning Mouth Syndrome (BMS) is a chronic painful disorder characterized by unremitting bilateral burning oral pain often associated with taste abnormalities and complaints of dry mouth. The diagnosis is made by history and symptom presentation in the absence of an identifiable cause or oral lesion. It is commonly seen in perimenopausal women but is also seen in men, and is considered a small-fiber neuropathy. Management can be challenging and few effective treatments are available. This article presents a case report of stellate ganglion blockade as a treatment for recalcitrant pain from BMS.

Sympathectomy for hyperhidrosis: should we place the clamps at T2-T3 or T3-T4?

Endoscopic thoracic sympathectomy is routinely used to treat severe hyperhidrosis. It is usually performed at the T2-T3 level of the nerve, but may produce less severe compensatory hidrosis if performed at a lower level. This study evaluates the outcome of 1,274 patients who underwent endoscopic thoracic sympathectomy for plamar, plantar, axillary or facial hyperhidrosis/blushing. Half of the patients were clamped at the T2-T3 level and half were clamped at the T3-T4 level. Postsurgical symptoms and side effects were assessed by interview. All of patients with palmar hyperhidrosis were cured o

Practical treatment of pain in patients with chronic wounds: pathogenesis-guided management.

In addition to its own inherent morbidity, the pain associated with chronic wounds presents a primary obstacle to healing. To initiate safe and effective therapy, we used a multidisciplinary approach that required the wound-healing clinician and pain-management practitioner to work together to diagnose and treat the pain associated with these wounds. This approach emphasizes wound pathophysiology, which facilitates treatment modalities that focus on the cause of the pain as well as more efficient analgesia. All wound patients were approached with the assurance that they should not experience p

Transfer function analysis of ventilatory influence on systemic arterial pressure in the rat.

We evaluated the hypothesis that fluctuations in systemic arterial pressure (SAP) are under the influence of the respiratory pumping mechanism subjected to a modulatory action by the autonomic nervous system that is exerted primarily on the heart. Computer-generated broad-band mechanical ventilation (0-3 Hz) was applied to Sprague-Dawley rats that were anesthetized with ketamine and paralyzed with pancuronium. We observed excellent coherence between lung volume and SAP signals at ventilatory rates between 0.5 and 2.5 Hz; this coherence was unaffected by phentolamine, propranolol, atropine, bil

[The effect of drug therapy and stellate ganglion block with or without oxygen inhalation on sudden hearing loss].

Forty-one patients suffering from sudden hearing loss were studied by the following method. Twenty patients (group A) were treated with oral administration of prednisolone, intravenous administration of vitamin B and C, furosemide and stellate ganglion block. Another 21 patients (group B) were treated with oral administration of these drugs, stellate ganglion block and oxygen inhalation. Forty six percent of all these patients, 35 percent of group A and 57 percent of group B, regained less than 20 dB of their normal hearing level. The patients who are younger, having shorter duration from firs

Carotid artery surgery. Local versus general anaesthesia as related to sympathetic activity and cardiovascular effects.

Arterial plasma catecholamines, blood pressure and heart rate were determined in 75 patients before, during and after carotid endarterectomy. Local anaesthesia given as a cervical block with skin infiltration containing 200 micrograms adrenaline was used in 28 patients (LA-group), general anaesthesia (nitrous oxide, fentanyl, isoflurane) with skin infiltration containing 200 micrograms adrenaline in 32 patients (GAs-group) and general anaesthesia without skin infiltration in 15 patients (GAo-group). In the LA-Group plasma noradrenaline (P-NA) levels were significantly higher during anaesthesia

[Effect of ganglionic blockade and subsequent i.v. infusion of catecholamines and angiotensin II on the formation and transport of lymph. Studies in anesthesized dogs and a review of the literature].

The effect of ganglionic blockade (GB) and subsequent i.v. infusion of adrenaline (AD), noradrenaline (NA), isoproterenol (IP), and angiotensin II (AII) on thoracic duct lymph flow (LF) and transcapillary escape rate of plasma proteins (FER) was studied in anaesthetized (Nembutal), paralyzed (pancuronium), and artificially ventilated dogs. The carotid artery, the external jugular vein, and the thoracic duct were cannulated. Following a control period, GB was performed by pentolinium tartrate and thereafter the other drugs were infused in a random sequence for 30-60 min with 1 h intervals. Mean

Complex Regional Pain Syndrome With Central and Peripheral Nervous System Involvement in a Patient With Metastatic Lung Cancer.

Complex regional pain syndrome (CRPS) is a chronic pain disorder characterized by sensory, motor, autonomic, and trophic disturbances, often following trauma, surgery, or neurologic injury. We present a diagnostically complex case with overlapping features of both type I and type II CRPS in a 64-year-old man with metastatic lung adenocarcinoma involving the central nervous system (CNS). The patient developed neuropathic pain, weakness, and edema in the left upper and lower extremities following trauma, radiation, and cervical and lumbosacral radiculopathies. Although his presentation met the B
