The aetiology of dizziness and how to examine a dizzy patient.

Dizziness embraces various sensations of spatial disorientation. A common symptom, it has been experienced by at least one third of the population by the age of 65. Peripheral causes include disorders of the labyrinth and vestibular nerve such as Ménière's disease, benign paroxysmal positional vertigo, acute vestibulopathy (vestibular neuronitis) and acoustic neuroma. The most common lesions of the central nervous system that cause dizziness are infratentorial ischaemia, mechanical trauma, multiple sclerosis and cerebellar atrophy. Other aetiological factors include intoxications, psychogenic …

Effect on brainstem auditory evoked responses of posterior semicircular canal occlusion in guinea pigs.

The site of origin of benign paroxysmal positional vertigo is the posterior semicircular canal and when persistent, the condition is known as cupulolithiasis. Singular neurectomy is the current procedure of choice for the treatment of incapacitating cupulolithiasis. The neurectomy is difficult to learn and carries a significant risk of hearing loss. An alternative procedure, whereby the posterior canal is occluded, is discussed. This was performed on 11 guinea pigs and the effect on hearing was measured using brainstem evoked audiometry. Eight animals showed no hearing loss while the other thr…

Investigation of hearing loss in elderly vertigo and dizziness patients in the past 10 years.

BACKGROUND: Vertigo and hearing loss are both prevalent in the elderly. This study retrospectively analyzed hearing test results from elderl … nd 22.53% in left ear). Meniere's disease (MD; 30.87%), benign recurrent vertigo (BRV; 19.07%), and benign paroxysmal positional vertigo (BPPV; 15.66%) were the most common etiologies in elderly vestibular diseases. We observed statistically significant differences in hearing thresholds among these vestibular diseases (P < 0.001). K-Means clustering analysis suggested that the optima …

Dizziness in the emergency room: diagnoses and misdiagnoses.

BACKGROUND: Dizziness is among the most frequent neurological chief complaints in emergency room (ER) patients. Although the majority of underlying disorders are benign, serious causes that require immediate in-hospital treatment may occur that are difficult to identify clinically. METHODS: Retrospective study of 475 consecutive ER neurological consultations with dizziness as the chief complaint. RESULTS: Of all ER dizziness patients, 73% were initially assigned to benign and 27% to serious diagnoses. The two most frequent disorders were benign paroxysmal positional vertigo (22%) and stroke …

[Medicolegal assessment of post-traumatic vertigo].

Post-traumatic vertigo refers to a group of different disorders which occur following trauma, mainly closed head injury and whiplash injury of the cervical spine. Aside from headaches, vertigo is the most common symptom in this group of patients. In general, there are two main groups of patients with post-traumatic vertigo: those with documented vestibular dysfunctions and those without. The most common post-traumatic vestibular disorders are benign paroxysmal positioning vertigo, labyrinthine concussion, canal dehiscence and otolithic lesions. Some of these disorders are characterized by spon

PURPOSE OF REVIEW: The purpose of this narrative review is to discuss current literature about vestibular migraine and other cochleovestibul … ith risk for developing numerous otologic conditions, including Meniere's disease, vestibular loss, Benign Paroxysmal Positional Vertigo, and sudden sensorineural hearing loss. There is now some evidence that patients may experience fluctuating hearing loss and aural fullness without vertigo in association with migraine, which is called cochlear migraine. SUMMARY: Migraine can cause …

Skull Vibration-Induced Nystagmus Test (SVINT) in Vestibular Migraine and Menière's Disease.

BACKGROUND: Vestibular migraine (VM) and Menière's disease (MD) are the two most frequent episodic vertigo apart from Benign Paroxysmal Positional Vertigo (BPPV) differential diagnosis for them may be troublesome in the early stages. SVINT is a newly proposed vestibular test, which demonstrated to be fast and reliable in diagnoses above all of peripheral vestibular deficits. METHODS: We retrieved clinical data from two groups of subjects (200 VM and 605 MD), enrolled between 2010 and 2020. Among others, these subjects were included when performing a SVINT. The purpose of the study is to asses…

Effects of Peripheral Vertigo on Inflammatory and Immunologic Laboratory Markers.

The neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and mean platelet volume (MPV) are markers of subclinical inflammation and atherothrombosis. We investigated whether these easily accessible blood count parameters are affected by peripheral vertigo (PV). We studied 142 patients who were diagnosed with PV and referred to our hospital along with 135 healthy patients who were free of inflammation as a control group. The patient group was divided into 3 study groups depending on the presence of benign paroxysmal positional vertigo (BPPV): BPPV, vestibular dysfunction (V…

BACKGROUND: Residual dizziness affects a substantial proportion of patients after successful repositioning for benign paroxysmal positional vertigo (BPPV), posing a challenge for post-treatment management. MATERIAL AND METHODS: Sixty-three eligible patients, one week after successful repositioning, were alternately assigned to a cervical exercise group (n = 35) or a control group (n = 28). AIM: The aim of this article is to explore the progression of neurological, neuro-otological and cochlear features in benign paroxysmal vertigo (BPV) in children over time and its relation with migraine, neuro-otological and cochlear disorders in adulthood. METHODS: From January 2002 to December 2002, 15 children with BPV were prospectively recruited and then evaluated during a 10-year observational follow-up. All patients underwent detailed neurological, neuro-otological and cochlear examinations during interictal phases. Six children were also studied during ictal periods. RESULTS: At firs
