Involvement of monocyte-derived extracellular vesicle-associated tissue factor activity in convallatoxin-induced hypercoagulability.

OBJECTIVES: Convallatoxin (CNT) is a natural cardiac glycoside extracted from lily of the valley ( Convallaria majalis ). Although it is empirically known to cause blood coagulation disorders, the underlying mechanism remains unclear. CNT exerts cytotoxicity and increases tissue factor (TF) expression in endothelial cells. However, the direct action of CNT on blood coagulation remains unclear. Therefore, herein, we investigated the effects of CNT on whole blood coagulation system and TF expression in monocytes. METHODS: Blood samples were collected from healthy volunteers to measure plasma th…

Galla Chinensis Polyphenol-Loaded Hemostatic Granules for Rapid Hemostasis, Antibacterial Action, and Wound Healing Promotion.

Uncontrolled bleeding, coagulation disorders, and infection-related complications still present substantial challenges in emergency medicine and trauma care. Developing multifunctional hemostatic materials represent an effective strategy for addressing clinical hemostasis problems. In this study, Galla chinensis polyphenols, the effective extract of Galla chinensis, were loaded onto calcium alginate-mesoporous silica granules (CMS-GC). The CMS granules were prepared by in situ liquid-phase technology and GC was loaded by impregnation methods. In vitro and in vivo studies showed that CMS-GC not

Point-of-care coagulation testing and transfusion algorithms.

Patients with cardiovascular disease have an array of haemostasis disorders that predispose to the development of thrombotic and embolic disease states. These patients are often maintained on anti-thrombotic medication to prevent adverse cardiovascular events. Patients undergoing cardiac surgery also have haemostatic disorders that include their intrinsic disease state, adjunctive medication, and the coagulation disturbances induced by cardiopulmonary bypass. The following review introduces the monitors that are available for monitoring perioperative coagulation, with an emphasis on cardiovasc

Endotheliopathy within the thromboinflammatory network of sepsis-induced coagulopathy.

BACKGROUND: Sepsis represents a life-threatening syndrome driven by dysregulated host responses in which inflammation, coagulation, and endothelial dysfunction intersect. Increasing evidence suggests that endotheliopathy is a dynamically interacting component linking systemic inflammation, immunothrombosis, and microvascular dysfunction. Rather than acting as a singular upstream trigger, endothelial dysfunction evolves in parallel with platelet, neutrophil, and monocyte activation, forming a self-amplifying thromboinflammatory network that drives organ failure. MAIN TEXT: The vascular endothe

From molecular networks to precision therapeutics: the evolving landscape of trauma-induced coagulopathy.

BACKGROUND: Trauma-induced coagulopathy (TIC) affects approximately one quarter of severely injured patients and is linked to markedly increased mortality. Despite improvements in trauma care, understanding of TIC mechanisms and development of targeted treatment strategies remain limited. This review summarizes recent advances in mechanistic studies and translational research that inform targeted approaches for TIC. METHODS: A systematic PubMed search identified English-language basic research on TIC published from January 2016 to July 2025. The review focused on studies utilizing established

Neonatal Sepsis-Induced Coagulopathy in the Light of Developmental Hemostasis: Friend or Foe?

The concept of 'developmental hemostasis' from birth to infancy and onwards to childhood and adulthood was introduced in the 1980s and is used to indicate the fundamental discrepancies of hemostatic mechanism between children and adults. The underlying differentiations are more pronounced in term and even more in preterm neonates. Hemostatic alterations tend to improve throughout childhood and adolescence but still imply a great example of the basic concept that children do not simply represent small adults. Many neonatal coagulation disorders lead to severe morbidities, such as intraventricul

Management of abnormal uterine bleeding in the adolescent patient.

PURPOSE OF REVIEW: Abnormal uterine bleeding (AUB) is a common complaint in adolescents and is mostly treated with hormonal medication. This review provides an update on adolescent AUB evaluation and treatment. RECENT FINDINGS: Anovulatory cycles and coagulopathies remain the most common reasons for AUB. Structural causes of AUB are still overall rare in adolescents, but recent advances in imaging techniques show they are more prevalent than previously thought. Since hormonal medication is the main treatment for AUB, acknowledging updates about their safety in certain populations is necessary

Point-of-Care Testing in Patients with Hereditary Disorders of Primary Hemostasis: A Narrative Review.

Inherited disorders of primary hemostasis, such as von Willebrand disease and congenital platelet disorders, can cause extensive, typically mucocutaneous bleeding. Assays to diagnose and monitor these disorders, such as von Willebrand factor activity assays and light transmission aggregometry, are performed in specialized hemostasis laboratories but are commonly not available in local hospitals. Due to the complexity and relative scarcity of these conventional assays, point-of-care tests (POCT) might be an attractive alternative in patients with hereditary bleeding disorders. POCTs, such as th

Hematologic Emergencies: Recognition and Initial Management.

Hematologic emergencies are bleeding or clotting disorders that are hereditary or acquired and must be treated emergently to avoid significant morbidity or mortality. Patients experiencing a hematologic emergency may present with spontaneous bleeding, jaundice, petechiae, or purpura. Initial diagnostic testing should include a complete blood count. Patients who have bleeding associated with a hereditary disorder should receive clotting factor replacement before diagnostic testing. Acute chest syndrome is an uncommon but serious complication of sickle cell disease. Hemolysis caused by autoimmun

Coagulopathy and Emergent Reversal of Anticoagulation.

More patients than ever are presenting for urgent or emergent procedures while therapeutically anticoagulated for various medical indications. Medications including warfarin, antiplatelet agents such as clopidogrel, direct oral anticoagulants such as apixaban, and even heparin or heparinoids may be present. Each of these medication classes presents its own challenges when coagulopathy needs to be quickly corrected. This review article presents evidence-based discussions of monitoring and reversal of these medication-induced coagulopathies. In addition, there will be a brief discussion of other

Use of viscoelastic tests in the principle bleeding scenarios in Spanish hospitals.

Viscoelastic tests are designed to study the dynamics of clot formation, identify coagulopathies in real time, arrive at a diagnosis, and guide patient-specific administration of haemostatics. They are mainly used to treat clinically significant bleeding in any setting, and are also used in other situations involving clinically relevant alterations in haemostasis, such as coagulopathy in critically ill patients. These tests are administered following evidence-based algorithms that vary depending on the clinical context. This review summarises the results of a survey conducted in several hospit
