Comparative finite element analysis of fixation strategies for sports-related tibial tuberosity avulsion fractures in adolescents: biomechanical evidence for optimized clinical decision-making.

OBJECTIVE: This finite element study aimed to biomechanically compare screw fixation vs. K-wire fixation for adolescent tibial tubercle avulsion fractures (TTAF), investigating the differential biomechanical outcomes and physeal insult associated with each fixation method. METHODS: A finite element model of an adolescent tibia with a Modified Ogden type III fracture was developed. Three fixation types-(A) screws, (B) two non-crossing K-wires, and (C) four crossing K-wires-were tested both as standalone constructs and in their tension band-augmented counterparts. Simulations applied a 1,654 N

Bio-integrative screws versus metallic screws for calcaneus osteotomies: A non-inferiority randomized clinical trial.

BACKGROUND: Potential advantages of bio-integrative implants, such as lower removal rates, and superior bone interaction, have been advocated. While many biomechanical and histological reports sustain its structural and biological properties, few clinical studies have supported its use. This trial aimed to determine the bio-integrative screws' capacity to achieve non-inferior clinical outcomes as metallic screws in calcaneal osteotomies. Our main hypothesis was that both implants would not differ in healing or complications. METHODS: This was a single-center, parallel-group, randomized non-in

Detachment of titanium and fluorohydroxyapatite particles in unloaded endosseous implants.

The shape, surface composition and morphology of orthopaedic and endosseous dental titanium implants are key factors to achieve post-surgical and long-term mechanical stability and enhance implant osteointegration. In this study a comparison was made between 12 titanium screws, plasma-spray-coated with titanium powders (TPS), and 12 screws with an additional coating of fluorohydroxyapatite (FHA-Ti). Screws were implanted in the femoral and tibial diaphyses of two mongrel sheep and removed with peri-implant tissues 12 weeks after surgery. The vibrational spectroscopic, ultrastructural and morph

The effect of screw insertion configuration of Sinus Tarsi plate on biomechanical performance using finite element analysis.

Sinus Tarsi plates are used as implants for minimally invasive surgery of calcaneus bone fractures. This study evaluated the screw fixation patterns of Sinus Tarsi plates for optimal biomechanical performance. Six three-dimensional (3D) finite element models with different positional screws were evaluated for calcaneus fracture stabilization using Sinus Tarsi plates with 5, 6, and 7 holes. Walking stance conditions as heel strike, midstance, and push-off phases were used to compare loading. Results indicated that the equivalent (EQV) stress exhibited in the implant was higher than in the surro

The effect of additional reduction screw fixation for basicervical femoral neck fracture: a finite element analysis.

PURPOSE: Using a finite element (FE) model, we aimed to validate the efficacy of our new additional reduction screw technique to basicervical femoral neck fractures. METHODS: To investigate the biomechanical effects of an additional screw in an FE model of a femoral fracture, we recorded von Mises stress distributions. The fracture was simulated along the trochanteric line, with the additional screw positioned as inferior to the femoral neck as possible without invading the cortical bone. We compared models with and without the additional reduction screw to assess differences in external stre

Different patterns of bone fixation with hydroxyapatite and resorbable CaP coatings in the rabbit tibia at 6, 12, and 52 weeks.

Applying bioactive coatings on orthopedic implants can increase the fixation and long-term implant survival. In our study, we compared a resorbable electrochemically deposited calcium phosphate coating (Bonit®) to a thin (40 μm) plasma-sprayed hydroxyapatite (HA) coating, applied on grit-blasted screw-shaped Ti-6Al-4V implants in the cortical region of rabbit tibia, implanted for 6, 12, and 52 weeks. The removal torque results demonstrated stronger bone-to-implant fixation for the HA than Bonit-coated screws at 6 and 12 weeks. After 52 weeks, the fixation was in favor of the Bonit-coated screw

Unrecognized chondral penetration by a Herbert screw in the scaphoid.

The Herbert bone screw as developed by T. Herbert in the early 1980s has been used in the treatment of scaphoid fractures and nonunions. Reports on clinical efficacy have been accompanied by discussions of technical difficulties. This study determines if the curvilinear surface of the proximal pole of the scaphoid leads to errors in screw length and penetration into the joint. The screw was inserted in cadaveric wrists using the technique described by Herbert. Plain x-ray films were taken in the anteroposterior and lateral planes and fluoroscopy was used through 360 degrees. From the imaging s

Evaluation of screw pull-out from plate fixation of en bloc distal radius resection with ulnar reconstruction: Finite element analysis and comparison with experiments on Thiel cadavers.

Fractures of the distal radius often require surgical intervention, with plate fixation being a standard stabilization method. Screw loosening and pull-out propose significant complications, necessitating comprehensive understanding of fixation stability factors. This study introduces a novel approach by the combination of finite element analysis (FEA) and experimental investigations on Thiel cadavers to evaluate screw pull-out behavior from plate fixation in en bloc distal radius resection with ulnar reconstruction. In comparison with previous investigations that used computational modeling o

Prognosis of Patient With Resorbable Plates and Screws Under the Incision Site.

Various resorbable plates and screws were used for facial bone fractures because of several strengths. However, there are few studies on their clinical course and long-term follow-up concerning their degradation and resorption time. The authors present rare case of long term follow-up of resorbable plates and screws under the incision site.

Biomechanical analysis of protective plating configurations for interimplant femoral fracture prevention.

INTRODUCTION: Interimplant femoral fractures (IFFs), occurring between or adjacent to implants such as hip prostheses and intramedullary nails, pose complex treatment challenges, particularly in osteoporotic patients. Biomechanical research highlights the need to protect the interimplant region from high strains, however, optimal configurations for plate fixation remain unclear. This study analyzes strains in the interimplant region under various protective plate configurations. MATERIALS AND METHODS: Twelve synthetic proximal femora, mimicking osteoporotic bone, were instrumented with a prox

Manual extraction is superior to power tools when removing stripped titanium locking screws.

INTRODUCTION: Removal of screws from a titanium locking plate is often difficult once the screw has seized and the head is damaged. Such stripped screws are removed with an extraction screw, which can be used manually or on power. We aim to compare the extraction rates using both these methods. MATERIAL AND METHODS: We used 50, 3.5 mm diameter/20 mm long titanium locking screws. Fourth-generation saw bone models (radii) were used and 3.5 mm locking plates were fixed. The screws were deliberately over tightened and heads were damaged using a carbide drill to mimic stripped, seized screws. A le

Influence of the proximal screws of buttress plates on the stability of vertical femoral neck fractures: a finite element analysis.

BACKGROUND: The treatment of vertical femoral neck fractures (vFNFs) in young patients remains challenging, with a high complication rate by using traditional techniques. The use of cannulated screws (CSs) combined with a buttress plate represents an alternative approach for treating vFNFs. However, the biomechanical influence of the use or non-use of the proximal screws of buttress plates on vFNFs stability remains unclear. This study aims to analyse the biomechanics of buttress plate fixation with or without the use of proximal screws through finite element analysis (FEA) to further understa

Osseointegrated titanium implants. Requirements for ensuring a long-lasting, direct bone-to-implant anchorage in man.

A total of 2895 threaded, cylindrical titanium implants have been inserted into the mandible or the maxilla and 124 similar implants have been installed in the tibial, temporal or iliac bones in man for various bone restorative procedures. The titanium screws were implanted without the use of cement, using a meticulous technique aiming at osseointegration--a direct contact between living bone and implant. Thirty-eight stable and integrated screws were removed for various reasons from 18 patients. The interface zone between bone and implant was investigated using X-rays, SEM, TEM and histology.

Biomechanical stability of magnesium plate and screw fixation systems in LeFort I osteotomy: a three-dimensional finite element analysis.

BACKGROUND: Titanium (Ti-6Al-4 V) is used for fixation in LeFort I osteotomy, a procedure for treating midface deformities. This study assessed the biomechanical stabilities of two Mg alloys (WE43 and ZK60) as biodegradable alternatives and compared them against Ti using finite element analyses. The LeFort I osteotomy procedure was simulated, and various plate and screw configurations were tested. The maximum principal and peak von Mises stresses in the metal plates and bone screws were measured under four load conditions, and the stability was evaluated. RESULTS: The holes in the Mg screws, …

Evaluation of cortical bone thickness at the nasomaxillary and zygomaticomaxillary buttresses using cone-beam computed tomography imaging for Le Fort I osteotomy.

BACKGROUND/PURPOSE: Studies have indicated that 50%-55% of the population have malocclusion, and approximately 5%-10% require orthognathic surgery to correct this condition. Optimal placement of plates and screws significantly affects the success rate of the surgery and postoperative stability. This study evaluates the cortical thickness of the maxillary bone in the nasomaxillary and zygomaticomaxillary buttress regions in Taiwanese patients based on cone-beam computed tomography (CBCT) images. MATERIALS AND METHODS: 128 Patients undergoing Le Fort I osteotomy were selected for this study. Th

A comparative study of wire fixation and screw fixation in arthrodesis for the correction of hallux rigidus using an in vitro biomechanical model.

BACKGROUND: Arthrodesis of the great toe metatarsophalangeal joint for osteoarthritis usually involves internal fixation. Ideally, the fixation method should be reproducible, leading to a high rate of fusion and minimal complications. METHODS: This biomechanical study compares circumferential wire and screw fixation methods of arthrodesis. RESULTS: The two arthrodesis models have remarkably similar limits of elastic and plastic deformation. However, in the screw, failure by permanent deformation occurs at a load six times higher than the wire. CONCLUSIONS: The use of circumferential wire fi
