Are the irreversibly comatose still here? The destruction of brains and the persistence of persons.

When an individual is comatose while parts of her brain remain functional, the question arises as to whether any mental characteristics are still associated with this brain, that is, whether the person still exists. Settling this uncertainty requires that one becomes clear about two issues: the type of functional loss that is associated with the respective profile of brain damage and the persistence conditions of persons. Medical case studies can answer the former question, but they are not concerned with the latter. Conversely, in the philosophical literature, various accounts of personal ide

Landscape of Deceased Donor Pathways: A 4-Year Experience in the Tuscany Region.

OBJECTIVES: Donor pathways (donation after neurological death, ie, brain death; and donation after circulatory death ) are influenced, within each country, by several factors such as social trends and disease prevalence, which profoundly affect the mechanism and age of deaths in a population. An informed perspective on this process requires a rigorous analysis of donor efficiency (number of organs retrieved and transplanted, utilization rate, and organs per donor ) and procurement policies. MATERIALS AND METHODS: We assessed the procurement activity regarding deceased donors in the Tuscany re…

[Irreversible loss of brain function : Requirements and Clinical diagnosis].

Brain death (irreversible loss of brain function), according to German regulations, is investigated exclusively by qualified specialists in a strictly hierarchical three-step pattern and a four-eyes principle. In step 1 all necessary prerequisites are to be checked and the pathophysiology of brain damage has to be classified. Step 2 comprises the clinical investigation of reactivity to external stimuli and the upper, middle and lower brain stem reflexes including apnea testing. Step 3 exclusively checks for irreversibility of this condition. The latter is achieved by appropriate technical inve…

Automated screening for clinically ascertained loss of cerebral functions in patients with severe brain injury-study protocol for a cluster-randomized interventional trial.

BACKGROUND: The low organ donation rate in Germany is associated with deficiencies in the identification of patients at risk of developing brain death. An automated digital clinical support system (DETECT) was designed to prospectively identify intensive care patients who are at risk of developing brain death. The objective of the study is to evaluate the effectiveness of DETECT in the detection of patients with severe brain injury who progress toward brain death compared to standard practice without DETECT. METHODS: The study will follow a multicentre, cluster-randomized, controlled design c…

Warm ischemic time-dependent effects on oxidative stress, endoplasmic reticulum stress, and inflammation in cold storage human donor hearts.

OBJECTIVE: Direct procurement and cold preservation with Celsior solution are standard for donor hearts after brain death; however, whether Celsior solution can be used in donor after circulatory death hearts is unknown. Because myocardial oxidative stress, endoplasmic reticulum stress, and inflammation are pivotal in ischemic injury, we compared Celsior solution with normal saline with respect to these pathways in human donor after brain death and donor after circulatory death hearts, assessing the impact of warm ischemic time and cold ischemia. METHODS: Forty-seven human donor after brain d…

Right ventricular function in the donor heart.

OBJECTIVES: Early morbidity and mortality post cardiac transplantation is frequently caused by right ventricular failure; this is usually attributed to an elevated pulmonary vascular resistance in the recipient. Brain death in the donor is recognised as causing left ventricular dysfunction, but its effects on the right ventricle have not previously been studied. The aim of this study was to investigate right ventricular function following brain death, using a canine model. METHODS: The hearts of 33 dogs were instrumented with micromanometers, flow probes and dimension transducers to measure m…

Reframing "Good Death" in Taiwan: An Ethical Review of Legislation on Deceased Organ Donation.

Taiwan developed legal frameworks for organ transplantation earlier than many other East Asian countries and has achieved relatively success … en 1987 and 2021. It argues that Taiwanese law reframes death with organ donation-whether following brain death or cardiac arrest-as a form of "good death," integrating consent into broader end-of-life decision-making. By clarifying the legal and policy background, the analysis highlights how the government has sought to reconcile biomedical definitions of de …

Sudden unexpected death in epilepsy: Respiratory mechanisms.

Epilepsy is one of the most common chronic neurologic diseases, with a prevalence of 1% in the US population. Many people with epilepsy live normal lives, but are at risk of sudden unexpected death in epilepsy (SUDEP). This mysterious comorbidity of epilepsy causes premature death in 17%-50% of those with epilepsy. Most SUDEP occurs after a generalized seizure, and patients are typically found in bed in the prone position. Until recently, it was thought that SUDEP was due to cardiovascular failure, but patients who died while being monitored in hospital epilepsy units revealed that most SUDEP

Perspectives on the basis of seizure-induced respiratory dysfunction.

Epilepsy is an umbrella term used to define a wide variety of seizure disorders and sudden unexpected death in epilepsy (SUDEP) is the leading cause of death in epilepsy. Although some SUDEP risk factors have been identified, it remains largely unpredictable, and underlying mechanisms remain poorly understood. Most seizures start in the cortex, but the high mortality rate associated with certain types of epilepsy indicates brainstem involvement. Therefore, to help understand SUDEP we discuss mechanisms by which seizure activity propagates to the brainstem. Specifically, we highlight clinical a

The ethical obligation of the dead donor rule.

The dead donor rule (DDR) originally stated that organ donors must not be killed by and for organ donation. Scholars later added the requirement that vital organs should not be procured before death. Some now argue that the DDR is breached in donation after circulatory determination of death (DCDD) programs. DCDD programs do not breach the original version of the DDR because vital organs are procured only after circulation has ceased permanently as a consequence of withdrawal of life-sustaining therapy. We hold that the original rendition of the DDR banning killing by and for organ donation is

[Documentation of clinical death in preliminary death certificates].

The emergency physician acting within the framework of organised first-aid service, is not obliged to perform a post-mortem examination or to hold a coroner's inquest and to issue a death certificate. It is easy to understand why the relevant authorities must insist on the immediate submission of a death certificate. Such a certificate can be issued only if unequivocal signs of death definitely confirm that the decrease is an indisputable fact. To cope with this situation, a printed form has been designed which is used to document the occurrence of clinical death.

Defining Death: Lessons From the Case of Jahi McMath.

Death is defined biologically as the irreversible loss of the functioning of the organism as a whole, which typically occurs after the loss of cardiorespiratory function. In 1968, a Harvard committee proposed that death could also be defined neurologically as the irreversible loss of brain function. Brain death has been considered to be equivalent to cardiorespiratory arrest on the basis of the belief that the brain is required to maintain functioning of the organism as a whole and that without the brain, cardiorespiratory arrest and biological death are both rapid and certain. Over the past 2…

Quantitative analysis of GFAP- and S100 protein-immunopositive astrocytes to investigate the severity of traumatic brain injury.

Previous studies have shown that diffuse cortical astrocyte damage is seen in acute deaths due to brain injury and mechanical asphyxiation. The present study quantitatively investigated the number of astrocytes that showed GFAP- and S100-protein immunopositivity in the cerebral white matter and hippocampus at the sites distant from primary injury with regard to survival time, complication, and the immediate cause of death of brain injury cases. Autopsy cases of brain injury (8-48 h postmortem) comprising acute/subacute deaths (survival time, <3/6 h-3 days; n=27/42) and delayed deaths (survival

Dead donor rule unfazed: a response to Masek.

Lawrence Masek argues that the dead donor rule (DDR) is rationally unjustifed because it relies on mistaken assumptions about harm, killing and intention in medical ethics. I believe his case does not succeed, because his analysis relies on ambiguous terms such as 'harm' and 'death' and his action theory does not inspire trust, something that is integral to public policy. So long as upholding public confidence in transplantation practices depends on respecting the donor's life and minimising conflict of interests, the DDR will remain in place as the defining norm of deceased organ donation.

Novel chemical-physical autopsy investigation in sudden infant death and sudden intrauterine unexplained death syndromes.

Aim: Verify the presence of inorganic nanoparticle entities in brain tissue samples from sudden infant death syndrome (SIDS)/sudden intrauterine unexplained death syndrome (SIUDS) cases. The presence of inorganic debris could be a cofactor that compromises proper brain tissue functionality. Materials & methods: A novel autopsy approach that consists of neuropathological analysis procedures combined with energy dispersive spectroscopy/field emission gun environmental scanning electron microscopy investigations was implemented on 10 SIDS/SIUDS cases, whereas control samples were obtained from 10

Organismal death, the dead-donor rule and the ethics of vital organ procurement.

Several bioethicists have recently discussed the complexity of defining human death, and considered in particular how our definition of death affects our understanding of the ethics of vital organ procurement. In this brief paper, we challenge the mainstream medical definition of human death-namely, that death is equivalent to total brain failure-and argue with Nair-Collins and Miller that integrated biological functions can continue even after total brain failure has occurred. We discuss the implications of Nair-Collins and Miller's argument and suggest that it may be necessary to look for al

Misunderstanding death on a respirator.

As Tom Tomlinson's study indicates, there is considerable misunderstanding and confusion among physicians and nurses working in the area. Brain death is often not well-understood by healthcare professionals and hence inadequately explained to relatives, leaving them confused as to whether their loved-one is alive or dead. Whilst these healthcare professionals are using whole-brain death as the criterion of death, they have no clear understanding of the definition of death which underpins the brain-death criterion. Lack of this philosophical rationale makes the use of the brain-death criterion …
