Advanced atrioventricular block in a neonate with lupus erythematosus and anti-SS-A antibodies.

Recent studies suggested that the involvement of anti-SS-A and anti-SS-B antibodies in the pathogenesis of neonatal lupus erythematosus and congenital heart block. We here describe a female neonate with lupus erythematosus, advanced atrioventricular block, and a positive test for anti-SS-A antibodies. The findings of His-bundle recordings are presented.

The conduction system in hypoplasia of the aortic tract complex.

This is a study of the course of the conduction system in two cases of hypoplasia of the aortic tract complex, one with mitral stenosis and the other with mitral atresia. In both there was a posterior atrioventricular (AV) node which formed the AV bundle. In case 1 the bundle was short and bifurcated early. The branching bundle gave off a large left bundle branch (LBB), many Mahaim fibers to the septum and a small right bundle branch (RBB). In case 2 the AV node was divided into two parts which formed two posterior bundles which joined together to form a short branching bundle. Instead of a LB

[Atrial re-entry tachycardia].

By means of coupled atrial stimulation short bouts of atrial tachycardia could be elicited in a 61-year-old patient. As an explanation for this unusual response to stimulation a reentry phenomenon within the atria is postulated.

Conduction disturbances in the Kearns-Sayre syndrome.

The Kearns-Sayre syndrome is an uncommon disease, characterized by the triad of external ophthalmoplegia, retinitis pigmentosa, and heart block. Cardiac manifestations of this syndrome in a 31-year-old man are presented. Electrocardiogram revealed intermittent left bundle branch block and right bundle branch block with left anterior hemiblock. His bundle recording disclosed a prolonged HV interval. Clinical features of the syndrome are discussed and other published cases reviewed.

Therapeutic potential of conduction system pacing as a method for improving cardiac output during ventricular tachycardia.

BACKGROUND: Ventricular tachycardia (VT) reduces cardiac output through high heart rates, loss of atrioventricular synchrony, and loss of ventricular synchrony. We studied the contribution of each mechanism and explored the potential therapeutic utility of His bundle pacing to improve cardiac output during VT. METHODS: Study 1 aimed to improve the understanding of mechanisms of harm during VT (using pacing simulated VT). In 23 patients with left ventricular impairment, we recorded continuous ECG and beat-by-beat blood pressure measurements. We assessed the hemodynamic impact of heart rate and

Anatomopathological changes of the cardiac conduction system in sudden cardiac death, particularly in infants: advances over the last 25 years.

Sudden cardiac death (SCD) is defined as the unexpected death without an obvious noncardiac cause that occurs within 1 h of witnessed symptom onset (established SCD) or within 24 h of unwitnessed symptom onset (probable SCD). In the United States, its incidence is 69/100,000 per year. Dysfunctions of the cardiac conduction and autonomic nervous systems are known to contribute to SCD pathogenesis, even if most clinicians and cardiovascular pathologists lack experience with detailed examination of the cardiac conduction system and fail to recognize lesions that are crucial to explain the SCD its

New-onset intrinsic and paced QRS morphology of right bundle branch block pattern after atrioventricular nodal ablation: Longitudinal dissociation or anatomical bifurcation?

We performed left bundle pacing combined with atrioventricular nodal (AVN) ablation in a patient with persistent atrial fibrillation and refractory symptomatic heart failure. The major findings were new-onset intrinsic and paced QRS morphology of right bundle branch block (RBBB) pattern after AVN ablation which was performed at a more atrial site compared with the pacing site and the paced RBBB pattern could not be corrected regardless of the pacing output. Longitudinal dissociation cannot explain this observation, while anatomical separation could. We also confirm this was proximal left bundl

The junctional premature beat: an instructional exercise in modes of concealment.

?The relative brevity of the main His bundle refractory period compared with that of the A-V node above, and the trifascicular system below, makes it likely that premature beats originating in the His bundle will encounter physiologic delay, or block in both antegrade and retrograde modes. Two clinical cases of junctional premature beats are presented, which demonstrate many facets of concealment (antegrade, retrograde and bidirectional). Hitherto unreported is a ventricular echo which was induced by a junctional premature beat, the antegrade concealment of which was due to functional trifasci

[Paroxysmal supraventricular tachycardias due to re-entry via concealed retrograde atrio-His bypass (author's transl)].

Recently there have been proposed electrophysiologic criteria for the diagnosis of a concealed atrio-His bypass tract in patients with paroxysmal supraventricular tachycardia (PST). In order to verify the reliability of the proposed criteria--among the patients we studied for PST without ventricular preexcitation and in whom retrograde His bundle activation was recorded--we have chosen those with normal anterograde A-V conduction, with constant (or minimal increases) V-A interval during ventricular pacing and with short (less than 50 msec) and constant H2-A2 interval during ventricular prematu

[Bundle branch reentry tachycardia: a possible mechanism of flecainide proarrhythmia effect].

A 42-year old man with non-obstructive myocardiopathy complicated by paroxysmal atrial fibrillation treated with amiodarone (200 mg per day) received flecainide in daily doses of 400 mg for undocumented palpitations. Ten syncopes and numerous malaises occurred during the following two months. Electrophysiological testing was performed, showing prolongation of HV to 80 ms and discreet widening of QRS to 100 ms. Programmed atrial stimulation failed to demonstrate a second degree subnodal block and to induce tachycardia. In contrast, ventricular stimulation elicited a critical SH delay (260 ms),

[Unexpected sudden cardiac death. An evaluation of 1000 autopsies].

The aim of this study was to search for a cardiac cause of death after an otherwise negative medico-legal autopsy in 1,000 adults under 65 years of age with no previous history, who suffered a sudden death. Four hundred and seven coronary causes were established with 152 negative autopsies and 441 other cardiac lesions. These 441 cases comprised 193 cardiomyopathies, 106 His bundle lesions, 50 partial forms of Uhl's anomaly (or arrhythmogenic right ventricular dysplasia), 14 isolated mitral valve prolapses, 22 chronic valvular lesions, 8 myocarditis, 16 pulmonary embolisms, 11 chronic cor pulm

Left bundle branch pacing utilizing three dimensional mapping.

INTRODUCTION: Permanent His bundle pacing is feasible and effective in patients with atrioventricular block or left bundle branch block. However, pacing thresholds to capture the distal His bundle is often higher. Recently left bundle branch area pacing (LBBP) has been shown to be feasible by advancing the lead transvenously, deep into the interventricular septum to reach the left ventricular endocardial surface. In this article we describe the utility of three dimensional (3D) mapping to achieve LBBP. METHODS: Ensite Precision (Abbott) mapping system was used to perform LBBP. A decapolar cat

Left ventricular subendocardial pacing provides the same left ventricular synchrony and work efficiency as direct left bundle branch capture.

BACKGROUND: Left ventricular (LV) septal myocardial pacing (LVSP) is present in the areas where left bundle branch (LBB) capture is not feasible with high pacing output (general LVSP), or in LV subendocardial locations, where it transits from LBB pacing (LBBP) during the decremental output pacing (LV subendocardial pacing [LVSeP]). OBJECTIVE: This study aimed to compare electrical ventricular synchrony and acute hemodynamic response between LBBP and LVSeP. METHODS: Consecutive cardiac resynchronization therapy (CRT) patients with transition from nonselective left bundle branch capture to LVS

Left bundle branch vs biventricular pacing: mechanistic insights from a canine model.

BACKGROUND AND AIMS: Left bundle branch pacing (LBBP) for cardiac resynchronization therapy is emerging as an alternative pacing strategy to biventricular pacing (BiVP) in dyssynchronous heart failure (DHF). This study aimed to explore the differences in treatment effects between the two pacing modalities using electrocardiographic, echocardiographic, and molecular measurements. METHODS: Adult canines underwent left bundle branch ablation, then either followed by 6 weeks of atrial tachypacing (DHF group, n = 8), or 3 weeks of atrial tachypacing followed by another 3 weeks of BiVP (n = 8) or L

Long-Term Outcomes and Safety of His-Purkinje Conduction System Pacing in China: The ChiCSP Study.

BACKGROUND: Conduction system pacing (CSP), including His bundle pacing (HBP) and left bundle branch area pacing (LBBAP), offers a physiological alternative to conventional pacing. However, current evidence is limited by small sample sizes, short follow-up, and inconsistent LBBAP definitions. OBJECTIVE: This study evaluated the long-term outcomes, safety, and lead performance of CSP in a large multicenter cohort, and provided a precise LBBAP classification for investigating its impact on clinical outcomes. METHODS: This prospective registry-based study included patients receiving CSP at 5 Ch

Implantation of standard defibrillator leads in the left bundle branch area: Experience from an international cohort.

BACKGROUND: Left bundle branch area defibrillation (LBBAD) therapy is currently an off-label procedure. Nevertheless, it is a promising technique that reduces the number of leads in implantable cardioverter-defibrillator patients who require conduction system pacing. OBJECTIVE: This study aimed to collect real-world data on LBBAD implantation and follow-up. METHODS: Retrospective periprocedural and follow-up data on electrocardiographic and electrical parameters, transthoracic echocardiography data, and clinical outcome were assembled from 12 international centers. RESULTS: In total, 163 pa
