Endoscopic third ventriculostomy for management of vein of Galen aneurysmal malformation-associated hydrocephalus with spontaneous thrombosis: case report.

Vein of Galen aneurysmal malformations (VGAMs) are rare cerebrovascular anomalies due to lack of involution of the prosencephalic vein of Markowski in early gestation. Children born with such pathology may present with high-output cardiac failure, developmental delay, seizures, and hydrocephalus (Lasjaunias et al., 2006; Berenstein et al., 2010). Hydrocephalus may present due to aqueductal obstruction, intraventricular hemorrhage, or venous hypertension leading to absorptive deficit. Historical data suggests extremely high rates of complications associated with ventriculoperitoneal shunting wh

Pulmonary hypertension in hereditary haemorrhagic telangiectasia.

Hereditary haemorrhagic telangiectasia (HHT) is an autosomal dominant inherited disorder characterised by vascular malformations in predominantly the brain, liver and lungs. Pulmonary hypertension (PH) is increasingly recognised as a severe complication of HHT. PH may be categorised into two distinct types in patients with HHT. Post-capillary PH most often results from a high pulmonary blood flow that accompanies the high cardiac output state associated with liver arteriovenous malformations. Less frequently, the HHT-related gene mutations in ENG or ACVRL1 appear to predispose patients with HH

[Dynamics of the level of plasma vasopressin and its role in the regulation of water-electrolyte metabolism in patients following aortocoronary bypass].

Changes in plasma vasopressin, K+, Na levels, osmolality in blood and urine samples, as well as hemodynamic parameters, cardiac output and circulating blood volume including, have been assessed in 38 patients 3, 6, 18 and 48 hours after aortocoronary bypass surgery. It has been found that vasopressin blood level in the postoperative period is considerably (5-7 times) higher than physiological hormone level. The main reasons for vasopressin blood level increase are surgical stress, increase in blood plasma osmolality, hypokalemia, a drop of the arterial blood pressure and central venous pressur

Access flow reduction for cardiac failure.

High-flow in hemodialysis arteriovenous angioaccesses is frequent. It may result in high-output cardiac failure, which should be prevented by fistula flow reduction. The most frequently reported flow reduction procedure is banding but immediate and long-term results are questionable. Alternative techniques are related here with personal results. Juxta-anastomosis "Proximal Radial Artery Ligation" (PRAL) is a very simple and effective reduction technique for side-to-end radio-cephalic fistulas (82 patients; reduction rate [RR]: 54% ± 19%). For brachial artery-based fistulas flow reduction two v

Sandwich-like electrochemical immunosensor based on thionine-modified graphene aerogels for ultrasensitive assay of cardiac troponin I in human serum.

Cardiac troponin I (cTnI) is the primary disease marker for acute myocardial infarction (AMI). In this study, we successfully constructed an electrochemical immunosensor based on diazotization reaction to achieve ultrasensitive detection of cTnI. Taking their structural advantages, gold nanostars (Au NSs) were introduced to combine more capture antibodies. Through diazotization reaction, graphene aerogels (GAs) successfully captured thionine (Thi) molecules obtaining GAs-Thi. Specifically, the three-dimensional porous network structure of GAs was utilized to capture more Thi molecules and the

Non-contact assessment of cardiac physiology using FO-MVSS-based ballistocardiography: a promising approach for heart failure evaluation.

Continuous monitoring of cardiac motions has been expected to provide essential cardiac physiology information on cardiovascular functioning. A fiber-optic micro-vibration sensing system (FO-MVSS) makes it promising. This study aimed to explore the correlation between Ballistocardiography (BCG) waveforms, measured using an FO-MVSS, and myocardial valve activity during the systolic and diastolic phases of the cardiac cycle in participants with normal cardiac function and patients with congestive heart failure (CHF). A high-sensitivity FO-MVSS acquired continuous BCG recordings. The simultaneous

Effects of obstructive sleep apnea on hemodynamic parameters in patients entering cardiac rehabilitation.

PURPOSE: Obstructive sleep apnea (OSA) is a prevalent form of sleep-disordered breathing. Evidence suggests that OSA may lead to cardiac remodeling, although the literature is equivocal. Previous literature suggests a high percentage of individuals entering a cardiac rehabilitation (CR) program also have OSA. The objective of this study was to determine whether resting hemodynamic variables were altered in OSA subjects entering CR compared with those without OSA, as determined by impedance cardiography. METHODS: Subjects entering an early outpatient CR program were screened for OSA using an a

[Cardiovascular diseases during pregnancy. Considerations for the anesthesiologist].

Pregnancy causes substantial changes in the cardiovascular system: cardiac output (40%-50%) and blood volume (40%) increase whereas systemic peripheral resistance and arterial pressure decrease. The rise in progesterone and estrogen levels accompanying pregnancy stimulates the renin-angiotensin-aldosterone system. Increased prostaglandin production follows renin activation; simultaneously, the arterial vessels show diminished angiotensin sensitivity. The result is pronounced vasodilatation. Cardiovascular diseases remain the most important nonobstetric cause of maternal death. The physiologica

Portosystemic venous shunt in the patients with Fontan circulation.

Portosystemic venous shunt (PSVS) is a vascular anomaly between the portal and systemic veins, resulting in several critical complications. Although PSVS is often associated with congenital heart diseases, the clinical association between Fontan circulation and PSVS has not been elucidated. This study aimed to investigate the clinical features of Fontan patients with PSVS. Two hundred thirteen patients who underwent Fontan procedure are being followed up at Adult Congenital Heart Disease clinic in Kyushu University Hospital. Among them, 139 adult patients underwent cardiac catheterization betw

[Anesthesia for renal transplantation in patients with dilated cardiomyopathy: a retrospective study of 31 cases].

BACKGROUND AND OBJECTIVES: Dilated cardiomyopathy is a state of progressive enlargement of cardiac chambers mainly left ventricle which leads to decreased cardiac output and ultimately cardiac failure. Although it has multifactorial etiology, it is quite common in patients with end stage renal disease who require renal transplant surgery for their cure. Both conditions go side by side and anesthetic management of such cases poses real challenge to anesthesiologist. Strict monitoring and control of cardiac physiology is of utmost importance besides meticulous fluid management, thus preserving r

The role of mechanical support devices during percutaneous coronary intervention.

The practice of interventional cardiology has changed dramatically over the last four decades since Andreas Gruentzig carried out the first balloon angioplasty. The obvious technological improvements in stent design and interventional techniques have facilitated the routine treatment of a higher risk cohort of patients, including those with complex coronary artery disease and poor left ventricular function, and more often in the setting of cardiogenic shock (CS) complicating acute myocardial infarction (AMI). The use of mechanical cardiac support (MCS) in these settings has been the subject of

Effects of thyroid hormones on the heart.

Thyroid hormones have a significant impact on heart function, mediated by genomic and non-genomic effects. Consequently, thyroid hormone deficiencies, as well as excesses, are expected to result in profound changes in cardiac function regulation and cardiovascular hemodynamics. Thyroid hormones upregulate the expression of the sarcoplasmic reticulum calcium-activated ATPase and downregulate the expression of phospholamban. Overall, hyperthyroidism is characterized by an increase in resting heart rate, blood volume, stroke volume, myocardial contractility, and ejection fraction. The development

Hemodynamics in hypertension at rest and during exercise.

The results of the last 20 years have shown that the hemodynamic mechanisms behind the increased blood pressure in essential hypertension differ widely and depend at least on the age of the patients and the stage of the hypertensive disorder. The cardinal hemodynamic disturbance is increased vascular resistance seen in most vascular beds, but to a different extent in various other areas. Even in relatively young subjects (20-40 years) with mild hypertension, where the typical hemodynamic pattern is an increased cardiac index during rest with almost "normal" total peripheral resistance, exercis

Clinical determinants and consequences of left ventricular hypertrophy.

The left ventricle adapts to an increased afterload such as that produced by arterial hypertension with concentric left ventricular hypertrophy. However, this adaptive process can be modified by a variety of physiologic and pathophysiologic states. Progressive aging, black race, and perhaps disorders with an increased sympathetic outflow seem to accelerate left ventricular hypertrophy. Obesity and other high cardiac output states predominantly produce dilatation of the left ventricle, and their combination with arterial hypertension results in eccentric left ventricular hypertrophy. Similarly,

A fatal case of extensive arteriovenous malformation.

UNLABELLED: Arteriovenous malformation (AVM) is a rare congenital vascular anomaly that can progress to severe complications, including high-output heart failure and coagulopathy. We describe an 84-year-old woman with an extensive AVM extending from the right upper limb to the anterior chest. She was admitted with decompensated heart failure and elevated fibrin degradation products. Despite initial improvement, she experienced sudden cardiac arrest on hospital day 13. Resuscitation allowed transient electrocardiographic recording, which showed ST-segment elevation in precordial leads and aVR,

Adaptation of closed-chest infarction porcine model to adult Pannon minipigs.

The aim of the recent study was to collect data on the genotype characteristics of the Hungarian self-bred Pannon minipigs by adapting a standardized infarct model procedure. Closed chest AMI was induced by balloon occlusion for 90 min in the left anterior descendent coronary artery (LAD) in 24 adult intact female minipigs followed by reperfusion. To assess the left ventricular (LV) function, serial cardiac magnetic resonance imaging (cMRI) was performed prior to the experimental procedure, on day 3 post-AMI (72 ± 12 h), and at 1 month follow-up (Day 30 ± 2 days). Compared to baseline cMRI sca

Identification of an optimal CPR chest compression protocol.

In this study, we used a high-fidelity integrated computational model of the respiratory and cardiovascular systems to investigate cardiopulmonary resuscitation (CPR) after cardiac arrest in a virtual healthy subject. For the purpose of this work, a newly developed thoracic model has been integrated to the current model, to study the influence of external chest compressions upon the arrested circulation during CPR. We evaluated the chest compression (CC) parameters, namely, end compression force, compression rate, and duty cycle to optimize the coronary perfusion pressure and the systolic bloo
