OBJECTIVE: To report a rare case of spontaneous ovarian hyperstimulation syndrome (OHSS) associated with spontaneous pregnancy and a FSH-secreting pituitary adenoma.
DESIGN: Case report.
SETTING: University hospital.
PATIENT(S): A 32-year-old woman with spontaneous OHSS.
INTERVENTION(S): Transsphenoidal resection of the tumor.

Although many of the clinical features accompanying lupus anticoagulant positivity were documented in the early 1960s and many "non-lupus patients" were also published, it was not until the discovery of antibodies to cardiolipin in the 1980s that the existence and true ramifications of a distinct antiphospholipid syndrome was defined. A primary syndrome was in fact recognized in 1985 by the author while at the Hammersmith Hospital and comprised 25 patients who conformed to this new subset of disease, which has now overtaken lupus-associated (secondary) antiphospholipid syndromes in frequency.

BACKGROUND: To report a case of severe acute retinal necrosis syndrome (ARN) associated with initial features of orbital cellulitis.
METHODS: A 49-year-old immunocompetent woman presented with unilateral painful orbital inflammation associated with deep visual loss due to ARN. Polymerase chain reaction (PCR) for detection of viral DNA was applied to ocular fluids. Immunohistochemistry for detection of viral antigens and histopathologic studies were performed on orbital biopsy specimens.

BACKGROUND: The acute respiratory distress syndrome (ARDS) is a non-cardiogenic form of pulmonary oedema characterised by severe hypoxaemia refractory to oxygen therapy, with diffuse pulmonary infiltrates on chest radiographs. It can be precipitated by various serious medical and surgical conditions, including systemic autoimmune diseases. The "catastrophic" variant of the antiphospholipid syndrome (APS) is an accelerated form of this systemic autoimmune condition which results in multiorgan failure because of multiple small vessel occlusions.

This study investigated the role of both negative and positive cognitions in predicting panic severity in an international sample of patients diagnosed with panic disorder (with and without agoraphobia). We describe a 22-year-old woman who developed extensive and fatal bone marrow necrosis along with involvement of liver, lung, and central nervous system during pregnancy in the background of very high titers of antiphospholipid and anti-beta2 glycoprotein antibodies. Cardiac manifestation of antiphospholipid syndrome (APS) is mainly in the form of left-sided valvular insufficiency, intra-cardiac thrombi or coronary artery occlusion. Dilated cardiomyopathy is a rare but important cardiac manifestation of APS, and responds well to adequate anticoagulation and steroids. We describe a case in which APS presented with dilated cardiomyopathy and bilateral retinal artery thrombosis.

A response surface for critically ill patients is described. The coordinates of the three-dimensional response surface are two control variables, or state variables, related to aerobic and anaerobic metabolism, and a response variable, the A-VO2 difference. The data conform to a cusp catastrophe manifold. Cardiac insufficiency, adaptive response to stress, and sepsis may be distinguished by this model.
