[A case of spontaneous fusiform aneurysm in a middle cerebral artery branch which causes rapidly thrombosed formation in a short period].

A 61-year-old man presented with the complaint of headache. Investigations revealed a fusiform middle cerebral artery aneurysm at the M2 part. The formation of the aneurysm rapidly developed to a partially thrombosed aneurysm in the course of four months. As regards the treatment of the aneurysm, at first we tried surgery with a superficial temporal artery middle cerebral artery bypass (STA-MCA bypass) and trapping of the aneurysm. However, during the procedure, it was difficult to control bleeding from the temporal muscle, bone flap, and subdural space. Because of this, we finished the STA-MC

Rupture of a giant basilar aneurysm after saphenous vein interposition graft to the posterior cerebral artery. Case report.

A patient with a giant aneurysm of the top of the basilar artery presented with severe progressive symptoms of brain-stem compression. There was inadequate collateral circulation to the upper basilar system. She underwent exploration of the aneurysm and, after it was found to be impossible to clip, a tourniquet was placed on the basilar artery for future occlusion with the patient awake. A saphenous vein graft was interposed between the left external carotid and the left posterior cerebral arteries. The previously unruptured aneurysm bled fatally 36 hours after surgery, just before intended oc

Management of Complex Cerebral Aneurysms.

BACKGROUND: Complex intracranial aneurysms (CIAs) are challenging pathologies to treat. Characteristics making an aneurysm complex include dimension, history of previous treatment, location, absence of collateral circulation, intraluminal thrombosis, and calcification of the wall. The goal of the therapeutic process is to exclude the malformation from the cerebral circulation and both endovascular and surgical treatments are valid procedures. METHODS: Between 1990 and 2020, 170 CIAs were treated at our institution (33 ruptured, 137 unruptured). They were 3 prepetrous segment of the internal c

Angiogenic factors in moyamoya disease.

BACKGROUND AND PURPOSE: We previously reported that the level of basic fibroblast growth factor (bFGF) is high in cerebrospinal fluid (CSF) taken from patients with moyamoya disease. The present study investigated the levels of other angiogenic growth factors in the CSF of moyamoya patients and the clinical significance of bFGF in moyamoya disease. METHODS: The levels of bFGF, interleukin-8, platelet-derived growth factor, transforming growth factor-beta, endothelial growth factor, and vascular endothelial cell growth factor in CSF, taken from 38 patients with moyamoya disease and 16 patients

[Double extra-intracranial microvascular anastomoses in deconstructive surgeries for giant aneurysms of internal carotid and middle cerebral arteries].

BACKGROUND: Deconstructive interventions with exclusion of the parent artery are one of the possible methods of treatment for giant and fusiform aneurysms of cerebral arteries if aneurysm occlusion with coils or insertion of flow-diverting stents are technically impossible. Such interventions are associated with high risks of stroke. In such cases, surgical revascularization with double barrel extra-intracranial bypass may be advisable to reduce the risks of ischemic complications, perioperative mortality and improve long-term neurological outcomes. MATERIAL AND METHODS: The study included 25

Reappraisal of Microsurgical Revascularization for Anterior Circulation Ischemia in Patients with Progressive Stroke.

BACKGROUND: Intravenous thrombolysis using tissue plasminogen activator and endovascular treatment for acute ischemic stroke is becoming an established standard therapy. However, there is no consensus in the treatment of patients who are suffering from progressive neurologic symptoms in the later stages. The purpose of this study was to evaluate the safety and efficacy of microsurgical revascularization in such patients with progressive stroke. METHODS: We retrospectively reviewed the clinical and radiological records of 14 consecutive patients with progressive stroke who underwent emergency

"Bonnet bypass" using a radial artery interposition graft-how I do it.

BACKGROUND: Common carotid artery (CCA) occlusion frequently results in compromised hemodynamics of the ipsilateral hemisphere with risk of infarction but also a lack of blood flow in the ipsilateral superficial temporal artery (STA), requiring a more complex revascularization strategy than a standard extracranial-intracranial (EC-IC) bypass when indicated. METHOD: We describe the performance of a "Bonnet bypass" using a radial artery interposition graft (RAIG) from the contralateral STA to the ipsilateral middle cerebral artery (MCA). A groove is drilled in the skullcap to position the RAIG

The clinical outcome of emergency superficial temporal artery-to-middle cerebral artery bypass in acute ischemic stroke with large vessel occlusion.

The role of superficial temporal artery-to-middle cerebral artery (STA-MCA) bypass in acute ischemic stroke (AIS) is contentious, with no evidence in patients with AIS and large vessel occlusion (AIS-LVO). We conducted a cohort study to assess emergency STA-MCA outcomes in AIS-LVO and a meta-analysis to evaluate STA-MCA outcomes in early AIS treatment. From January 2018 to March 2021, we consecutively recruited newly diagnosed AIS-LVO patients, dividing them into STA-MCA and non-STA-MCA groups. To evaluate the neurological status and outcomes, we employed the National Institutes of Health Stro

Angiographic measurement of the superficial temporal artery for potential use in cerebral bypass surgery: a combined radiological and cadaveric study.

PURPOSE: This study aims to investigate the microsurgical anatomy of the superficial temporal artery (STA), explore the relationship between STA length and lumen diameter, and develop a reliable radiologic method for selecting STA segments for bypass surgery. METHODS: This study used 10 cadaveric dissections (20 STAs, both sides) and 20 retrospective radiological examinations (40 STAs, both sides), employing curved multiplanar reformation and flow color lookup table (CLUT) DICOM processing. Measurements included vessel lumen diameters and luminal cross-sectional thicknesses 3 mm proximal to t

Long-term Outcomes of Combined Revascularization Surgery for Moyamoya Disease in the Elderly: A Single Institute Experience.

The opportunity to treat older patients with Moyamoya disease (MMD) is increasing. However, the surgical outcomes after combined direct and indirect revascularization for elderly patients with MMD are not fully understood, especially for those ≥60 years old. This retrospective study examined 232 consecutive hemispheres of 165 adults with MMD who underwent combined revascularization. Clinical features and surgical outcomes were compared between the elderly (≥60 years) and nonelderly group (<60 years). Thirteen (5.6%, 64.4 ± 4.0 years old) and 219 hemispheres (94.4%, 40.2 ± 10.8 years old) were

Unilateral Myoclonus as an initial manifestation in Moyamoya disease with asymmetric vasculature.

Movement disorders have been identified as a rare early manifestation of the Moyamoya disease. Herein, we present a case of a 34-year-old man with a genetically confirmed Moyamoya disease who showed unilateral myoclonus as an initial manifestation. Neuroimaging studies showed prominent asymmetrically developed Moyamoya disease on the right hemisphere with near-complete loss of normal vessels while the left hemisphere was adjunctively fed with extension of posterior cerebral artery: uneven progression of vasculature. 99mTc-hexamethylpropylene amine oxime single-photon emission computed tomograp

Posterior Cerebral Artery Reconstruction by In-Situ Bypass with Superior Cerebellar Artery via Occipital Transtentorial Approach.

BACKGROUND: Posterior cerebral artery (PCA) aneurysms are relatively rare, and neck clipping is often difficult due to their fusiform shape. We report a case of a thrombosed aneurysm of the distal PCA for which curative trapping and parent artery reconstruction by in situ bypass were performed through an occipital transtentorial approach (OTA). CASE DESCRIPTION: A 67-year-old woman had been suffering from numbness in the right face and limbs for 4 months. Radiologic imaging demonstrated a thrombosed aneurysm on a distal portion of the left PCA. Curative trapping of the aneurysm and in-situ by

[Successfully performed STA-MCA anastomosis using a branch of the STA scarcely visible on cerebral angiography in two patients with ischemic type moyamoya disease].

Preparation of a scalp artery as a donor is the first step in the procedure of direct anastomosis for the treatment of ischemic type moyamoya disease. In some cases, the scalp artery is not visualized well on cerebral angiograms. Another scalp artery must be used as a donor or it is interposed between the proximal portion of the firstly selected scalp artery and a branch of the middle cerebral artery in that condition. Skin incision should be planned to include multiple scalp arteries. On cerebral angiograms, in the present two cases, the diameter and length of branches of the STA was thought

Symptomatic non-atherosclerotic bilateral extracranial vertebral artery occlusion treated with extracranial to intracranial bypass: case report.

Posterior fossa ischemia is not a very frequent situation. It is responsible for about 25% of all ischemic strokes, and the vast majority of the cases are related to atherosclerotic stenosis of the vertebral and/or basilar arteries. Acute ischemia can also occur in the setting of vertebral artery dissection, traumatic or spontaneous. Recently, blunt trauma has been increasingly recognized as a cause for craniocervical artery injury. The management options for both traumatic and atherosclerotic lesions of the posterior fossa are still under debate. We present a case of a delayed onset of hemody

Giant basilar artery aneurysms encorporating the posterior cerebral artery: bypass surgery and coil occlusion--two case reports.

Giant aneurysms of the basilar artery are rare. With a diameter of 25 mm or more they are often partially thrombosed and show atheromatous plaques. There are some problems in the treatment especially when the aneurysm is broadbased with bulbous origin encorporating the origin of the posterior cerebral artery (PCA). In many of these cases neither operative clipping alone nor coil embolization alone will be practical without causing an ischemia in the depending brain areas. We will report about two patients with giant aneurysms of the basilar artery involving the origin of the PCA and a combined

Thrombectomy and stenting of pseudoaneurysm from transcarotid artery revascularization.

BACKGROUND: Transcarotid artery revascularization (TCAR) is becoming an increasingly popular treatment of carotid stenosis. Despite this rapid adoption, little in the literature describes the associated complications of this procedure. CASE DESCRIPTION: We report a case of a left M1 large-vessel occlusion following treatment of symptomatic, high-grade carotid stenosis with a TCAR procedure approximately three weeks earlier. The initial angiography demonstrated a pseudoaneurysm in the left common carotid artery at the site of TCAR access with a distal clot in the carotid stent. The clot within
