Catheter infection control in parenteral nutrition.

Catheter sepsis rates related to total parenteral nutrition are variable and depend on several patient-specific factors. These factors include the presence of immunosuppression or critical illness, the use of multiple intravascular catheters, and bacterial translocation. Catheter-related sepsis may present in the patient as fever, chills, change in mental status, hypotension, and leukocytosis. In patients with suspected catheter-related infection whose peripheral blood cultures do not grow the same organism as a blood culture drawn from the catheter, a guidewire exchange of the catheter has be…

Staphylococcus aureus endocarditis in drug addicts: report of 2 cases.

Two young men presented with prolonged hectic fever and chills followed by chest pain, dyspnea and hemoptysis. The chest films revealed multiple lung infiltrates, and blood cultures yielded Staphylococcus aureus. Echocardiographic examination confirmed the diagnosis of tricuspid valve endocarditis. Multiple punctate lesions in the bilateral inguinal areas and dragon tattoos over the forechest gave rise to the suspicion of drug abuse. After prolonged antimicrobial therapy, bacteremia was eliminated, and elective vegetectomy and valvuloplasty were performed on one of the patients. The other one …

Acute rhabdomyolysis associated with an echovirus 9 infection.

A 28-year-old long-distance runner experienced fever, chills, and brown urine 24 hours after a six-mile run. This was accompanied by a large rise in the creatine phosphokinase level and a rise in echovirus 9 titers from 1:16 to greater than 1:256 during a two-week period. A muscle biopsy specimen showed acute rhabdomyolysis, but no viral inclusion particles. Muscle energy metabolism analysis demonstrated no abnormalities. The patient was treated with forced saline diuresis, and he maintained normal renal function. He subsequently returned to long-distance running and has remained well for one …

S2k guidelines for skin and soft tissue infections Excerpts from the S2k guidelines for "calculated initial parenteral treatment of bacterial infections in adults - update 2018".

These first German S2k guidelines for bacterial skin and soft tissue infections were developed as one chapter of the recommendations for "calculated initial parenteral treatment of bacterial infections" issued under the auspices of the Paul-Ehrlich Society, of which the main part is presented here. Well-calculated antibiotic therapies require precise diagnostic criteria. Erysipelas is defined as non-purulent infection considered to be caused by beta-hemolytic strepto-cocci. It is diagnosed clinically by its bright-red erythema and early fever or chills at disease onset. Penicillin is the treat…

Frequent bacterial skin and soft tissue infections: diagnostic signs and treatment.

Skin and soft tissue infections rank among the most frequent infections worldwide. Classic erysipelas is defined as a non-purulent infection by beta-hemolytic streptococci. The typical signs are tender, warm, bright erythema with tongue-like extensions and early systemic symptoms such as fever or at least chills. Erysipelas always and best responds to penicillin. Limited soft tissue infection or limited cellulitis are the terms we have introduced for infections frequently caused by S. aureus and often originating from chronic wounds or acute trauma. Clinically, they are marked by tender, eryth…

Increased 18F-FDG Uptake in the Spleen and Multiple Lymph Nodes in Dengue Fever.

A 62-year-old man underwent a whole-body FDG PET/CT for annual cancer screening. By an interview, he had an epigastric pain, and his body temperature was 37.0°C on the day. He just came back home from a travel to Southeast Asia 1 week ago and had presented with chill, high fever (temperature, 39.6°C), arthralgia, myalgia, and skin rash a few days before. Dengue fever was diagnosed by detecting dengue virus type 1 genome and antibody to the virus accompanied by thrombocytopenia and leukopenia. PET/CT examination revealed increased FDG uptake in the spleen and multiple lymph nodes.

SPONGIFORM DISEASE: Experts Downplay New vCJD Fears.

The chilling scenario of getting "mad cow disease" from eating products of other animals besides just cows was splashed across the mainstream British press last week based on a report from a leading U.K. lab that prions--abnormal proteins linked to bovine spongiform encephalopathy and its human form, variant Creutzfeldt-Jakob disease--can jump from one species to another more easily than previously believed. Although some experts say the findings raise concern about a threat to human health, others decry the media frenzy and contend that the new research adds little to what is known about the

Cold exposure and immune function.

The influence of cold exposure on immune function is reviewed. Data obtained mainly on small mammals suggest that the acute effect of severe chilling is a suppression of several cellular and humoral components of the immune response, including a decrease of lymphocyte proliferation, a down-regulation of the immune cascade, a reduction of natural killer (NK) cell count, cytolytic activity, activation of complement, and the induction of heat shock proteins. However, adaptation to a given cold stimulus appears to develop over the course of 2-3 weeks. Further work is needed to examine interactions

A Rare Case of Locally Acquired Malaria in Lebanon: A Public Health Concern.

Malaria, a mosquito-borne disease caused by five plasmodium species, still has a life-threatening risk worldwide. Clinical manifestations can range from mild nonspecific symptoms to severe disease. In non-endemic regions, sporadic cases frequently pose significant challenges to health workers as delayed diagnosis can lead to serious consequences and even death. In this study, we report the first documented case of locally transmitted malaria in Lebanon involving a patient with no recent travel history presenting with fever, chills, and headache. The patient was found to have anemia and thrombo…
