Vibrio cholerae amino acids go on the defense.

Gram-negative bacteria remodel their surfaces to interact with the environment, particularly to protect pathogens from immune surveillance and host defenses. The enzyme AlmG is known to be involved in remodeling the Vibrio cholerae surface, but its specific role was not clear. A new study characterizes AlmG at the molecular level, showing it defies phylogenetic expectations to add amino acids to lipopolysaccharide (LPS). This LPS modification plays a pivotal role in V. cholerae resistance to antimicrobial peptides, weapons of the innate immune system against infections.

Mesenteric Panniculitis Associated With Vibrio cholerae Infection.

We report the first case of acute Vibrio cholerae infection with computed tomography (CT) changes consistent with mesenteric panniculitis (MP). A 78-year-old Indian man returned from overseas travel with progressively severe nausea, vomiting, abdominal pain, and watery diarrhea. His stool tested positive twice for Vibrio cholerae. CT revealed prominent lymph nodes and a hazy mesentery consistent with MP. Antibiotic treatment resulted in complete resolution of MP on follow-up CT 8 months later. In the setting of Vibrio cholerae infection, the CT finding of MP appears to be the result of a immun…

Mechanisms of Cholera Toxin in the Modulation of TH17 Responses.

Numerous studies have shown that TH17 cells and their signature cytokine IL-17A are critical to host defense against various bacterial and f … es: identification of TH17-specific antigens and TH17-skewing adjuvants. Studies have revealed that cholera toxin (CT) induces a potent Th17 response following vaccination. Antigen vaccination along with CT induces a robust TH17 response, which is sometimes accompanied by TH1 responses. Due to the toxicity of CT, it is hard to apply CT in a clinical setti …

Use of Vibrio cholerae vaccine in an outbreak in Guinea.

BACKGROUND: The use of vaccines to prevent and control cholera is currently under debate. Shanchol is one of the two oral cholera vaccines prequalified by the World Health Organization; however, its effectiveness under field conditions and the protection it confers in the first months after administration remain unknown. The main objective of this study was to estimate the short-term effectiveness of two doses of Shanchol used as a part of the integrated response to a cholera outbreak in Africa. METHODS: We conducted a matched case-control study in Guinea between May 20 and October 19, 2012. …

Diagnosis of Vibrio cholerae O1 infection in Africa.

Isolation of Vibrio cholerae O1 is necessary for cholera outbreak confirmation. Rapid diagnostic testing of fecal specimens, based on lipopolysaccharide detection of V. cholerae O1 or O139, may assist in early outbreak detection and surveillance. Cary-Blair transport medium is recommended for specimen transport. Filter paper, although used in epidemics, needs evaluation against rectal swab specimens. Fecal specimens are subcultured onto selective and nonselective media, including 5% blood agar and TCBS agar, for detection of V. cholerae O1 or O139. Suspicious, oxidase-positive isolates are ser…

The greatest steps towards the discovery of Vibrio cholerae.

In the 19th century, there was extensive research on cholera: the disease was generally attributed to miasmatic causes, but this concept was replaced, between about 1850 and 1910, by the scientifically founded germ theory of disease. In 1883, Robert Koch identified the vibrion for the second time, after Filippo Pacini's discovery in 1854: Koch isolated the comma bacillus in pure culture and explained its mode of transmission, solving an enigma that had lasted for centuries. The aim of this article is to reconstruct the different steps towards the explanation of cholera, paying particular atten…

An overview of cholera vaccines and their public health implications.

PURPOSE OF REVIEW: The current cholera pandemic now involves almost the entire developing world and represents an important global challenge. Though improved water and sanitation remain the mainstays of cholera prevention efforts, major improvements to infrastructure continue to be a goal far out of reach for many of those affected and near-term interventions, including vaccines, need consideration. RECENT FINDINGS: Prolonged and frequent epidemics, increased antimicrobial resistance, and heightening awareness of the role of climate change in disease burden have returned cholera to the forefr…

Preventing maritime transfer of toxigenic Vibrio cholerae.

Organisms, including Vibrio cholerae, can be transferred between harbors in the ballast water of ships. Zones in the Caribbean region where distance from shore and water depth meet International Maritime Organization guidelines for ballast water exchange are extremely limited. Use of ballast water treatment systems could mitigate the risk for organism transfer.

Vibrio cholerae: lessons for mucosal vaccine design.

The ability of Vibrio cholerae to persist in bodies of water will continue to confound our ability to eradicate cholera through improvements to infrastructure, and thus cholera vaccines are needed. We aim for an inexpensive vaccine that can provide long-lasting protection from all epidemic cholera infections, currently caused by O1 or O139 serogroups. Recent insights into correlates of protection, epidemiology and pathogenesis may help us design improved vaccines. This notwithstanding, we have come to appreciate that even marginally protective vaccines, such as oral whole-cell killed vaccines,…

Modeling the role of bacteriophage in the control of cholera outbreaks.

Cholera is a waterborne diarrheal disease that continues to plague the developing world. Individuals become infected by consuming water from reservoirs contaminated by virulent strains of the bacterium Vibrio cholerae. Epidemiological and environmental observations of a cholera outbreak in Dhaka, Bangladesh, suggest that lytic bacteriophage specific for V. cholerae may limit the severity of cholera outbreaks by killing bacteria present in the reservoir and in infected individuals. To quantify this idea and generate testable hypotheses, we analyzed a mathematical model that combines the epidemi…

Drosophila melanogaster is susceptible to Vibrio cholerae infection.

Infection of Drosophila melanogaster adults with 6 Vibrio species revealed that V. cholerae was lethal (100% mortality) within 20 h as a result of systemic infection. Avirulent infection by V. vulnificus restricted the subsequent virulent infection by V. cholerae. The immediate transcription of antimicrobial peptides (AMPs), most notably Attacin A, was delayed in V. cholerae infection compared to V. vulnificus infection. Ectopic expression of Attacin A and Metchnikowin enhanced the survival of D. melanogaster upon V. cholerae infection. These results suggest that AMPs are important in the resp…

Acidosis in a patient with cholera: a need to redefine concepts.

A patient presented with cholera and a severe degree of ECF volume contraction. Despite large losses of bicarbonate (HCO3-)-containing diarrhoeal fluid, laboratory acid-base values were remarkably close to normal. A detailed analysis emphasizing principles of physiology and a quantitative approach provided new insights and eventually better definitions of metabolic and respiratory acidosis. A shift in focus from HCO3- concentration to HCO3- content in the extracellular fluid (ECF) compartment revealed the presence of metabolic acidosis. Central to this analysis was an emphasis on the haematocr…

Vibrio cholerae O1 serotype Ogawa in a neonate.

In India, cholera is endemic and affects usually the 3 to 5-year-old age group. There have been occasional reports in the neonatal period with Vibrio cholerae O139 Bengal. We report here a case of Vibrio cholerae O1 diarrhea in a 2-day-old, breastfed male, who had been delivered in the hospital and developed severe dehydration.

Expression of Shigella sonnei lipopolysaccharide in Vibrio cholerae.

Making use of a newly designed mobilizable suicide vector, the genetic determinants encoding Shigella sonnei lipopolysaccharide (LPS) were stably integrated into the chromosome of the live attenuated Vibrio cholerae vaccine strain CVD103-HgR. Expression studies showed that the production of complete S. sonnei O-polysaccharide (O-PS)-bearing LPS was limited in bivalent recombinant strains that were also proficient in the synthesis of the host-encoded Inaba O-PS. Conversely, high amounts of LPS carrying S. sonnei O-PS are produced in monovalent Inaba-deficient derivatives, even in those strains …

Acute acalculous cholecystitis due to Vibrio cholerae.

The case of a 57-year-old woman admitted with symptoms and signs suggesting an intestinal infection caused by Vibrio cholerae, and who also developed a clinical picture compatible with acute cholecystitis, is presented. Cholera was diagnosed by examining a fresh sample of stools and cultures. An abdominal sonogram disclosed signs of acute acalculous cholecystitis. She underwent cholecystectomy, and cultures of a clear fluid and a "milky" sediment found within the gallbladder were also positive for V. cholerae. This microorganism was seen at the gallbladder mucosa microscopically. The strain wa…

Thymine auxotrophy as an attenuating marker in Vibrio cholerae.

Vibrio cholerae CVD102 is a thymine-dependent auxotroph of CVD101, a cholera toxin A-B+ candidate live oral cholera vaccine. Previous clinical experience with these strains suggested that, by restricting intestinal growth, thymine auxotrophy is attenuating for V. cholerae. Studies in the infant mouse cholera model cast doubt upon this conclusion however. Stable thyA mutants selected from each of three pathogenic strains showed unimpaired gut colonization in mixed-infection competition experiments. Similar results were obtained using thyA mutants selected from two atoxigenic strains, including …
