Extracellular vesicles are the primary source of blood-borne tumour-derived mutant KRAS DNA early in pancreatic cancer.

Up to now, the field of liquid biopsies has focused on circulating tumour DNA and cells, though extracellular vesicles (EVs) have been of increasing interest in recent years. Thus, reported sources of tumour-derived nucleic acids include leukocytes, platelets and apoptotic bodies (AB), as well as large (LEV) and small (SEV) EVs. Despite these competing claims, there has yet to be a standardized comparison of the tumour-derived DNA associated with different components of blood. To address this issue, we collected twenty-three blood samples from seventeen patients with pancreatic cancers of know

Role of Cell-Free DNA and Deoxyribonucleases in Tumor Progression.

Many studies have reported an increase in the level of circulating cell-free DNA (cfDNA) in the blood of patients with cancer. cfDNA mainly comes from tumor cells and, therefore, carries features of its genomic profile. Moreover, tumor-derived cfDNA can act like oncoviruses, entering the cells of vulnerable organs, transforming them and forming metastatic nodes. Another source of cfDNA is immune cells, including neutrophils that generate neutrophil extracellular traps (NETs). Despite the potential eliminative effect of NETs on tumors, in some cases, their excessive generation provokes tumor gr

Cell-Free DNA Release Kinetics and Fragmentation Reflect Treatment Response and Resistance In Vitro.

BACKGROUND: Circulating tumor DNA (ctDNA) has emerged as a clinically valuable biomarker for cancer detection, treatment monitoring, and minimal residual disease assessment. Despite its growing clinical utility, the biological mechanisms governing ctDNA release remain incompletely understood. The objective of this study was to investigate how chemotherapy-induced cytotoxicity and chemoresistance influence the kinetics and fragmentation patterns of cfDNA released by cancer cells in vitro. METHODS: Human lung adenocarcinoma (A549) and esophageal adenocarcinoma (FLO-1, OE19) cell lines were used…

Stimuli-Induced Upgrade of Nuclease-Resistant DNA Nanostructure Composed of a Single Molecular Beacon for Detecting Mutant Genes.

As a kind of cell-free DNA in the bloodstream liberated from tumor cells, circulating tumor DNAs (ctDNAs) have been recognized as promising biomarkers in the field of early cancer diagnosis. However, robust, sensitive, and accurate detection of ctDNA in serum remains extremely challenging, especially toward the mutant KRAS gene, one of the most frequently mutated genes. Although DNA oligonucleotides as emerging practical signaling materials have been developed as sensitive and accurate tools, some intrinsic defects need to be overcome, such as fragility in complex biological environments. In t…

Sensitive cell-free tumor DNA analysis in supernatant pleural effusions supports therapy selection and disease monitoring of lung cancer patients.

Supernatant pleural effusions (PE) have shown to be a valuable source for the detection of driver mutations in circulating tumor DNA (ctDNA). In this prospective study, the clinical value of ctDNA analysis in supernatant PE to support therapy selection and disease monitoring in lung cancer patients is assessed. Paired PE and plasma samples were collected from lung cancer patients before initiation of therapy (N = 2) and from EGFR positive patients during therapy (N = 3). Supernatant PE and plasma were tested for mutations in EGFR, KRAS and BRAF by droplet digital PCR. In PE of two patients wit…

Neutrophil extracellular traps in tumor microenvironment for radiotherapy: friend or foe?

Neutrophil extracellular traps (NETs) are chromatin-protein complexes released by neutrophils that have dual roles in the tumor microenvironment (TME), participating in anti-tumor immunity and driving malignant progression by promoting tumor proliferation, metastasis, and immune escape. Radiotherapy (RT) exerts anticancer effects by inducing DNA damage and microenvironmental remodeling, but the inflammatory response it triggers can activate the formation of NETs, which exacerbate radioresistance by degrading the extracellular matrix, encapsulating circulating tumor cells, inhibiting CD8+ T-cel

Integrated inference of cancer gene expression from cell-free plasma chromatin.

Gene expression is a defining determinant of tumor identity, behavior, and therapeutic response, yet remains challenging to measure noninvasively. Here, we introduce APEX (Associating Plasma Epigenomic features with eXpression), a framework for inferring expression from circulating cell-free chromatin. Trained on ~270,000 gene-sample pairs from matched tumor RNA-seq and plasma cfChIP-seq across multiple cancers and validated on >15 unseen cancer subtypes, APEX accurately infers cancer gene expression across a range of tumor fractions and outperforms existing plasma-based approaches by integrat

Liquid biopsy for early diagnosis of non-small cell lung carcinoma: recent research and detection technologies.

Liquid biopsy, an innovative method for early diagnosis of cancer, has changed the traditional method of diagnosing lung cancer and is considered a feasible auxiliary diagnostic tool. To date, various reports emphasize the need for non-small cell lung carcinoma (NSCLC), both with higher incidence and mortality and less effective treatments; thus, emphasizing the need for early detection of NSCLC for improved patient outcomes. Invasive tissue biopsy is a common diagnostic tool that is usually extracted from the primary tumor to indicate molecular composition. In comparison, liquid biopsy taken

Combined Point Mutation in a Single Allele of the TP53 Gene Potentiates the Oncogenic and Transcriptional Properties of TP53 in Lung Cancer.

Although multi-hit TP53 mutations correlate with rapid disease progression, treatment resistance, recurrence, and poor survival, their occurrence within an individual TP53 gene molecule (cbTP53) remains unexplored. Consequently, the impact of cbTP53 on protein conformation, transcriptional architecture, and oncogenicity is unknown. We show that cbTP53 with combined mutations (Y220C+R248Q+R282Q) does not drastically alter local stability or structural conformation, but significantly affects TP53 binding to DNA promoters comparing it to a single Y220C mutation. Functionally, cbTP53 promotes dive

Failure of the Galleri multi-cancer detection trial to meet its primary endpoint.

Cancer screening has been at the forefront of the war against cancer for decades. Yet professional organizations such as the U.S. Preventive Services Task Force (USPSTF) recommend cancer screening for only a handful of cancers (cervical, lung, colon, breast, prostate). Screening for other serious cancers (ovarian, pancreatic, etc.) is not recommended, at least for individuals with low/moderate risk. The discovery of circulating tumor DNA (ctDNA) as a non-invasive biomarker for diverse cancers prompted many companies to develop screening tests for one or more cancer types. Particularly, one tes…

AR and Beyond: ctDNA Maps Resistance Evolution in mCRPC.

Longitudinal plasma cell-free DNA profiling from the phase 3 Alliance A031201 trial revealed divergent resistance trajectories in metastatic castration-resistant prostate cancer. Rapid progressors harbored non-androgen receptor (AR) alterations suggesting intrinsic resistance, whereas delayed progressors showed progressive AR amplifications, structural rearrangements, and extrachromosomal DNA-associated evolution. These findings support biomarker-guided strategies targeting AR-dependent and AR-independent disease states. See related article by Valentín López et al., p. 2413.
