Coccidioidomycosis.

Coccidioides immitis and C posadasii are pathogenic dimorphic fungi responsible for causing coccidioidomycosis in the Southwestern United States and Central and South America. Antifungal therapy is beneficial and entails careful periodic assessment with therapies ranging from none or short courses of therapy to prolonged antifungal therapy. Factors that influence the decision to treat are the duration or severity of infection, radiographic findings, anticomplementary titers, presence of underlying immunosuppression, and comorbidities. Cure for disseminated infection is infrequent with current …

Coccidioidomycosis Testing Among Patients Hospitalized With Community-Acquired Pneumonia.

Among 1141 inpatients from Arizona with community-acquired pneumonia, 865 (75.8%) were tested for Coccidioides infection and 85 (9.8%) had positive results. Patients more commonly underwent testing if they had concerning radiographs, fever, dyspnea, or fatigue. Patients with night sweats, rash, headache, or eosinophilia more commonly had positive results.

Systemic fungal infections: diagnosis and treatment. I. Sporotrichosis.

Sporotrichosis is a fungal infection caused by the dimorphic fungus Sporothrix schenckii. Although this fungal microorganism is labeled a true pathogen, its virulence is less than that of other fungal pathogens, such as Coccidioides immitis, Histoplasma capsulatum, and Blastomyces dermatitidis. This article discusses its clinical manifestations, experimental models, diagnosis, and therapy.

Expanding understanding of epidemiology of coccidioidomycosis in the Western hemisphere.

Coccidioidomycosis is a disease of both national and worldwide importance that is most often diagnosed in nonendemic regions. The endemic region for Coccidioides spp. lies exclusively in the Western Hemisphere. Coccidioides spp. has long been identified in semiarid areas of the United States and Mexico, and endemic foci have been described in areas of Central and South America. Infection is usually the result of activities that cause the fungus to become airborne and inhaled by a susceptible host. Underlying medical diseases that affect T cell function are known to increase the risk of dissemi…

Comparative efficacy of forphenicinol, cyclosporine, and amphotericin B in experimental murine coccidioidomycosis.

Cohorts of ten mice, uninfected and infected (intratracheal injection of coccidioidal arthroconidia), were treated for 23 days by intravenou … e higher doses, with no net loss of weight; however, all cultures of organs yielded heavy growth of Coccidioides immitis. With amphotericin B, all mice survived and gained weight; four mice from each of the two treatment groups yielded modest growth of C. immitis from the lungs, and one mouse of each group yielded sparse growth from liver and spleen. The parad …

Disseminated coccidioidomycosis in a liver transplant recipient with negative serology: use of polymerase chain reaction.

Coccidioidomycosis has been previously described in recipients of solid organ transplantation, especially in patients who have lived in or have visited areas endemic for Coccidioides spp. We present a case of coccidioidomycosis in a liver transplant recipient with several unique aspects, including negative serology and positive polymerase chain reaction results.

Coccidioidomycosis fungal infection in the hand mimicking a metacarpal enchondroma.

Coccidioidomycosis, an infection caused by the fungus Coccidioides immitis, rarely affects the hand, but we report an unusual case which mimicked the radiological appearance of an enchondroma in the metacarpal. Curettage and bone grafting in combination with long-term antifungal therapy are necessary for successful treatment of coccidioidomycosis of the hand.

Protection against lethal murine coccidioidomycosis by a soluble vaccine from spherules.

The formaldehyde-killed, whole-spherule vaccine, which is protective against lethal challenge of laboratory animals with Coccidioides immitis, was fractionated. It yielded a soluble, multicomponent, subcellular fraction termed the 27K vaccine. This vaccine, when it was accompanied by adjuvant, protected mice against lethal intranasal and intravenous challenge with C. immitis.

Reactivation of coccidioidomycosis in a fit American visitor.

The case history is presented of an American visitor, known to have had primary coccidioidomycosis previously, who became very unwell during a visit to the UK. Despite consideration of reactivation of coccidioidomycosis from the outset, other pathogens were identified while Coccidioides immitis was not initially, leading to a delay in treatment.

Novel Presentation of Coccidioidomycosis with Myriad Free-Floating Proteinaceous Spheres in the Pericardial Sac of a Southern Sea Otter (Enhydra lutris nereis).

A southern sea otter (Enhydra lutris nereis) stranded dead in central California, USA, with a distended pericardial sac containing thousands of free-floating proteinaceous masses. Serology, fungal culture, PCR, and sequencing confirmed the etiology of this novel lesion as Coccidioides immitis. Range expansion of this zoonotic pathogen is predicted with climate change.

Coccidioidomycosis Seroincidence and Risk among Military Personnel, Naval Air Station Lemoore, San Joaquin Valley, California, USA1.

We conducted a retrospective cohort study that tested 2,000 US military personnel for Coccidioides antibodies in a disease-endemic region. The overall incidence of seroconversion was 0.5 cases/100 person-years; 12.5% of persons who seroconverted had illnesses requiring medical care. No significant association was found between demographic characteristics and seroconversion or disease.

Coccidioidomycosis of cervical lymph nodes in an HIV-infected patient with immunologic reconstitution on potent HAART: a rare observation in a nonendemic area.

Coccidioidomycosis is caused by the dimorphic fungus Coccidioides immitis, which is endemic in southwestern United States. We report a case of coccidioidomycosis of cervical lymph nodes that occurred early after the introduction of highly active antiretroviral therapy during the phase of immune system recovery, demonstrating a rare disease in a nonendemic area.

Coccidioidomycosis in Latin America.

Coccidioidomycosis is a highly prevalent systemic mycosis in Latin America and has been reported (human and zoonotic cases) in México, Guate … ot considered as endemic. Genetic studies have shown that the prevalent species in Latin America is Coccidioides posadasii. Coccidioides immitis has been reported sporadically in indigenous cases from Mexico and Colombia. Coccidioidomycosis and tuberculosis share some risk factors such as immunosuppression and residing in areas endemic for these conditions, so …

Treatment considerations in pulmonary coccidioidomycosis.

INTRODUCTION: Coccidioidomycosis is an endemic fungal infection caused by the soil-dwelling fungi, Coccidioides species. Coccidioidal infections may be asymptomatic in up to two-thirds of infected persons. Pulmonary coccidioidomycosis is the most common form of symptomatic infection. Fluconazole is the antifungal agent typically used to treat pulmonary coccidioidomycosis. Other azoles and amphotericin B products may be prescribed to treat nuanced aspects of coccidioidomycosis. AREAS COVERED: This review discusses current literature regarding medical treatment options, including the various tr…

New perspectives on coccidioidomycosis.

Coccidioidomycosis is a growing problem in the endemic regions of Arizona and California. The reasons for its increase are unclear, but may relate to an influx of nonsusceptible individuals into the endemic regions, construction and dust, and climate. Most cases of coccidioidal infection are completely asymptomatic, but approximately 40% present as a pulmonary process that may be difficult to distinguish from a bacterial community-acquired pneumonia. This has led to underdiagnosis of coccidioidomycosis and inappropriate antibacterial therapy. Serology has been the mainstay of diagnosis, but it

Coccidioidomycosis in patients who have undergone transplantation.

In the early years of transplantation in Arizona, coccidioidomycosis occurred in 7% to 9% of recipients, with a mortality rate as high as 72% in some cases. In current transplant programs, however, evolution of immunosuppression and institution of targeted prophylaxis have resulted in coccidioidal infection rates ranging from 1% to 2%. The clinical characteristics of this infection among transplant recipients range from asymptomatic to fulminant and fatal. Dissemination is common, and mortality is high (28%). Because serologic response is often absent or slow, diagnosis can be challenging and
