Spontaneous retroperitoneal sigmoid diverticular perforation presenting as subcutaneous cervical emphysema is rare. We describe a case successfully treated by surgery. Patients presenting with subcutaneous emphysema with negative thoracic findings should be scrutinised for signs of retroperitoneal hollow viscus perforation to improve their outcome.

BACKGROUND/AIMS: To assess the accuracy of a new generation endoscopic ultrasonography (EUS)(GF-M200) in pre-operative staging of recto-sigmoid colon carcinoma invasion and lymph node metastasis.
METHODOLOGY: Seventy-three patients with biopsy proven colon cancer were included in this study. These comprised 60 patients with rectal carcinoma and 13 patients with sigmoid carcinoma. All patients were pre-operatively examined by EUS. Pathological findings of the depth of tumor invasion and presence of lymph node metastasis were correlated with EUS.

A 42-year-old-women with sigmoid colon adenocarcinoma was found to have isolated prolonged activated partial thromboplastin time (aPTT 102.5 s, normal range 24-36 s) preoperatively. The purpose of this study was to determine the feasibilty of performing transcutaneous electrosigmoidography (TC-ESG). The study involved 19 healthy volunteers (11 men, 8 women; mean age 38.2 +/- 14.8 years). To validate the results of TC-ESG, the latter was performed simultaneously with intra-sigmoid ESG. A 49-year-old woman with 45,XO karyotype Turner's syndrome and a recent onset of loose-to-watery stool passage was found to have a sigmoid polyp and adenocarcinoma in the ascending colon. Turner's syndrome is a result of a variety of abnormalities of the X chromosome. There are reports that the risk of malignancy, both gonadal and nongonadal in origin, is increased in patients with such chromosomal defects. Although, diet and heredity play important roles in the etiology of colorectal cancer, the relationship between cancer and certain congenital defects should be taken into account.

During a 5-year period, 32 patients with colorectal carcinoma underwent a Hartmann procedure. Twenty operations were performed as emergencies for obstruction or peritonitis, and 12 for the elective treatment of colorectal malignancies. Of 22 surviving patients with potentially curable resections, 17 had restoration of colorectal continuity without complication. Five patients refused this option. Two case reports of giant gas filled cysts of the sigmoid colon are presented. It is considered that radiology provides the only useful and conclusive diagnostic investigation. Although colon diverticulum is a common disease, few cases have been previously reported on colon cancer associated with a diverticulum.
