Out-of-home life spaces valued by urban older adults with limited income.

BACKGROUND: Access to, and occupational performance in, out-of-home-life-spaces is linked to health, wellbeing and quality of life for older adults. There is little evidence of how this relates to older adults with limited resources in an African urban context. OBJECTIVES: To describe the out-of-home-life-spaces accessed and valued by older adults with limited resources, living in an urban South African setting. METHOD: An exploratory concurrent mixed methods study saw 84 rehabilitation clinicians conduct 393 face-to-face interviews with older adults. Clinicians produced reflective field not

Creating Safe Spaces: A Community Health Worker-Academic Partnered Approach to Addressing Intimate Partner Violence.

Intimate partner violence (IPV) is a persistent public health problem in the United States, with an estimated one in three women experiencing rape, physical violence, and/or stalking by an intimate partner within her lifetime. Non-Hispanic Black women disproportionately experience IPV, but there has been limited success in implementing culturally appropriate prevention programs and services for members of this population. Community health workers (CHWs) are trusted members of under-resourced communities who provide reliable health information and improve the cultural appropriateness of service

Experience with Dog Sterilization Campaigns for Surveillance of Zoonotic Diseases in Ecuador: Building Bridges with Animal Organizations to Improve One Health Research.

The extensive population of free-roaming dogs poses significant public health risks in low- and middle-income countries because of their potential role in spreading infectious and zoonotic diseases. This article discusses the successful collaboration between academia and nonprofit animal welfare organizations in Ecuador to enhance One Health surveillance despite limited research resources. This collaborative model could serve as a valuable example for other countries in the Americas and globally, where community-based strategies may improve One Health policies.

Caring for the mother-child dyad as a family physician.

Infant social, emotional and neurological development is shaped by the mother-child dyad. Dysfunction in this bond, as well as maternal mental health problems, can negatively impact child development. The family physician is well-placed to spot dysfunction in the mother-child dyad and screen for postnatal depression during well-child visits. If any issues are identified, the family physician can provide support and help the mother-child dyad to access community resources and specialist psychiatric services.

Committee opinion No. 576: health care for homeless women.

: Homelessness continues to be a significant problem in the United States. Women and families represent the fastest growing segment of the homeless population. Health care for these women is a challenge but an important issue that needs to be addressed. Homeless women are at higher risk of injury and illness and are less likely to obtain needed health care than women who are not homeless. It is essential to undertake efforts to prevent homelessness, to expand community-based services for the homeless, and to provide adequate health care for this underserved population. Health care providers ca

Action for Access: Equity for Children With Disabilities in Lowand Middle-Income Countries.

Sixteen percent of the world's population experiences significant disability. This is likely an underestimation, as there is not a uniformly accepted definition of disability and there are limited validated tools to support clinicians in their diagnoses, especially in low- and middle-income nations. Even when a diagnosis is made, access to specialized health care and community resources is limited, with increased travel distances and high cost of services presenting additional challenges for patients and families. The World Health Organization and the United Nations have provided additional fr…

The role of the orthopaedic surgeon in natural disasters.

A natural disaster is a phenomenon causing significant human, material, economic, and environmental losses, exceeding the capacity of the affected community or country to respond with its own resources. These events progress through phases: hyperacute, acute, subacute, chronic, and rehabilitation/recovery. Orthopedic surgeons play a vital role in each phase, from initial rescue to long-term rehabilitation. In the acute phase, their responsibilities include triaging and managing injuries such as fractures (commonly in the lower limbs), wound infections, and crush injuries. Effective treatment i

Evidence-Based Management of Heart Failure: Clinical Impact and National Health Service Implications.

Heart failure is a leading cause of morbidity and mortality worldwide and poses a major health challenge in the United Kingdom (UK), where it affects a substantial proportion of the population and consumes a substantial proportion of National Health Service (NHS) resources. Despite therapeutic advances, prognosis remains poor, and recurrent hospital admissions contribute significantly to the overall burden. This review summarises current strategies for heart failure management, including pharmacological therapy, device-based interventions, lifestyle modification, and multidisciplinary team app

Psychiatry's Past Can Be Psychiatry's Future.

In the last half of the 20th century, psychiatry lost many of the conditions needed for unhindered practice. I compiled from searches of the literature the 20th century changes in the arenas of psychiatric practice and the sources of these changes. I determined how these changes are shaping 21st century health and well-being. The neglect of the severely mentally ill, first in Bedlams and now on Boulevards, reflects a wide loss of resources. Psychiatry's patients have lost a past of community-based mental health services, interdisciplinary care teams, preventive consultation with social agencie

Elder Abuse in the African Diaspora: A Review.

BACKGROUND: As with many other populations, abuse of older adults is a growing problem across the Africa Diaspora. Modernization and urbanization are eroding the traditional values of respect for older adults. Also, older adults living in environments with limited social and economic resources, and having no means of economic support create a recipe for elder abuse and neglect. METHODS: This article reviews the current literature on the epidemiology, risk factors, and interventions used for elder abuse across the African Diaspora. RESULTS: Reports of elder abuse range from 24.9% to 81.1% acr

Towards Cognitive Impairment Screening in Elderly Communities with Audio-Visual Modal Disentangled Representation Learning.

Alzheimer's disease (AD) is pressing global health concerns, for which early diagnosis is critical to effective intervention. However, conventional approaches, including neuropsychological assessments and neuroimaging techniques, are resource-intensive and impractical for community-level screening. In contrast, artificial intelligence-driven behavioral analyses, including speech pattern and facial expression recognition, have demonstrated considerable potential for scalable and non invasive cognitive assessment. This work presents a community-oriented intelligent screening system for cognitive

California provider and advocate perspectives about opportunities to optimize nutrition services and resources in the first 1000 days.

BACKGROUND: Nutrition in the first 1000 days of life, from conception to age 2 years, plays a critical role in shaping offspring's physical and mental development, yet many families from underserved backgrounds suffer from nutrition inequity during this important stage of development. The objective of this study is to assess nutrition services and resources provided to families during the first 1000 days across diverse settings in California. METHODS: A semistructured survey was disseminated to healthcare and educational providers who offer services to pregnant and/or postpartum women and chi

A resource reallocation model for school dental screening: taking advantage of teledentistry in low-risk areas.

OBJECTIVE: To estimate the scale of resource transfer that could be achieved by screening low-risk schoolchildren using teledentistry rather than using traditional visual dental examination. METHODS: This study was based on a previous cost-minimisation study that compared the costs of two dental-screening approaches (visual and teledentistry). The data for the population of children 5-14 years of age was obtained from the Australian Bureau of Statistics and was divided across Australia according to statistical local area (SA2). The cost models (for teledentistry and visual screening) for each

Blood Tests for People with Severe Learning Disabilities Receiving Dental Treatment under General Anaesthesia.

People with learning disabilities (LDs) have poorer health than their non-disabled peers due to failures in reasonable adjustments. One hundred patients with severe LD and challenging behaviour attended for dental treatment under GA, during which routine blood testing was provided. Communication with general medical practitioners (GMPs) and blood test results were evaluated, showing poor communication with GMPs and significant undiagnosed disease among this group. Blood tests generate similar costs in primary and secondary care but a holistic approach to care under GA reduces expenses brought

A Community-Directed Integrated Strongyloides Control Program in Queensland, Australia.

This paper describes two phases of a community-directed intervention to address strongyloidiasis in the remote Aboriginal community of Woorabinda in central Queensland, Australia. The first phase provides the narrative of a community-driven 'treat-and-test' mass drug administration (MDA) intervention that was co-designed by the Community Health Service and the community. The second phase is a description of the re-engagement of the community in order to disseminate the key factors for success in the previous MDA for Strongyloides stercoralis, as this information was not shared or captured in t

Resource availability and capacity to implement multi-stranded cholera interventions in the north-east region of Nigeria.

BACKGROUND: Limited healthcare facility (HCF) resources and capacity to implement multi-stranded cholera interventions (water, sanitation, and hygiene (WASH), surveillance, case management, and community engagement) can hinder the actualisation of the global strategic roadmap goals for cholera control, especially in settings made fragile by armed conflicts, such as the north-east region of Nigeria. Therefore, we aimed to assess HCF resource availability and capacity to implement these cholera interventions in Adamawa and Bauchi States in Nigeria as well as assess their coordination in both sta

Socio-hydrological analysis: a new approach in water resources management in western Iran.

Human consumption patterns have a significant impact on the amount of available water. However, the human effect on water resources is perceived to have been poorly studied. For the effective management of water resources, social and hydrological components should be studied. To fill this gap, the aim of this study was to investigate the socio-hydrological system of the Gavshan Dam in western Iran. Therefore, the qualitative method and root cause analysis (RCA) were used to investigate the causes of the imbalance between water consumption and water resources. Root cause analysis was used to in

Bridging the Global Divide: Advancing Traumatic Brain Injury Research Through Globally Representative Data.

Traumatic brain injury (TBI) is a significant global health issue, yet its impact is disproportionately borne by low-income and middle-income countries (LMICs), where higher exposure to risk factors and limited health care resources exacerbate the burden. Despite this, the majority of TBI research is derived from data sets curated in high-income countries (HICs), where research infrastructure and resources are more extensive. As a result, clinical guidelines for TBI management are primarily shaped by data from HICs, with little consideration for the distinct factors and health care challenges

Cesarean section rates and local resources for perinatal care in Japan: A nationwide ecological study using the national database of health insurance claims.

AIM: High cesarean section (CS) rates in middle and high-income countries are partly attributable to provider factors, such as staffing patterns and fear of litigation. However, the relationship between CS rates and healthcare resources in the community is poorly understood. Official data on CS rates has been particularly limited in Japan. In this study, we examined nationwide CS statistics and evaluated the association with local resources for perinatal care. METHODS: We used accumulated data for CS registered in the Japan National Database of health insurance claims in 2013 and calculated c
