Diagnostic accuracy of the Family Confusion Assessment Method for delirium detection: A systematic review and meta-analysis.

BACKGROUND: Delirium is frequently disproportionately under-recognized despite its high prevalence, detrimental impact, and potential lethality. Informant-based delirium detection tools can offer structured assessment and increase the timeliness and frequency of detection. We aimed to examine the diagnostic accuracy of the Family Confusion Assessment Method (FAM-CAM) for delirium detection. METHODS: We systematically searched the MEDLINE, EMBASE, PsycINFO, CINAHL, CNKI, WANFANG, and SinoMed databases from January 1988 to December 2022. Two reviewers independently screened studies and evaluate…

Clinical Reasoning: A 47-Year-Old Man With an Upper Respiratory Infection, Acute Confusion, Dysarthria, and Ataxia.

A patient presenting with acute confusion, dysarthria, and appendicular ataxia with gait instability warrants a broad differential including emergent consideration of acute ischemic or hemorrhagic stroke. Moreover, in acute to subacute presentations, a wide array of etiologies including infectious causes, toxins, or autoimmune conditions may be considered. This article features a 47-year-old man who presented acutely with confusion, severe dysarthria, left upper extremity dysmetria, and unsteady gait. In this case, these neurologic signs were preceded by symptoms of an upper respiratory infect…

Large sized common iliac artery aneurysm with thrombus developing a diagnostic confusion in a patient with sciatica.

The causes of sciatica are variable and include musculoskeletal, dermatologic, infectious, neoplastic, and vascular disorders. In many cases, the symptom is usually caused by degenerative disease in the spine with the compression or irritation of spinal nerve. On the other hands, there are also several announced extra-spinal causes including aneurysm, diabetes, and radiation for sciatica in a low rate. Among the extra-spinal cases, aneurysms arising from iliac vessels are sometimes developing a diagnostic confusion with the spinal causes, and delayed diagnosis can lead to poor prognosis. It is…

Lipid-rich and clear cell neuroendocrine tumors ("carcinoids") of the appendix: potential confusion with goblet cell carcinoid.

The so-called clear cell change has been described in neuroendocrine tumors at several locations. Those associated with von Hippel Lindau di … disease. The importance of recognizing this variant of carcinoid tumor in the appendix is to avoid confusion with goblet cell carcinoid tumors with or without a signet ring adenocarcinoma. The presence of multi-vacuolated, foamy and clear cells, some resembling signet ring or goblet cells, in otherwise classic carcinoid tumors is rare but should be conside …

Large benign hepatocellular nodules in cirrhosis due to chronic venous outflow obstruction: diagnostic confusion with hepatocellular carcinoma.

Budd-Chiari syndrome (BCS) in patients progressing to cirrhosis is an indication for liver transplantation. At this stage of disease, it is common to find large benign hepatocellular nodules (LBHNs) of undetermined cause that may be confused with hepatocellular carcinoma (HCC). Patients with indications for liver transplantation are currently classified according to the MELD (Model for End-Stage Liver Disease) severity score. When they fit Barcelona and Milan eligibility criteria for HCC, they receive 20 points. Thus, misdiagnosis of HCC leads to a privileged position on the waiting list. Here

Involvement of collagenous spherulosis by lobular carcinoma in situ. Potential confusion with cribriform ductal carcinoma in situ.

We describe five cases in which the cells of lobular carcinoma in situ seemed to form round, regular lumens similar to the cribriform spaces of ductal carcinoma in situ. After careful inspection, we concluded that the spaces indicate the presence of collagenous spherulosis and that this unusual pattern arises through the confluence of lobular neoplasia and spherulosis. We base this conclusion on three lines of evidence. First, attenuated myoepithelial cells, rather than carcinoma cells, form the spaces in question. Immunohistochemical staining for smooth-muscle actin and keratin 8/18 establish

Inflammatory changes related to active tuberculosis: confusion with oat cell carcinoma of the lung on cytology specimens.

One case of small cell lung carcinoma diagnosed on cytology specimens could not be confirmed histologically. We feel this likely represents cytopathologic changes induced in response to the patient's active tuberculosis. To avoid aggressive systemic therapy in such cases, patients with localized lesions should have further diagnostic procedures to confirm the cytologic diagnosis histologically.

Art healing or art therapy? Untangling conceptual confusion in China and advancing a pathway toward community institutionalization.

The field of art therapy in China is currently facing severe conceptual confusion between art therapy and art healing. While international scholarship has widely noted the inherently fluid boundaries among these practices, this paper argues that in China, where an institutional vacuum exists, such fluidity has resulted not in productive debate but in a conceptual conflation. This paper examines how such confusion manifests across academic research, educational practices, and the commercial market, arguing that it has led to de-professionalization and hindered the sustainable development of com…

From Stress to Obsession: The Role of Inferential Confusion and Obsessive Beliefs in Adolescent Obsessive-Compulsive Symptoms.

Adolescence is a critical developmental period during which obsessive-compulsive disorder (OCD) frequently emerges. This study examined cognitive models of OCD, the associations between stressful life events (SLEs) and symptom severity, and the unique challenges faced by adolescents living with OCD symptoms. Two indirect pathways were tested: one involving SLEs and the other perceived stress, with both pathways including inferential confusion and obsessional beliefs as mediators. A sample of adolescents presenting with OCD symptoms completed validated measures assessing stress, obsessive belie…

Acute Expressive Aphasia and Confusion As the Initial Neurologic Manifestation of Herpes Simplex Virus Type 1 Encephalitis.

Herpes simplex virus type 1 encephalitis is a neurologic emergency that can present with focal language deficits before the diagnosis becomes clinically apparent. We present an 82-year-old man with diabetes mellitus, hypertension, and hyperlipidemia who developed acute confusion and expressive aphasia after an emergency department visit the previous day for fever, chills, fatigue, sore throat, dizziness, and abdominal discomfort. On arrival for the aphasia presentation, the temperature was 100.3°F, and there were no motor or sensory deficits. Computed tomography of the head, computed tomograph…

What's in a name? Examining the confusion of meat-like terminology on meat imitating plant-based products.

Meat alternatives are becoming increasingly popular amongst consumers. The names on these products, specifically the use of meat-like designations on non-animal products, remains a major point of contention. The aim of this study was to investigate whether meat-like vs. non-meat-like names are potentially confusing or even misleading. In this study, a categorisation task was used where participants classified products as animal-based or plant-based. Our results show that the presence of a meat-like name on a meat alternative label leads to significantly more mistakes when classifying a product

Radiology Synthetic Confusion: How Generative Artificial Intelligence Amplifies Misunderstandings of Radiologists and Technologists in Patient-Facing Media.

Rationale and Objectives: Artificial intelligence (AI) tools, particularly generative models, are increasingly used to depict clinical roles … AI frequently misrepresents radiologist roles and demographics, reinforcing stereotypes and public confusion. Greater oversight and inclusion standards are needed to ensure equitable AI-generated healthcare content.

Detection of postoperative delirium by family and caregivers: Evaluation of the family confusion assessment method (FAM-CAM).

OBJECTIVE: The primary objective was to evaluate agreement between researchers' and family members' postoperative delirium assessment. The secondary objective was to assess the incidence of positive FAM-CAM after hospital discharge up to 30-days postoperatively. METHODS: This was a pre-specified sub-study of two multicenter randomized controlled trials that evaluated interventions to prevent postoperative delirium in older adults undergoing major elective surgery. In the hospital, delirium was ascertained using the Confusion Assessment Method (CAM) long-form or the Confusion Assessment Method…

Rapid Improvement in Consciousness After Low-Dose Ketamine in a Patient With Acute Confusion: A Case Report.

Ketamine, widely used for procedural sedation, may possess underrecognized neuroprotective and cognitive-modulating effects. We report the case of a 66-year-old female patient who presented with acute confusion (Glasgow Coma Scale (GCS) 12/15; E3V4M5), likely secondary to a urinary tract infection. Remarkably, she regained full consciousness (GCS 15/15; E4V5M6) within 45 minutes of receiving 40 mg of intravenous ketamine for nasogastric tube insertion. No other sedatives or interventions were administered during this period. This observation raises the possibility that ketamine may play a role…

Color polymorphism, taxonomic confusion and cryptic diversity in the kukri snake Oligodon albocinctus (Cantor, 1839) (Squamata: Colubridae).

The kukri snake Oligodon albocinctus (Cantor, 1839) is a widespread element of the Indo-Himalayan and Indo-Burmese snake fauna with two distinct color patterns. The first and more typical morphotype has light bands with black edges, while individuals of the second morphotype have brown blotches or narrow crossbars. The polychromatic nature of this species has led to considerable taxonomic confusion between O. albocinctus and members of Oligodon occurring in sympatry or parapatry. In this study, we investigate patterns of genetic and morphological variation in O. albocinctus and explore whether…

Addressing the taxonomic confusion of Mesocoelium Odhner, 1910 (Trematoda: Plagiorchioidea: Mesocoeliidae) in Japanese urodelan and anuran amphibians.

Trematodes of the genus Mesocoelium Odhner, 1910 (Digenea: Plagiorchioidea: Mesocoeliidae) are globally distributed and parasitize amphibians, reptiles, or occasionally fishes. This genus is one of the most confusing taxa in trematodes because of its poor morphological features. In this study, we examined species of Mesocoelium collected from Japanese amphibians and found that they can be morphologically assigned to two species of Mesocoelium. Mesocoelium brevicaecum Ochi in Goto and Ozaki, 1929 parasitizes various both urodelan and anuran amphibians and occurred widely in Japan, while M. japo

A rare case of multiple high-grade dural arteriovenous fistulas manifesting as progressive confusion and abnormal behavior.

INTRODUCTION AND IMPORTANCE: Multiple dural arteriovenous fistulas (AVFs) are rare intracranial vascular malformations with fatal consequences. Patients usually manifest with variable clinical presentations, which depend on the fistula's size, location, and the development of complication. Digital subtraction angiography continues to be the gold standard neuroimaging modality for diagnosis. CASE PRESENTATION: A 47-year-old female patient was transferred from Gaza Hospital intubated under complete sedation after she had progressive confusion and abnormal behaviors. She was immediately admitted…
