The association of pain impact and sleep disruption with opioid withdrawal during opioid-use disorder treatment.

AIMS: Persons with opioid-use disorder (OUD) often experience opioid withdrawal and opioid craving, which can drive continued opioid use and treatment discontinuation. In addition, hyperalgesia is common among persons with OUD, yet few studies have examined the role of pain impact during OUD treatment. The purpose of the present study was to test whether opioid withdrawal and craving were elevated in the context of greater pain impact (i.e. greater pain intensity and interference), and whether these associations changed throughout treatment. METHODS: Participants in residential OUD treatment …

Effects of stress on emotional reactivity in hostile heavy social drinkers following dietary tryptophan enhancement.

AIM: Because individuals high on hostility may be at risk for alcohol abuse due to serotonergic dysfunction and greater reactivity to stress, we examined the effects of acute dietary tryptophan enhancement and stress on mood and craving for alcohol in low-hostile (LoH) and high-hostile (HiH) individuals. METHODS: Thirty-four LoH and 33 HiH heavy social drinkers [selection based on the Hostility scale from the Buss and Perry Aggression Questionnaire (1992)] received either tryptophan-enriched or control diet and underwent a stress-induction procedure. Trait differences between the two hostile …

Heart Rate Variability Biofeedback for Substance Use Disorder: A Randomized Clinical Trial.

IMPORTANCE: Preliminary studies suggest heart rate variability biofeedback (HRVB) may reduce craving and negative affect in individuals with substance use disorder (SUD), but few studies have evaluated whether this translates into improved substance use outcomes, and no prior studies have examined second-generation wearable HRVB technology in this context. OBJECTIVE: To evaluate the effects of second-generation HRVB on negative affect, positive affect, craving, and alcohol and other drug (AOD) use in adults with SUD. DESIGN, SETTING, AND PARTICIPANTS: This phase 2 randomized clinical trial i…

Development and initial validation of the obsessive compulsive eating scale.

OBJECTIVE: Food cravings are common and have been implicated in eating-related pathology, including binge eating, bulimia nervosa, and unhealthy weight problems, but difficulties in defining and quantifying the phenomenon of craving are well documented. There has been an increase in focus on the study of cognitive mechanisms underlying craving, in particular the role of intrusive thoughts; however, existing craving measures fail to fully capture these aspects of the craving experience. The present study was designed to develop a psychometrically sound measure of the obsessive-compulsive aspect…

Chocolate: food or drug?

Although addictive behavior is generally associated with drug and alcohol abuse or compulsive sexual activity, chocolate may evoke similar psychopharmacologic and behavioral reactions in susceptible persons. A review of the literature on chocolate cravings indicates that the hedonic appeal of chocolate (fat, sugar, texture, and aroma) is likely to be a predominant factor in such cravings. Other characteristics of chocolate, however, may be equally as important contributors to the phenomena of chocolate cravings. Chocolate may be used by some as a form of self-medication for dietary deficiencie…

Thirsty heroin addicts show different fMRI activations when exposed to water-related and drug-related cues.

Relapse to drug use is frequently preceded or caused by craving, an intense desire for drug. Advances in functional brain imaging techniques make it possible to directly investigate this special mental state in vivo and non-invasively. Extant imaging studies on craving have been mostly on cocaine which is the dominant drug abused in the U.S. Employing functional MRI, we examined substance specificity of the neural circuitry underlying craving for heroin. Heroin is the primary drug abused in south-east Asia and has, particularly, become a serious social problem for China in recent years. Follow…

Ceria-nanocluster based therapy for Alzheimer's disease through the modulation of activated microglia and attenuation of amyloid-β deposition.

Relapse is a major challenge in treating drug addiction, and drug seeking progressively increases after abstinence, a phenomenon termed "incubation of drug craving". Previous studies demonstrated both sex differences and an effect of estrous cycle in female rats in incubation of cocaine craving. In contrast, while incubation of methamphetamine craving is similar across sexes, whether estrous cycle plays a role in this incubation has yet to be fully addressed. Moreover, whether neural mechanisms underlying incubation of methamphetamine craving differ across estrous cycles is largely unknown. To…

The role of baseline insomnia in moderating the hypnotic properties of quetiapine.

INTRODUCTION: The primary aim of this study was to investigate the effect of quetiapine on insomnia and alcohol craving (craving) in subjects with co-occurring insomnia and AUD. METHODS: Insomnia was assessed with the Insomnia Severity Index (ISI) and craving with the Penn Alcohol Craving Scale (PACS, primary) and Obsessive-Compulsive Drinking Scale (OCDS, secondary). A multivariable model adjusted for covariates (N = 123) evaluated the relationship between craving (PACS and OCDS total scores) and insomnia (ISI total score). To simultaneously assess the effects of treatment arm allocation and…

Transcranial direct current stimulation of the prefrontal cortex modulates the desire for specific foods.

We aimed to assess whether modulation of the dorsolateral prefrontal cortex (DLFPC) with noninvasive brain stimulation, namely transcranial direct current stimulation (tDCS), modifies food craving in healthy subjects. We performed a randomized sham-controlled cross-over study in which 23 subjects received sham and active tDCS (anode left/cathode right and anode right/cathode left) of the DLPFC. Subjects were exposed to food and also watched a movie of food associated with strong craving. Desire for food consumption was evaluated by visual analogue scales (VAS) and food consumption before and a…

Biobehavioral mechanisms of topiramate's effects on alcohol use: an investigation pairing laboratory and ecological momentary assessments.

Topiramate reduces drinking, but little is known about the mechanisms that precipitate this effect. This double-blind randomized placebo-controlled study assessed the putative mechanisms by which topiramate reduces alcohol use among 96 adult non-treatment-seeking heavy drinkers in a laboratory-based alcohol cue reactivity assessment and in the natural environment using ecological momentary assessment methods. Topiramate reduced the quantity of alcohol heavy drinkers consumed on drinking days and reduced craving while participants were drinking but did not affect craving outside of drinking epi…

Diethylpropion pharmacotherapeutic adjuvant therapy for inpatient treatment of cocaine dependence: a test of the cocaine-agonist hypothesis.

Fifty cocaine-dependent patients completed a 2-week double-blind, double-dummy, parallel-group comparison of four dosage levels of diethylpropion and placebo. This clinical trial was designed to evaluate both diethylpropion's ability to attenuate cocaine cue-induced craving and its potential for development as a medication with cocaine-agonist properties. The results indicated that diethylpropion was not superior to placebo and confirmed earlier reports that craving for cocaine diminishes over the course of an inpatient hospitalization. Moreover, the results showed that the cocaine cue-induced…

Increased Mesostriatal Intrinsic Connectivity Associated With Cue Exposure in Adult Cannabis Users: Preliminary Findings.

Cue-induced craving-the desire to use a drug triggered by exposure to cues associated with prior use-is a central mechanism in the development and maintenance of problematic substance use behaviours. Drug cues have the power to induce craving even in long-term abstinent individuals, which has led clinicians to advise patients to avoid the people, places and objects that are associated with their use. This preliminary study builds on prior behavioural research that demonstrates that exposure to multimodal drug cues can increase craving even after the drug cues are removed from the environment. …

Validity of the DSM-5 tobacco use disorder diagnostics in adults with problematic substance use.

BACKGROUND: DSM-5 tobacco use disorder (TUD) nosology differs from DSM-IV nicotine dependence (ND) by including craving and DSM-IV abuse criteria, a lower threshold (≥ 2 criteria), and severity levels (mild; moderate; severe). We assessed concurrent and prospective validity of the DSM-5 TUD diagnosis and severity and compared validity with DSM-IV ND diagnosis. METHODS: The sample included U.S. adults with current problematic substance use and past year cigarette smoking (N = 396). Baseline assessment collected information on DSM-IV ND and DSM-5 TUD criteria, smoking-related variables, and psy…

Positive Allosteric Modulation of mGlu1 Reverses Cocaine-Induced Behavioral and Synaptic Plasticity Through the Integrated Stress Response and Oligophrenin-1.

BACKGROUND: Cue-induced cocaine craving progressively intensifies (incubates) during abstinence from cocaine self-administration. Expression of incubated cocaine craving depends on elevated calcium-permeable AMPA receptors (CP-AMPARs) on medium spiny neurons in the nucleus accumbens (NAc) core. After incubation has occurred, stimulation of NAc metabotropic glutamate 1 (mGlu1) receptors or systemic administration of mGlu1 positive allosteric modulators removes CP-AMPARs from NAc synapses via dynamin-dependent internalization (mGlu1 long-term depression [LTD]) and thereby reduces incubated cocai…

Day-to-day links between negative affect, distress intolerance, and alcohol use.

OBJECTIVE: Negative reinforcement models theorize that state negative affect should predict alcohol use; however, research examining the relationship between heightened negative affect and same-day drinking has produced equivocal results. Focusing on alcohol craving, a psychological mechanism that has clear implications on behavior, as an outcome may help elucidate this process. Further, it may be that an individual's ability to tolerate distress associated with negative affect drives cravings for alcohol to alleviate such emotions. Thus, the goal of this study was to investigate the extent to…

Hypothalamic response to cocaine cues and cocaine addiction severity.

The dopaminergic motive system is compromised in cocaine addiction. Abundant research has examined the roles of the dopaminergic midbrain and ventral striatum (VS) in cue-induced craving and habitual drug consumption. Interconnected with the dopaminergic circuits, the hypothalamus is widely implicated in motivated behavior, including food and drug seeking. However, very few studies have investigated how the hypothalamus responds to drug cues and whether hypothalamic responses are related to clinical features such as craving and addiction severity. Here, in 23 cocaine-dependent individuals (CD)…

OPRM1 Moderates Daily Associations of Naltrexone Adherence With Alcohol Consumption: Preliminary Evidence From a Mobile Health Trial.

BACKGROUND: Initial evidence that OPRM1 genotype moderates the clinical response to naltrexone has not been replicated in prospective clinical trials. However, the use of traditional statistical analyses and clinical endpoints might limit sensitivity for studying pharmacogenetic associations, whereas the use of intensive daily assessments and person-centered analytic methods might increase sensitivity. This study leveraged person-centered analyses and daily measures of alcohol use, craving, and medication adherence to investigate OPRM1 as a moderator of changes in clinical outcomes during nalt…

Prediction of Smoking Abstinence in Women Living With Human Immunodeficiency Virus Infection.

BACKGROUND: This study examined whether baseline negative emotional states (depression and anxiety) would predict craving for cigarettes and other nicotine withdrawal symptoms in early abstinence and whether those emotional states and withdrawal symptoms would predict failure in quitting smoking at 3 months postquit among U.S. women living with the human immunodeficiency virus (HIV). METHOD: The study is a secondary analysis of data from two smoking cessation studies of women living with HIV. Craving for cigarettes and other withdrawal symptoms were assessed weekly with a total of 229 observa…
