Cutaneous reactions to proton pump inhibitors: a case-control study.

BACKGROUND: Even though proton pump inhibitors (PPIs) are commonly used in clinical practice, a limited number of studies are available about cutaneous adverse reactions from PPIs, and most of these are case reports. OBJECTIVE: To demonstrate the pattern of cutaneous reactions related to PPI usage and to evaluate the risk of developing PPI drug eruptions among adult patients. METHODS: We reviewed the spontaneous reports of any adverse events associated with PPI use, as reported from January 2005 through May 2010 to the Adverse Drug Reaction Center at Siriraj Hospital in Thailand. Each contro

Comparison of irritatant reactions between using lyophilized and commercial food allergen extracts in atopy patch tests in a normal population.

BACKGROUND: Several authors have investigated the use of the atopy patch tests (APT) for the diagnosis of non-IgE mediated food allergy, primarily in patients with atopic dermatitis and digestive disorders. However, one of the difficulties in atopy patch testing is the lack of standardization. Several commercial APTs containing freeze-dried food extracts are now available, but their diagnostic accuracy is still largely undefined. The objective of this study is to evaluate the irritant reactions and safety of atopy patch tests in healthy subjects by using lyophilized and commercial food allerge

Contact dermatitis associated with food: retrospective cross-sectional analysis of North American Contact Dermatitis Group data, 2001-2004.

BACKGROUND: Allergic and irritant contact dermatitis to food is likely underreported. OBJECTIVES: To characterize relevant allergens and irritants associated with food in patients referred to the North American Contact Dermatitis Group (NACDG) for patch testing. METHODS: Retrospective analysis of cross-sectional data from the NACDG from 2001 to 2004. RESULTS: Of 10,061 patch-tested patients, 109 (1.1%) had a total of 122 reactions associated with food. Approximately two-thirds of patients (66%) were female, and one-third (36%) were atopic. The hands were the most common sites of dermatitis

Synthesis of 9Z-9-substituted retinoic acids by palladium catalyzed coupling reaction of a vinyl triflate with alkenyl stannanes.

Palladium catalyzed cross coupling reactions of a vinyl triflate intermediate and various alkenyl stannanes afforded trisubstituted Z-olefins stereoselectively in high yields. These olefins were then converted to the corresponding 9Z-retinoic acids via Horner-Emmons reaction and subsequent basic hydrolysis in excellent yields.

Updating the classification and routine diagnosis of NSAID hypersensitivity reactions: A WAO Statement.

Hypersensitivity reactions to non-steroidal anti-inflammatory drugs (NSAIDs) have been classified as immediate (or acute) and delayed. Immediate reactions can be further classified into 4 clinical types: NSAID-exacerbated respiratory disease (N-ERD), NSAID-exacerbated cutaneous disease (NECD), NSAID-induced urticaria/angioedema (NIUA), and single NSAID-induced urticaria/angioedema/anaphylaxis (SNIUAA). Specifically, the NIUA type references reactions to ≥2 NSAIDs belonging to different chemical groups, involving urticaria and/or angioedema in patients with no underlying chronic spontaneous urt

Decarboxylative Cross-Coupling of Acyl Fluorides with Potassium Perfluorobenzoates.

We report the transition metal-free decarboxylative cross-coupling reactions of acyl fluorides with potassium perfluorobenzoates. Compared with traditional transition metal-catalyzed cross-couplings, this protocol presents an extremely environmentally benign pathway to afford unsymmetrical diaryl ketones. To install perfluorophenyl groups, this method highlights highly selective, inexpensive, and nontoxic conditions. The reaction system tolerates various functional groups in acyl fluorides. Remarkably, all of the starting materials can be prepared from abundant carboxylic acids and the reactio

Identification of antigens for use in immunodiagnosis of Taenia saginata cysticercosis.

Fifteen metacestode antigens from Taenia saginata were defined by Laurell crossed immunoelectrophoresis and investigated for their potential use in immunodiagnosis of bovine cysticercosis. Several antigens cross reacted with those of some common cattle parasites. Three of the antigens, designated as numbers 4, 8 and 11, were selected on the basis of their restricted cross reactions and were isolated by affinity chromatography. These antigens showed high sensitivity and specificity values in the enzyme-linked immunosorbent assay (ELISA) for serodiagnosis of bovine cysticercosis.

Alkenylation of C(sp3 )-H Bonds by Zincation/Copper-Catalyzed Cross-Coupling with Iodonium Salts.

α-Vinylation of phosphonates, phosphine oxides, sulfones, sulfonamides, and sulfoxides has been achieved by selective C-H zincation and copper-catalyzed C(sp3 )-C(sp2 ) cross-coupling reaction using vinylphenyliodonium salts. The vinylation transformation proceeds in high efficiency and stereospecificity under mild reaction conditions. This zincative cross-coupling reaction represents a general alkenylation strategy, which is also applicable for α-alkenylation of esters, amides, and nitriles in the synthesis of β,γ-unsaturated carbonyl compounds.

Stuck in a Moment: Tonic Immobility Predicts Poor Quality of Life in Treated PTSD Patients.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a prevalent and disabling multisystem disorder, with significant physical and psychiatric morbidity and poor quality of life (QOL). Although peritraumatic reactions - tonic immobility and dissociation - are established predictors of PTSD severity and development, there is a dearth of investigation assessing the impact of peritraumatic reactions on QOL of PTSD patients. Quality of life has become increasingly important in health care and research as a reliable outcome measure. It comprises psychological, physical, social and environmental doma

Hypersensitivity reactions to some synthetic dextran polypeptides in guinea-pigs.

The hypersensitivity reactions of several synthetic dextran-polypeptides have been studied in guinea-pigs. Insulin, oxidized insulin peptide B and their dextran derivatives were found to cross-react. Oxidized insulin peptide A did not cross-react with insulin or dextran-insulin peptide A. Similarly poly-L-tyrosine did not cross-react with dextran-poly-L-tyrosine. The determinants involved are discussed. An immunogenic response could not be promoted towards the carbohydrate by coupling it to polypeptides; the response was always directed towards the polypeptide. However, the immunogenicity of t

Sodium tetrachloropalladate for diagnosing palladium sensitization.

BACKGROUND: Exposure to palladium (Pd) may lead to clinical allergic reactions. With frequent nickel (Ni) exposure and cross-reactivity between Ni and Pd at the T cell recognition level, positive Pd reactions on patch testing are surprisingly uncommon. PdCl(2) is often used for epicutaneous patch testing. OBJECTIVES: To compare the sensitivity and specificity of sodium tetrachloropalladate (Na(2) PdCl(4)) and PdCl(2) for Pd patch testing in metal-allergic patients and non-allergic controls. METHODS: Twenty-six metal-allergic patients and 26 non-allergic controls were selected on the basis of

Neurotoxicants and the cytoskeleton.

Exposure to occupational and environmental toxicants can result in distal axonopathies through reaction with various components of the axonal cytoskeleton. The solvents n-hexane and methyl n-butyl ketone are metabolized to the beta-diketone, 2,5-hexanedione, which covalently cross-links neurofilaments, resulting in large paranodal axonal swellings filled with neurofilaments. Carbon disulfide exposure leads to an identical axonopathy, achieving neurofilament cross-linking through a parallel series of reactions. Acrylamide and ethylene oxide, on the other hand, adduct proteins but do not lead to

Structural features of the quinidine and quinine molecules necessary for binding of drug-induced antibodies to human platelets.

Although most quinidine- and quinine-induced platelet antibodies react only in the presence of the drug that provoked sensitization, some are active with either quinidine or its stereoisomer, quinine; that is, they are "cross-reactive." This suggests that the activity of drug-dependent antibodies is dependent on different structural features of the quinidine or quinine molecules. To investigate this possibility, we studied reactions of 16 quinidine- and quinine-induced antibodies with quinidine, quinine, and analogues of these drugs modified at the quinoline ring (desmethoxy-derivative), the q

Olefin Metathesis Catalyzed by a Latent Ruthenathiete Complex.

Four-membered chelate complexes of ruthenium are rare, with only a handful of examples present in the literature. The first four-membered sulfur chelate, a cis-dichloro ruthenathiete complex, is reported to demonstrate latency toward olefin metathesis reactions until heated or irradiated with UV light. This complex's reactivity was benchmarked through ring-closing metathesis, cross-metathesis, and ring-opening metathesis polymerization reactions in deuterated chloroform and toluene as solvents. Despite the more strained 4-membered ring, the complex displays similar latency to 5-membered analog

Case Report: Severe IgE-mediated hypersensitivity to carboxymethylcellulose with tolerance to crosscarmellose and microcrystalline cellulose.

We present a rare case of severe IgE-mediated hypersensitivity to carboxymethylcellulose (CMC) following an intraarticular knee injection with triamcinolone acetonide (Triamcort®). The patient experienced a grade IV anaphylactic reaction shortly after administration. Diagnostic workup, including skin prick testing and basophil activation test, confirmed sensitization to CMC. Importantly, the patient tolerated medications containing crosscarmellose and microcrystalline cellulose without adverse reactions, suggesting no clinically relevant cross-reactivity. This case highlights the need to consi

Cytochemical and Histochemical Staining with Peptide Antibodies.

Peptide antibodies are particularly useful for immunocytochemistry (ICC) and immunohistochemistry (IHC), where antigens may denature due to fixation of tissues and cells. Peptide antibodies can be made to any defined sequence, including unknown putative proteins and posttranslationally modified sequences. Moreover, the availability of large amounts of the antigen (peptide) allows inhibition/absorption controls, which are important in ICC/IHC, due to the many possibilities for false-positive reactions caused by immunoglobulin Fc receptors, nonspecific reactions and cross-reactivity of primary a

Modes of failure in metal-on-metal total hip arthroplasty.

Use of large-head metal-on-metal (MoM) bearing surfaces in total hip arthroplasty (THA) has created new and unique modes of failure for this type of articulation. These unique modes are in addition to the traditional modes of failure seen in conventional THA, which include instability, osteolysis, infection, iliopsoas tendinitis, aseptic loosening, and periprosthetic fracture. Ion levels and cross-sectional imaging are helpful when evaluating a MoM patient in the identification of adverse local tissue reactions. Unique modes of failure in MoM THA include tissue necrosis, metallosis-induced ost
