Seventeen patients with pulmonary cryptococcosis collected at VGH-Taipei from 1967 to 1991 were analysed. Their ages ranged from 16 to 70 years with a mean of 45.2. Twelve were men and five were female. The chief characteristic symptoms and signs were cough (58.8%), fever (29.4%) and dyspnea (23.5%). Nearly one third of cases were asymptomatic and presented with abnormal chest roentgenogram. The chest roentgenogram included single nodule or mass (41.2%), multiple nodules (35.3%), pneumonic patch or consolidation (23.5%). In addition, 5 cases (29.4%) and 4 cases (23.5%) were combined with cavit…

A patient underwent resection of a superior sulcus tumor after an extensive workup was nondiagnostic. Postoperatively, primary pulmonary cryptococcosis was diagnosed. Management of this patient is presented along with a review of the literature.

Cryptococcus neoformans var. neoformans was isolated from domestic dust in 54% of houses occupied or recently occupied by AIDS+ cryptococcosis patients and in 20% of control houses randomly chosen in Bujumbura, Burundi. It was not isolated from wards of similar patients in the University Hospital. This seems to prove that patients could not contaminate their own environment and that the presence of C. neoformans var. neoformans had another origin. In 40% of the cases, patients admitted contact with pigeons in the city and pigeon coops are frequently positive. We are convinced that the pigeon c…

A 37-year-old woman was admitted with fever and cough. The chest roentgenogram revealed a solitary large round mass in the right lower lung field. Diagnosis was made by transbronchial lung biopsy. There was no evidence of abnormal cell-mediated immunity. The patient made an uneventful recovery and the chest X-ray showed progressive clearing without specific therapy. After 6 months, the mass lesion disappeared. This case suggests that specific therapy may not be necessary for all cases of primary pulmonary cryptococcosis.

Pulmonary cryptococcosis, a less common fungal infection, frequently reveals nodular lesions (single or multiple) and patchy infiltrations (alveolar or interstitial) on chest X rays. A large mass-like pattern is less common. Here we report 2 cases of pulmonary cryptococcosis with negative bronchoscopic findings. One patient received a right lower lobectomy under the impression of malignancy; the other received a transthoracic needle aspiration, and numerous cryptococci were found by India ink preparation. Thus, pulmonary cryptococcosis should be included in the differential diagnosis of a mass…

To better understand the radiographic appearance of pulmonary cryptococcosis, 23 human cases and three cases in zoo animals were studied in which both radiographic and pathologic material was available for correlation. Radiographs in 44 additional cases were studied in which documentation was microbiologic but not pathologic. Three forms of pulmonary cryptococcal disease are recognized: (1) air-space collections of fungus with minimal or no inflammatory reaction, causing well defined masses both radiographically and pathologically, as documented by five cases in this series; (2)granulomatous i…

Ocular cryptococcosis was produced in cats by the intracarotid injection of Cryptococcus neoformans. Infected eyes developed a progressive, multifocal chorioretinitis which was comparable to that found in the naturally occurring feline disease. The severity of the ocular disease, the development of infection in the fellow eye, and the degree of systemic involvement were shown to be related to the number of organisms inoculated. Mice infected intracerebrally with the same organism developed optic nerve meningitis. The pathogenesis of the ocular lesions thus produced is discussed.

Pulmonary cryptococcosis was diagnosed in a 7-year-old dolphin which had been in captivity for about 4 years. Cryptococcosis has been reported in a variety of animals, but this is the first report in a cetacean.

We present a 19-year-old female, eight weeks postpartum, with a seven-week history of migratory arthralgias followed by progressive dyspnea and pleuritic chest pain. A large mass in her right lower lobe was discovered on computed tomography. Laboratory evaluation demonstrated elevated anti-proteinase 3 (PR3) antibody, along with elevated inflammatory markers. The bronchoalveolar lavage culture grew Cryptococcus neoformans, and serum cryptococcal antigen was positive. The patient was diagnosed with pulmonary cryptococcosis and treated with amphotericin and flucytosine, given the severity of her…

Orbital fungal infections are initially misdiagnosed, typically only considered in immunocompromised patients, and may be empirically treated with systemic corticosteroids-potentially exacerbating disease progression. The authors report the third case described in medical literature of Cryptococcus neoformans invading the orbit in an immunocompetent host and the first in a child with bilateral presentation.

BACKGROUND: Disseminated cryptococcosis is a rare and fatal disease involving the central nervous system (CNS) that often presents with severe clinical symptoms and a poor prognosis.
OBJECTIVE: To describe the neuroimaging findings of disseminated cryptococcosis in children and investigate their correlation with patients' prognosis.
MATERIALS AND METHODS: The information of hospitalized patients with disseminated cryptococcosis was collected between January 2012 and December 2023. The study comprehensively evaluated clinical manifestations, laboratory tests, imaging findings, and long-term o…

Cryptococcosis is an opportunistic fungal infection commonly occurring amongst immunocompromised individuals. However, in rare cases it can affect immunocompetent individuals particularly those with exposure to fungal reservoirs in environment. We report a 32-year-old immunocompetent male farmer who was diagnosed with disseminated cryptococcosis. He initially presented with prolonged cough, exertional dyspnoea and weight loss and later developed nodulo-ulcerative skin lesion and central nervous system cryptococcomas. The diagnosis was confirmed by histopathology and cryptococcal antigen testin…
