Unmasking multifactorial dermatitis: The case for comprehensive diagnostic guidelines in adult atopic dermatitis.

Eczematous dermatitis is a heterogeneous group of inflammatory skin disorders characterized by pruritus, erythema, and scaling. The most common subtype is atopic dermatitis (AD), which has a continuously rising prevalence, particularly within industrialized regions, e.g., the United States. Distinguishing AD from other dermatoses, particularly among adult patients with recalcitrant disease, can be challenging. Conditions such as allergic contact dermatitis may mimic or coexist with AD, which complicate both diagnosis and management. This diagnostic complexity has been unmasked with the introdu…

Gene profiling in active dermatitis lesions strengthens the diagnosis of allergic contact dermatitis.

BACKGROUND: Distinguishing between allergic and nonallergic forms of Contact Dermatitis (CD) is challenging and requires investigations based on patch-testing. Early detection of allergy biomarkers in active CD lesions could refine and simplify the management of CD patients. OBJECTIVE: To characterize the molecular signatures of active CD lesions. METHODS: We studied the expression of 12 allergy biomarkers by qRT-PCR in active lesions of 38 CD patients. Allergic CD (ACD) was diagnosed based on patch test (PT) results and exposure assessment. Molecular signatures of active lesions, as well as…

Pediatric Contact Dermatitis Registry Data on Contact Allergy in Children With Atopic Dermatitis.

IMPORTANCE: Atopic dermatitis (AD) and allergic contact dermatitis (ACD) have a dynamic relationship not yet fully understood. Investigation has been limited thus far by a paucity of data on the overlap of these disorders in pediatric patients. OBJECTIVE: To use data from the Pediatric Contact Dermatitis Registry to elucidate the associations and sensitizations among patients with concomitant AD and ACD. DESIGN, SETTING, AND PARTICIPANTS: This retrospective case review examined 1142 patch test cases of children younger than 18 years, who were registered between January 1, 2015, and December …

Susceptibility of atopic dermatitis patients to irritant dermatitis caused by sodium lauryl sulphate.

Basal transepidermal water loss, skin thickness, blood flow and skin colour were examined before and after exposure of 28 patients with atopic dermatitis and 28 healthy controls to sodium lauryl sulphate. Transepidermal water loss was measured with an evaporimeter, skin thickness by ultrasound A-scanning, blood flow by laser Doppler flowmetry and skin colour by a chroma meter using the L*, a* and b* values, respectively. Patients with atopic dermatitis were found to have higher basal transepidermal water loss than controls (p less than 0.0001), and had an inclination towards an increased basal…

Dermatitis herpetiformis in monozygous twins--concordance for dermatitis herpetiformis and gluten-sensitive enteropathy.

A pair of monozygous male twins concordant for dermatitis herpetiformis (DH) and gluten-sensitive enteropathy (GSE) are reported. Presentation of DH in the twins was asynchronous, with one affected at the age of 20, the other at the age of 27. Neither twin had symptoms of malabsorption but duodenojejunal villous atrophy was confirmed histologically.

Prevalence of Atopic Dermatitis Criteria among Textile Workers with Occupational Allergic Contact Dermatitis and Effects of Having Atopic Dermatitis on Contact Antigenic Diversity.

OBJECTIVES: Contact dermatitis (CD) is a common skin disease. Occupational contact dermatitis (OCD) is the most frequently seen occupational skin disease and includes both occupational allergic CD (OACD) and occupational irritant CD (OICD). One of the most common sources of OACD is textile products. Individuals with atopic dermatitis (AD) have an increased risk for development of allergic contact dermatitis (ACD). However, the role of AD in the etiopathogenesis of the development of OACD among textile industry workers is not well known. The aim of the present study was to determine the prevale…

Psoriasis dermatitis: a new entity or a different phenotypic expression of psoriasis and atopic dermatitis?

BACKGROUND: Psoriasis (P) and atopic dermatitis (AD) share some common characteristics. The resulting clinical picture with shared manifestations is a new entity called psoriasis dermatitis (PD), atopic psoriasis or psorema. The purpose of this study was to identify the clinical manifestations of this new dermatological condition, focusing on the adult population. METHODS: We recruited adult patients from two outpatient clinics, the "severe psoriasis" and the "adult atopic dermatitis," between January 1st, 2021, and December 31st, 2021. The 26 patients meeting the inclusion criteria were foll…

Two differing presentations of periocular dermatitis as a side effect of dupilumab for atopic dermatitis.

Dupilumab is a monoclonal antibody used to treat atopic dermatitis. Worsening of atopic dermatitis and conjunctivitis following dupilumab use are reported adverse effects; however, there is little reported on the nature and mechanism of these complications. Here, we describe two patients with chronic atopic dermatitis who developed new or severely worsened periocular dermatitis, believed to be a side effect of dupilumab injections, and resolution after its discontinuation. We explore the possibility of dupilumab-induced suppression of Th2 mediated inflammation and upregulation of Th1 and IFNγ …

The prevalence of Malassezia yeasts in patients with atopic dermatitis, seborrhoeic dermatitis and healthy controls.

Cultures for Malassezia yeasts were taken from both normal-looking skin and lesional skin in 124 patients with atopic dermatitis, 16 patients with seborrhoeic dermatitis and from normal skin of 31 healthy controls. Positive Malassezia growth was found in fewer patients with atopic dermatitis (56%) than in patients with seborrhoeic dermatitis (88%) or in healthy controls (84%, p<0.01). In the patients with atopic dermatitis, fewer positive cultures were found in lesional (28%) than in non-lesional skin (44%, p<0.05), while positive cultures were found in 75% of both lesional and non-lesional sk…

[The role of bacteria on facial rosacea-like dermatitis in adult type atopic dermatitis].

To investigate the role of bacteria on facial rosacea-like dermatitis lesion in adult type atopic dermatitis, we measured the numbers of bacteria on the skin, nasal and pharyngeal surface of each individual, and then examined its sensitivity to antibiotics. The numbers of bacteria on the surface of skin increased according to the severity of skin eruption. Staphylococcus aureus was detected from the skin and nasal surface more than pharyngeal surface. Bacteria on the pharyngeal surface were both staphylococcus aureus and haemophilus parainfluenzae. Therefore, it is suggested that bacterial flo…

Dapsone for treatment of pruritic dermatitis (dermatitis herpetiformis and subcorneal pustular dermatosis) in dogs.

Pruritic dermatitis in 4 dogs was not responsive to treatment with corticosteroids and antibiotics. Histologic findings in 3 cases were suggestive of subcorneal pustular dermatosis, and those in the 4th case were compatible with dermatitis herpetiformis. All 4 dogs responded rapidly and completely to treatment with dapsone.
