Viral infections in atopic dermatitis: pathogenic aspects and clinical management.

A number of different widespread and disseminated viral infections can occur in patients with atopic dermatitis. Eczema molluscatum is troublesome but not dangerous. Although eczema vaccinatum is rare, it is life-threatening and of increased concern as smallpox vaccinations are reintroduced as a response to possible bioterrorism. There is little information on the course of smallpox itself in atopic dermatitis. Eczema herpeticum is the most common member of this group; recent advances in understanding its pathogenesis might contribute to a more successful management of this serious complicatio

Risk of atopic dermatitis and the atopic march paradigm in children of mothers with atopic illnesses: A birth cohort study from the United Kingdom.

BACKGROUND: Atopic dermatitis (AD) is thought to precede the onset of other allergic illness (OAI) in a temporal progression (ie, atopic march), yet the timing and progression has been questioned. It is also unclear how parental allergic illness impacts the development of these illnesses in offspring. OBJECTIVE: (1) Explore risk of incident AD and (2) timing of allergic disease onset in children of mothers with AD compared with mothers without AD from the United Kingdom. METHODS: We created a birth cohort of mother-child pairs using IQVIA Medical Research Data database and developed Cox prop

Clinical comparison of human and canine atopic dermatitis using human diagnostic criteria (Japanese Dermatological Association, 2009): proposal of provisional diagnostic criteria for canine atopic dermatitis.

Atopic dermatitis (AD) is a common skin disease encountered in both humans and dogs. Canine AD can be used in the analysis of naturally occurring AD; however, details of clinical comparison have been lacking. The purpose of this study is to compare those clinical features using the human diagnostic criteria (Japanese Dermatological Association, 2009). Fifty-one dogs with canine AD were evaluated by the human criteria. Prior to this study, canine AD was basically diagnosed by the fulfillment of two authentic canine AD criteria and a positive reaction against Dermatophagoides farinae in serum im

Atopy patch test reactions show a rapid influx of inflammatory dendritic epidermal cells in patients with extrinsic atopic dermatitis and patients with intrinsic atopic dermatitis.

BACKGROUND: Normal human skin harbors a single epidermal dendritic cell (DC) population, the CD1a(+++)CD11b(-) Langerhans cells. In many chronic inflammatory skin diseases, the epidermal DC pool bears a second population, the CD1a(+)CD11b(+++) inflammatory dendritic epidermal cells (IDECs). Immunophenotypic, ultrastructural, and functional aspects of IDECs have been investigated in chronic untreated skin lesions of intrinsic and extrinsic atopic dermatitis (AD), contact dermatitis (CD), and psoriasis, but little is known about freshly induced early skin lesions. OBJECTIVE: We sought to charac

Characterisation and Profiling of Standard Atopic Dermatitis Therapies in a Chronic Dermatitis Murine Model Induced by Oxazolone.

Atopic dermatitis (AD) is a common inflammatory skin disorder characterised by hypersensitivity to allergens, eczematous lesions and pruritus. The aim of this study was to comprehensively characterise a murine model of dermatitis and assess the similarity with the human disease, as well as to profile clinically relevant AD therapies. Four repeated topical administrations of oxazolone in the auricular skin of sensitised mice induced morphological features compatible with AD, including redness and swelling, as well as histological changes typical of spongiotic (eczematous) dermatitis and increas

A comparative study of childhood psoriasis and atopic dermatitis and greater understanding of the overlapping condition, psoriasis-dermatitis.

BACKGROUND AND OBJECTIVES: Psoriasis (Pso) in children may be confused clinically with atopic dermatitis (AD) and, indeed, the two conditions may co-exist. The aim of this study was to determine historical and clinical features that are different in paediatric Pso and AD and to describe children who have features of both: psoriasis-dermatitis overlap (PD). METHODS: Children with features of psoriasis or eczema, or both, who were referred to paediatric outpatients and/or private rooms were evaluated. Data were collected from 170 consecutive children aged less than 12 years between July 2011 an

Diagnostic definitions shape genome-wide association findings in atopic dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by heterogeneous pathophysiology and considerable interindividual variability in clinical manifestations. Due to this diversity, multiple diagnostic criteria have been established for AD. Variability in these criteria can substantially affect diagnostic accuracy and, consequently, the outcomes of genome-wide association studies (GWAS), whose reliance on this diagnosis variability has not been thoroughly examined. In this study, we conducted GWAS of AD using two diagnostic definitions within the same cohort: self-report

Transcriptomic Analysis Reveals Mitochondrial-Driven Subgroups in Atopic Dermatitis Lesions.

Atopic dermatitis (AD) is a common and chronic inflammatory skin disease with global prevalence. Its clinical heterogeneity and complex molecular mechanisms pose significant challenges for the development of effective therapies. To explore AD molecular heterogeneity using lesional skin transcriptomic data, characterize their biological and immune profiles, and identify key genes underlying the differentiation. Differentially expressed genes were identified using DESeq2, followed by pathway and co-expression analyses via GSEA and WGCNA, respectively. Mitochondrial-related genes were extracted b

Calcineurin Inhibitors in Atopic Dermatitis: Balancing Tradition with Emerging Therapeutics.

Atopic dermatitis (AD) is a common chronic inflammatory condition of the skin that has increased dramatically over the past decade and significantly impacts individual quality of life. Corticosteroids are still the primary therapy for AD, but there are limitations to their continued use due to potential adverse effects, particularly when used in sensitive areas. Topical calcineurin inhibitors (CNIs), such as tacrolimus and pimecrolimus, are available as a safe, steroid-sparing alternative that directly inhibit calcineurin-mediated activation of T cells and have been shown to be efficacious acc

Thermosensitive Hydrogel Enables Noninvasive Extracellular Vesicle Therapy for Atopic Dermatitis.

Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by impaired barrier function and helper T cell 2 (Th2)-dominant immune dysregulation. Subcutaneous administration of mesenchymal-stem-cell-derived extracellular vesicles (MSC-EVs) has shown substantial efficacy in preclinical AD models; however, its clinical utility remains restricted by injection-associated pain and poor patient compliance, necessitating more patient-friendly delivery strategies. Here, we propose a patient-friendly, noninvasive therapeutic strategy using clinical-grade MSC-EVs incorporated into a ther
