Nonsteroidal anti-inflammatory drugs (NSAID) are used extensively in the general population. It's well known the adverse effects of NSAID over the upper gastrointestinal tract and small intestine. Enteric-coated and slow release preparations were created in order to prevent those effects. Pleura, diaphragm, pericardial fat pad, intercostal muscles and omentum can be used to protect and revascularize the bronchial suture line. In three patients, two males aged 66 and 67 years with among other disorders chronic obstructive pulmonary disease (COPD) and one woman aged 24 years with a history of intestinal surgery and current abdominal pain, the chest X-ray showed free air below the diaphragm. Since the mid-1980s, unilateral agenesis of the diaphragm (DA) has attracted the attention of paediatric surgeons as more babies affected by this extreme form of congenital diaphragmatic hernia (CDH) survive. Some authors believe that it represents a separate clinical entity. Total eventration of the hemidiaphragm is a rare condition in adults. We report a 75-year-old woman with large eventration of the right diaphragm who required an emergency plication because of acute progressive respiratory distress. The symptom disappeared immediately after operation. Even in asymptomatic elderly patients with eventration, close follow-up is recommended.

Trichinella spiralis larvae infect and develop within skeletal muscle cells causing major changes to their mechanical properties. The aim of this investigation was to determine the effects of T. spiralis on the power output and fatigue resistance of the mammalian diaphragm under conditions simulating in vivo operation and to relate these to respiratory performance. Diaphragmatic disease is a rare manifestation of endometriosis. In the literature it is most often revealed by catamenial pneumothorax. We describe the case of a 21-year old woman presenting with tight-sided phrenic neuralgia, lasting 18 months. Signs of pelvic endometriosis appearing later on, laparoscopy showing thin lesions of endometriosis on the diaphragmatic undersurface. We review the few similar cases described earlier. Respiratory dysfunction is an important complication of acute stroke but its mechanisms are poorly understood. Previous indirect assessments suggest that paralysis of the diaphragm occurs contralateral to the cerebral lesion. A first case of lung herniation into abdominal cavity with traumatic diaphragmatic rupture is described. A 41-year-old Japanese man with chronic obstructive pulmonary disease suffered traumatic diaphragmatic rupture. He needed to get continuous mandatory ventilation for a long time. At the time of operation, left lung herniation into the abdominal cavity was observed. When those who have chronic obstructive pulmonary disease suffered thoraco-abdominal injury, we should take care that pulmonary herniation might exist.

During a 6-year period, 52 patients with nonpenetrating trauma to the diaphragm were treated in eight acute care hospitals, serving a 15-county area of Michigan. Charts were reviewed to identify patterns of injury, treatment, and outcome. Preoperative diagnosis was made in 50 per cent of cases based on chest x-ray findings; the remainder were diagnosed intraoperatively. The cause of the reduced lung volume in the 'shrinking lung' syndrome of systemic lupus erythematosus (SLE) was investigated in 12 patients with the condition.
