Heart transplantation for dilated cardiomyopathy.

Between 1988 and 1994, 23 patients underwent heart transplantation for dilated cardiomyopathy. The age of the 13 boys and 10 girls was from 8 months to 16 years (mean 7.1 years). Selection criteria included failure to thrive despite maximal antifailure treatment and/or intravenous inotrope dependence. The aetiology of cardiomyopathy was idiopathic (n = 13), congenital (n = 3), anthracycline induced (n = 4), Barth's syndrome (n = 1), and maternal systemic lupus erythematosus (n = 2). The waiting period of heart transplantation ranged from one day to 147 days (mean 22 days). Maintenance immunosu

Transgastric esophagoscopy with antegrade dilation.

An 80-year-old man in whom successful dilation of an obstructed esophagus via retrograde esophagoscopy was carried out is reported here. His nutrition had been previously sustained by a feeding gastrostomy tube inserted under radiologic guidance. This gastrostomy stoma was dilated to allow passage of a pediatric gastroscope into the stomach and esophagus. A guide wire was then negotiated past the esophageal stricture, under endoscopic vision and fluoroscopic guidance, into the oropharynx. Savary dilators were then passed over the guide wire perorally to dilate the stenosis. The patient was abl

Clinical genetics of dilated cardiomyopathy.

The pathogenesis of dilated cardiomyopathy (DCM) remains controversial. Over the last few years there has been a gradual accumulation of evidence suggesting that familial forms of DCM are common and are most likely due to the transmission of a rare autosomal dominant gene. Despite these observations absolute proof that familial DCM is genetic is not available and there are many unanswered questions about the molecular basis of familial DCM. Is familial DCM caused by a single rare genetic defect or are there a number of genes involved which can be transmitted by different mechanisms? What facto

Enteroviral RNA in dilated cardiomyopathy.

The exact cause of dilated cardiomyopathy (DCM) remains uncertain. However, a possibility of transition from coxsackievirus-infected myocarditis to DCM has been suspected. We investigated the role of enteroviral infection in the pathogenesis of DCM. The nested reverse transcriptase polymerase chain reaction (nRT-PCR) was used to detect enteroviral RNA in 45 endomyocardial biopsy tissues obtained from 35 patients with DCM and 10 patients (controls) with other non-infectious cardiac diseases. Enteroviral RNA was detected in 17 (49%) of the 35 patients with DCM. The progression to cardiac failure

The dilated urinary tract in children.

Dilatation of the urinary tract is detected in a number of settings throughout childhood. Children may be asymptomatic or present with urinary tract infection, hematuria, or voiding symptoms. Evaluation of the affected urinary tract may require standard radiographic techniques (VCUG, EU) complemented by ultrasound and nuclear medicine studies. The goal of imaging is to differentiate hydronephrosis (HN) from hydroureteronephrosis (HUN) and to provide the correct diagnosis so that appropriate treatment can be given. This article reviews and demonstrates the findings in some common and uncommon c…

Pathologic anatomy of the dilated cardiomyopathies.

Dilated cardiomyopathies are characterized by systolic pump failure and by dilatation of the ventricular cavity. Thus, they differ from the other 2 main types of cardiomyopathies, namely, hypertrophic cardiomyopathy and restrictive/obliterative cardiomyopathy. The term dilated cardiomyopathy designates a number of heterogeneous syndromes: idiopathic dilated cardiomyopathy, alcoholic cardiomyopathy, postpartal cardiomyopathy, infantile cardiomyopathy with histiocytoid change in cardiac muscle cells, anthracycline cardiomyopathy, Keshan disease, and several ultrastructurally distinct abnormaliti…

Balloon dilation of the sphincter of Oddi.

No controlled studies document either clinical or biochemical efficacy following balloon dilation of the sphincter of Oddi. Because of this and the unacceptable rates of pancreatitis and cholangitis associated with sphincter dilation when compared with the other available modalities of sphincterotomy, stent placement and surgery, this technology currently has little place when dealing with a spastic or stenotic sphincter mechanism.

Balloon dilation of rectal strictures.

Twelve strictures subsequent to colorectal (11) or ileorectal (1) anastomosis were treated with balloon dilation. Six of the anastomoses were located within 10 cm from the anal verge. The stricture diameter was 3-12 mm. The time from operation to finding of stricture was 6 +/- 4 (range 2-17) months and the post-dilation observation time was 9 +/- 7 (1-24) months. Relief of symptoms was obtained in ten patients, in six of them after only one dilation. Treatment is being continued in one of the other two cases, and one was classified as failure after five dilation attempts. Most dilations were p

Dilated cardiomyopathy: current concepts.

Dilated cardiomyopathy is a moderately common syndrome resulting from many causes, many of which are yet to be defined. The syndrome is relatively easy to diagnose in its late congestive stage if valvular abnormality, hypertensive disease, and gross myocardial infarction are absent. However, it should be suspected in patients with undiagnosed chest pain, in patients whose severe arrhythmia has no obvious cause, and in any patient with demonstrable ventricular dilatation or systolic malfunction. It may follow infections, especially viral ones and is found in many deficiency diseases, especially…

Mediastinal impressions on the dilated esophagus.

This article explores how the mediastinum accommodates a significantly dilated esophagus, using achalasia as an anatomic model. This accommodation introduces the concepts of vascular tethering and the paratracheal esophagus as part of a unifying theory of "the path of least resistance." Practical examples demonstrate the importance of understanding these concepts in any disease involving esophageal dilatation.

Percutaneous nephrostomy with immediate dilation.

Percutaneous nephrostomy may serve as an alternative to an open operation for urinary diversion. Difficulties with poor drainage and premature catheter dislodgement have prompted the development of a technique for dilation of the needle tract and placement of a large, self-retaining catheter at the time of the initial puncture.

Head injury and the dilated pupil.

The significance of the dilated pupil in head injury is discussed. History of this condition and its possible mechanisms are described. Three patient reports are presented with unilateral dilated pupil. The literature on the subject is reviewed. Finally, the diagnostic steps which may be taken to differentiate direct third nerve injury from expanding mass lesion are suggested.

Is routine dilatation after repair of esophageal atresia with distal fistula better than dilatation when symptoms arise? Comparison of results of two European pediatric surgical centers.

BACKGROUND/PURPOSE: The aim of this study was to determine whether routine dilatation of the anastomosis after repair of an esophageal atresia with distal fistula (EADF) is superior to a wait-and-see policy with dilatation only when symptoms arise. METHODS: The records of 100 consecutive patients operated on for EADF in 2 European pediatric surgical centers (A [n = 63], B [n = 37]) were reviewed. In center A, dilatation of the anastomosis was carried out in symptomatic cases only, whereas in center B dilatation was begun 3 weeks postoperatively and repeated every 1-3 weeks until a stable diam…
