Over the past several years, the selection criteria for marginal donor lungs have been extended. However, brain-dead patients with implanted mechanical circulatory support systems have not yet been considered as potential organ donors for lung transplantation. Our report presents the first successful procurement and transplantation of donor lungs from a patient supported by a left ventricular assist device. The procurement of the lungs demanded an extensive and careful anatomical preparation of the device, the heart, the lung and the mediastinum. The donor lungs were of good quality. The subse

BACKGROUND AND AIMS: Graft survival in HCV (hepatitis C virus) infected recipients is worse than those transplanted for other liver diseases. We studied whether several donor cardiovascular risk factors (including advanced age, smoking, hypertension, and diabetes mellitus) contribute to worse outcomes for HCV positive and HCV negative liver transplant recipients.
METHODS: We obtained data from the United Network for Organ Sharing on all adult liver transplants performed in the United States between January 1, 1998 and December 31, 2003. In total, 27,033 transplant cases were evaluated. Indepe

BACKGROUND: The living kidney donation has become increasingly important in recent years. Because of the decreasing number of postmortem donors, there has been a dramatic increase in morbidity and mortality due to the long waiting times for patients on dialysis. By timely living donation after dialysis entry or even preemptively, this can be avoided.
AIM: In addition, the living donor has better graft function and better graft survival which is due to the predictability of the donation, the optimal conditioning of donor and recipient, and the short ischemia time. To protect the donor, to prov

PURPOSE OF REVIEW: Living donor liver transplantation (LDLT) increasingly depends on biologically related donors, particularly in Asia and the Middle East, where deceased donation remains limited. This review evaluates genetic risks in living donors and proposes a system-based approach to donor selection.
RECENT FINDINGS: Heterozygous carriers exhibit variable functional capacity determined by enzyme thresholds and epigenetic mechanisms such as X-chromosome lyonization. In ornithine transcarbamylase deficiency (OTCD), maternal donors may have insufficient enzymatic activity, resulting in dono…

Donations from deceased donors have been increasing since the introduction of expanded criteria for donor kidney selection. Several studies have shown that patients receiving deceased donor kidneys using these expanded criteria have improved survival compared to those remaining on the waiting list during hemodialysis. It is important, however, to consider that some of the kidneys classed as usable under the expanded criteria may in fact be unacceptable. To address this concern, preoperative biopsy and imaging of deceased donor kidneys are increasingly being used to assess candidate kidneys. We

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2008eng
For acute liver failure (ALF), living donor liver transplantation (LDLT) may reduce waiting time and provide better timing compared to deceased donor liver transplantation (DDLT). However, there are concerns that a partial graft would result in reduced survival of critically ill LDLT recipients and that the rapid evolution of ALF would lead to selection of inappropriate donors. We report outcomes for ALF patients (and their donors) evaluated for LDLT between 1998 and April 2007 from the Adult-to-Adult Living Donor Liver Transplantation Cohort. Of the 1201 potential LDLT recipients, 14 had ALF,

Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a curative therapy for severe aplastic anemia (SAA), with donor sources including matched sibling donors (MSDs), haploidentical donors (HIDs), and unrelated donors (URDs). However, the optimal criteria for donor selection remain undefined. We performed a multicenter retrospective study of 795 consecutive SAA patients who underwent allo-HSCT between 2012 and 2020 across 11 transplant centers in China. The overall survival (OS), failure-free survival (FFS), and GVHD-free/FFS (GFFS) rates were 85.2%, 83.6%, and 76.1%, respectively.…

Liver transplantation (LT) for unresectable colorectal liver metastases has evolved from early feasibility studies to a biologically selected, multidisciplinary strategy for patients with liver-confined disease. Evidence from the SECA program and randomized data from TransMet support meaningful survival in carefully chosen candidates. Contemporary selection emphasizes sustained chemotherapy response, time-based observation, PET-derived metabolic burden, CEA kinetics, and extrahepatic disease. Living donor LT has expanded LT access within these established frameworks, offering elective timing a

The management of oral malignancy necessitates a comprehensive approach focusing on disease eradication and patient quality of life. Surgery remains pivotal, although extensive resection can lead to aesthetic and functional challenges. Reconstruction, often with osteocutaneous radial forearm free flaps (OCRFFF), is crucial for restoring form and function. Prophylactic plating aims to prevent donor site complications, although plate selection is critical. Despite OCRFFF's efficacy in anterior mandible reconstruction, concerns persist regarding donor site morbidity and adequacy for subsequent de

Bone marrow transplantation was carried out on 38 patients with mucopolysaccharidosis type I over a period of 15 years. The donor was an HLA identical relative in 10 cases, an HLA non-identical relative in 16 cases, and an HLA identical unrelated volunteer donor in 12 cases. Ten patients received a second transplant. One patient received three transplants. Thirteen engrafted patients have survived five years or more. Most patients have shown an arrest or slowing down of psychomotor regression. However, dysostosis multiplex has progressed. Careful selection of patients may be necessary to ensur

Two neonates who became infected with Plasmodium vivax subsequent to exchange transfusion are described. This complication of neonatal exchange transfusion is unusual in that only two other similar cases have been reported to the Centers for Disease Control. Although malaria is an unusual complication, it should be kept in mind when considering the differential diagnoses of fever following neonatal exchange transfusion. The risk of transfusion-induced malaria may be increasing due to the influx of immigrants from areas endemic for malaria. Careful blood donor selection and blood donor educatio…

Identifying primary donors and recipients of the tetM gene is crucial for gaining a deeper understanding of the dynamics underlying the dissemination of resistance to tetracyclines in natural bacterial populations, including those of human and animal pathogens. This study modeled the major donor-recipient network of tetM, also providing important insights into the primary evolutionary mechanisms of this gene. The RDP4- and SplitsTree-embedded algorithms were used to detect genetic recombination events of tetM loci from different bacterial species and genera. FUBAR, MEME, and MEGA11 were employ

Since the establishment of the first eye bank in the 1940s, their role has evolved to face new challenges. With the recent development of lamellar keratoplasties, eye banks play an even bigger role in the selection and preparation of donor tissues. The increasing number of keratoplasty techniques and the high demand for "ready-to-use" tissues are challenging eye banks to improve and develop new preparation techniques. Besides necessary examinations, new approaches of tissue analysis in eye banks allow a better/optimized selection of corneal tissues. These new challenges in tissue preservation,

INTRODUCTION: Living liver donation is a major operation performed without direct medical benefit to the donor. Prospective data on longitudinal donor-reported outcomes after minimally invasive donor hepatectomy remain limited, particularly for robotic approaches offered without donor selection.
METHODS: This prospective observational cohort study included all consecutive living liver donors undergoing robotic donor hepatectomy between April 2024 and March 2025. The robotic approach was offered to all donors without preselection. Donors completed the Short Form-36 (SF-36), Body Image Scale (B…

BACKGROUND AND AIMS: Hepatocyte transplantation for genetic liver diseases has several potential advantages over gene therapy. However, the low efficiency of cell engraftment has limited its clinical implementation. This problem could be overcome by selectively expanding transplanted donor cells until they replace enough of the liver mass to achieve therapeutic benefit. We previously described a gene therapy method to selectively expand hepatocytes deficient in cytochrome p450 reductase (Cypor) using acetaminophen (APAP). Because Cypor is required for the transformation of APAP to a hepatotoxi

INTRODUCTION: The persistent shortage of deceased kidney donors in Japan underscores the importance of optimizing donor selection. However, evidence regarding transplant outcomes from donors with impaired renal function remains limited.
METHODS: This retrospective study used data from the Japan Organ Transplant Network to evaluate 549 adult recipients of deceased donor kidney transplantation from brain-dead donors performed between 2001 and 2022. Death-censored graft survival served as the primary endpoint. Prognostic donor factors were identified, and these variables were incorporated into a…

Genetic testing is increasingly transitioning from being a research tool to standard clinical practice, but guidance for integrating genomics into living kidney donor assessment remains limited. This review provides an evidence-based framework for genetic testing in prospective kidney donors within clinical practice in Australia. Kidney donor genetic assessment requires first establishing a genetic diagnosis in the transplant recipient. Identifying which patients with chronic kidney disease warrant genetic testing depends on clinical phenotype, extrarenal manifestations, age of onset, and, cru
