Successful interventional treatment of an octogenarian presenting with syncope and Ebstein's anomaly of the tricuspid valve.

Syncope is a frequent occurrence in the elderly and cardiac causes are common. Therefore, accurate diagnosis is essential since specific treatments are often required. We present an 85-year-old woman with recurrent syncope secondary to sinus node reentrant tachycardia (SNRT), which was successfully treated by radiofrequency ablation. This is the first reported description of SNRT and its successful treatment in Ebstein's anomaly. Atrial arrhythmias and accessory atrioventricular bypass pathways are common in Ebstein's anomaly and are briefly discussed. Our patient is alive at age 87, which is

Unusual ST-t changes in Ebstein's anomaly with occlusion of a nondominant coronary artery.

We present a case report of a patient of Ebstein's anomaly presenting with unusual ECG changes during acute coronary syndrome. The patient had undergone radiofrequency ablation of right posteroseptal accessory pathway. Two years later, he presented with acute chest pain. His ECG revealed ST elevation of 6-7 mm in leads III, aVF. V3R and V1-V4 with atrioventricular dissociation. He was thrombolysed for the same. He subsequently underwent an angiogram for continuing angina. His angiogram showed a nondominant right coronary artery with a 95% stenosis. The left circumflex artery was dominant but w

Successful prenatal digoxin therapy for Ebstein's anomaly with hydrops fetalis. A case report.

BACKGROUND: Ebstein anomaly is a rare tricuspid valve anomaly. Some fetuses with Ebstein's anomaly have concurrent severe cardiac function impairment, which results in hydrops fetalis. Most of these fetuses are inevitably terminally ill. No reports have demonstrated the potential prenatal therapy for fetuses under such conditions. CASE: Ebstein's anomaly and hydrops fetalis were detected at 28 weeks' gestation. Tricuspid regurgitation with congestive heart failure was observed. From 28 to 34 weeks' gestation, intrauterine therapy with digoxin, 0.75 mg/d, was administered. The fetal hydrops st…

The Pediatric Radiofrequency Ablation Registry's experience with Ebstein's anomaly. Pediatric Electrophysiology Society.

INTRODUCTION: Abnormal anatomy and complex electrophysiology in patients with Ebstein's anomaly of the tricuspid valve may confound attempts at radiofrequency ablation (RFA). METHODS AND RESULTS: Data for 65 pediatric Ebstein's patients (9.8+/-5.4 years, 4 months to 20 years; 39+/-25 kg, 5.1 to 108 kg) were obtained from the Pediatric Radiofrequency Ablation Registry. The degree of tricuspid regurgitation (DOTR) and the degree of Ebstein's anomaly were assessed with echocardiography/Doppler. Leading indications were drug refractoriness (24 [37%] of 65 patients) and life-threatening arrhythmia

The 'sail sound' and tricuspid regurgitation in Ebstein's anomaly: the value of echocardiography in evaluating their mechanisms.

We describe a patient with Ebstein's anomaly in whom Doppler echocardiography was used to clarify the mechanism responsible for 'sail sound' and tricuspid regurgitation associated with this condition. Phonocardiography revealed an additional early systolic heart sound, consisting of a first low-amplitude component (T1) and a second high-amplitude component (T2, 'sail sound'). In simultaneous recordings of the tricuspid valve motion using M mode echocardiography and phonocardiography, the closing of the tricuspid valve occurred with T1 which originated at the tip of the tricuspid leaflets, whil

[A case of reoperation of Ebstein's anomaly with increased tricuspid regurgitation after Hardy's operation].

A 30-year-old man, who had undergone Hardy's operation and the direct closure of an atrial septal defect for Ebstein's anomaly at age 9, was reoperated on by Carpentier's procedure for a long-standing tricuspid regurgitation. He required IABP support to wean from the cardiopulmonary bypass for the acute heart failure. Nonetheless, his postoperative course was uneventful. Echocardiography, nine months after the operation, showed a decrease in moderate residual regurgitation and an increase in the right ventricular inflow velocity, which were considered signs of an improvement in the right ventr

Surgical treatment of patients with Wolff-Parkinson-White syndrome and associated Ebstein's anomaly.

Ebstein"s anomaly is the most common congenital heart disease associated with the Wolf-Parkinson-White syndrome. Between November 1973 and March 1993, we surgically treated 42 patients with Wolff-Parkinson-White syndrome and Ebstein's anomaly. The patient's ages ranged from 5 months to 59 years (mean 35.3 +/- 14.0 years). There were a total of 52 accessory pathways, 48 of which were located in the right (65%) or posteroseptal (29%) area. A left-sided accessory pathway was seen in only two patients (3.8%). Division of all right-sided accessory pathways was done during normothermic cardiopulmona

Extracorporeal membrane oxygenation for postoperative cardiac support in Ebstein's anomaly: a case report.

A case of successfully prolonged ECMO for post-operative cardiac support was reported on and a review of literatures with regard to the various aspects of this condition was presented. We hope that prolonged ECMO will become a valuable tool in the armamentarium of the surgeon treating postcardiotomy circulatory failure.

Morbid anatomy in neonates with Ebstein's anomaly of the tricuspid valve: pathophysiologic and clinical implications.

The hearts of six neonates with Ebstein's anomaly of the tricuspid valve who died in the 1st month of life were compared with hearts of six age- and size-matched control neonates. All six hearts had morphologically severe disease with gross right atrial dilation and marked apical displacement of the tricuspid valve. All had a secundum atrial septal defect and four had additional cardiac lesions (pulmonary atresia in two, ventricular septal defect in two). There was significant thinning of the right ventricular free wall distal to the tricuspid valve (3 +/- 0.2 mm vs. control 4.2 +/- 0.2, p les

Ebstein's anomaly and partial atrioventricular canal associated with double orifice mitral valve.

A case of Ebstein's anomaly and partial atrioventricular canal associated with double orifice mitral valve is reported. Patch closure of the primum defect, plication of the atrialized ventricle, and tricuspid valve replacement with a St. Jude Medical valve were carried out successfully, and the patient is doing well 2 years and 9 months after the operation. To our knowledge, this combination of congenital heart defects has not been reported in the literature.

Variability in the time interval between tricuspid and mitral valve closure in Ebstein's anomaly.

The time interval between mitral and tricuspid valve closure was measured from M-mode echocardiograms in patients with Ebstein's anomaly. It was found that this time interval demonstrated a range of values within each patient. The use of different transducer positions on the chest wall was found to be the predominant factor. There was no correlation between the measured time interval, its variability, and the moment of measurement during breathing. However, a period of breathholding significantly reduced the variability. This study indicates that a wide range of time intervals might be measure

Surgical intervention in cases of Ebstein's anomaly. Abnormal origin and structure of the tricuspid valve.

During the ten-year period 1972-81 four patients, two men and two women, with Ebstein's disease underwent operation at our institution. Their average age was 34. Central cyanosis at rest, clubbing and polycythaemia were the most common clinical features of the patients. Enlarged heart, a small pulmonary arterial arch and transluminal lung fields were seen in chest X-ray. Operative findings were a grossly enlarged right atrium and ventricle, the latter having a segment that was typically atrialized and thin-walled but was contracting synchronously with the true right ventricle, and a wide varia

Ebstein's anomaly: surgical treatment with tricuspid replacement without right ventricular plication.

Two patients with Ebstein's anomaly and severe cardiac impairment underwent surgical repair with tricuspid valve replacement and closure of associated congenital septal defects. No attempt was made to plicate the atrialized right ventricle. Good cardiac function was obtained at the end of the procedure, with uniform contractions of the whole right ventricle. Both patients are leading normal and active lives 38 and 32 months after operation. We believe that successful repair is achieved chiefly by placing a competent tricuspid valve on its true anatomical position, leaving the underlying atrial
