Missed elbow fractures misdiagnosed as radial head subluxations.

Over a 36-month period, 11 children were treated in our department for fractures about the elbow which initially went undiagnosed, as they had first undergone reduction maneuvers for a suspected radial head subluxation, without prior imaging. The most frequent final diagnosis was supracondylar humerus fracture (n = 4). A fall was reported for six children (54.5%); the mechanism of injury remained unknown in four children (36.4%). In four of eleven children (36.4%) there was moderate or major soft tissue swelling at the time of first presentation, which is uncommon in radial head subluxation. R…

Paediatric biepicondylar elbow fracture dislocation - a case report.

Paediatric elbow biepicondylar fracture dislocations are very rare injuries and have been only published in two independent case reviews. We report a case of 13 years old boy, who sustained this unusual injury after a fall on outstretched hand resulting in an unstable elbow fracture dislocation. Closed reduction was performed followed by delayed ORIF (Open Reduction and Internal Fixation) with K wires. Final follow-up at 14 weeks revealed a stable elbow and satisfactory function with full supination-pronation, range of motion from 0°-120° of flexion and normal muscle strength. This type of inj…

Terrible triad injury of the elbow: current concepts.

Fracture-dislocations of the elbow remain among the most difficult injuries to manage. Historically, the combination of an elbow dislocation, a radial head fracture, and a coronoid process fracture has had a consistently poor outcome; for this reason, it is called the terrible triad. An elbow dislocation associated with a displaced fracture of the radial head and coronoid process almost always renders the elbow unstable, making surgical fixation necessary. The primary goal of surgical fixation is to stabilize the elbow to permit early motion. Recent literature has improved our understanding of…

Median and radial nerve compression about the elbow.

It is important for physicians who treat upper extremity disorders to understand motor palsy or pain syndromes caused by compression of the median and radial nerves about the elbow and forearm. Patients with anterior interosseous nerve syndrome may report hand weakness, whereas those with pronator syndrome may present with pain and paresthesia that can be confused with carpal tunnel syndrome. Patients with posterior interosseous nerve syndrome report hand weakness, whereas those with radial tunnel syndrome report pain in the lateral elbow and forearm, which may be confused with lateral epicond…

Operative management of tennis elbow: a quantitative review.

INTRODUCTION: The results of operative management of tennis elbow are varied, and the indications for surgery are not well codified. Many operative techniques are reported, but a clear consensus on whether a given surgical procedure is more effective over another is yet to be reached. METHODS: We conducted a MEDLINE, CINAHL and EMBASE search on all available scientific articles that reported the outcomes of surgery for lateral epicondylopathy. Keywords used were 'tennis elbow', 'lateral epicondylitis', 'lateral epicondylalgia', 'tendinopathy', 'tendonitis' and 'tendon'. Subheadings used were …

Acute elbow trauma in children: role of ultrasonography.

BACKGROUND: The diagnosis of occult elbow fracture in children is often challenging due to equivocal or negative repeated radiographic findings. OBJECTIVE: To evaluate the potential diagnostic role of US in children who have sustained elbow trauma with an elbow joint effusion but no fracture seen on initial radiographs. MATERIALS AND METHODS: The study included 14 consecutive children (age range 5-15 years) with elbow trauma whose elbow radiographs showed an effusion without fracture who underwent emergency imaging (within the first 72 h) with US and MRI. The aim of US was to demonstrate a l…

Outcome of modified open surgery in tennis elbow.

BACKGROUND: Tennis elbow is a common disorder of upper extremity. It can be treated conservatively in majority of the patients but some resistant cases eventually need surgery. MATERIAL & METHODS: This descriptive study was conducted at Department of Orthopaedics, Ayub Teaching Hospital, Abbottabad, from January 2002 to June 2006. Sixteen patients had surgical release of the extensor tendon origin along with excision of the lateral epicondyle ridge. These patients did not respond to conservative treatment i.e. rest, non- steroidal anti inflammatory drugs (NSAIDS), local steroid injections and…

Treatment of complex below-the-elbow gunshot wounds.

Firearm injuries, many of which involve the hand, wrist, or forearm, pose a serious public health concern in the United States in both medical and economic terms. Despite the preponderance of low-velocity weapons, complex below-the-elbow gunshot injuries are commonly seen. A successful approach to these patients requires efficient treatment and early initiation of rehabilitation. Unfortunately, this goal is often not met because of multiple factors including wound infections, the need to coordinate multiple surgical specialties, long hospital stays, and delayed definitive reconstruction. At ou…

Ultrasound-guided autologous blood injection for tennis elbow.

OBJECTIVE: To assess the efficacy of autologous blood injection under sonographic guidance for the treatment of lateral epicondylitis. DESIGN AND PATIENTS: Thirty-five patients (23 men, 12 women, mean age 40.9) with refractory lateral epicondylitis (mean symptom duration 13.8 months) underwent sonographic evaluation prior to dry-needling the tendon and injection with autologous blood. Patients were reviewed, and measures of Nirschl and Visual Analogue Scores (VAS) were taken pre-procedure and post-procedure, at 4 weeks and 6 months. RESULTS: Following autologous blood injections, significant

Revision of unstable capitellocondylar (unlinked) total elbow replacement.

BACKGROUND: Instability is a recognized complication associated with unlinked total elbow implants. The best form of treatment of this problem is uncertain as very little has been written about it. METHODS: Twelve patients underwent operative treatment of instability at the site of a capitellocondylar unlinked total elbow replacement, and the results were reviewed retrospectively. The study group included ten women and two men with an average age of fifty-eight years. Ten patients had rheumatoid arthritis. Three elbows underwent conversion to a semi-constrained hinged prosthesis. In the other…

Souter-Strathclyde total elbow arthroplasty: medium-term results.

The Souter-Strathclyde unconstrained elbow prosthesis was prospectively studied in 36 patients (45 prostheses) with rheumatoid arthritis (Larsen grade 4 and grade 5). The mean age of the patients at the time of operation was 63 years (range: 39 to 75 years). Eight patients (9 prostheses) died within five years of implantation, from causes unrelated to the elbow arthroplasty. One patient was lost to follow-up, leaving 27 patients (35 prostheses) for review. The mean length of follow-up was 98 months (range: 60 to 174 months). At 8.2 years follow-up, the prosthesis showed a probability of surviv…

Elbow synovectomy on patients with juvenile rheumatoid arthritis.

Twenty-four primary elbow synovectomies were done between 1991 and 1998 at the authors' institution on 19 patients (15 females, four males) with juvenile rheumatoid arthritis. Five bilateral and 14 unilateral procedures were done. The mean age of the patients was 29 years (range, 11-64 years) at the time of surgery and the mean disease duration was 19 years (range, 2-51 years). Preoperatively radiographic destruction of Larsen Grade 1 was detected in 21% of elbows, Grade 2 in 54%, and Grade 3 in 4%. In 21% of elbows no radiographic destruction was present (Grade 0). The cumulative survival rat…

Reciprocal EMG control of elbow extension by FES.

Elbow extension is critical in performing activities of daily living. Individuals with a C5-C6 spinal cord injury have paralyzed elbow extensors, yet retain weak to strong voluntary control of elbow flexion. Previous studies have shown that functional electrical stimulation (FES) of the triceps provides sufficient elbow extension strength and control to greatly improve function. With triceps stimulation applied at a constant level, elbow angle is controlled naturally by voluntary flexion opposing the stimulated extension-referred to as voluntary antagonist control. We have investigated an alte…

Neurophysiological classification of ulnar entrapment across the elbow.

Ulnar nerve entrapment across the elbow (UAE), a common entrapment, requires neurophysiological evaluation for a diagnosis, but a standardised neurophysiological classification is not available. The aim of our study was to evaluate the validity of a neurophysiological classification of UAE, developed by us. To this end, we examined whether sensorimotor deficits, as observed by the physician and as referred by the patients, increased with the neurophysiological severity according to the classification. We performed a multiperspective assessment of 63 consecutive arms from 52 patients with a cli…

Traumatic elbow subluxation in a child: case report.

An 11-year-old boy fell on his outstretched right arm, following which he had pain on the lateral aspect of his right elbow with limitation of range of motion. Roentgenograms showed right elbow subluxation. The elbow was reduced and immobilized in a long arm splint for 5 days. The result was excellent. Elbow subluxation in previous reports was the result of recurrent elbow dislocation with instability. There are no reports of elbow subluxation in patients who did not have underlying recurrent elbow dislocation or elbow instability. This condition is easy to diagnose and has an excellent outcom…

Medial elbow problems in the overhead-throwing athlete.

The elbow is subjected to enormous valgus stresses during the throwing motion, which places the overhead-throwing athlete at considerable risk for injury. Injuries involving the structures of the medial elbow occur in distinct patterns. Although acute injuries of the medial elbow can occur, the majority are overuse injuries as a result of the repetitive forces imparted to the elbow by throwing. Injury to the ulnar collateral ligament complex results in valgus instability. Valgus extension overload leads to diffuse osseous changes within the elbow joint and secondary posteromedial impingement. …

Acute elbow injuries in the National Football League.

We performed a retrospective review to evaluate acute medial collateral ligament injuries of the elbow in professional football players from 1991 to 1996 (5 seasons). There were 5 acute medial collateral ligament injuries in 4 players (1 player with bilateral involvement). All injuries occurred with the hand planted on the playing surface while a valgus or hyperextension force was applied to the elbow. There were 2 centers, both involved with long-snapping situations, 1 running back, and 1 quarterback. All elbows had valgus instability on physical examination. Despite this instability, all pla…

[Results of posttraumatic elbow arthrolyses: a prospective study].

INTRODUCTION: Posttraumatic stiffened joints are mostly well restored by means of operation, however, the range of motion remains restricted by secondary soft tissue shrinking and scarring. Conservative treatment yields only limited success rates. Open arthrolysis is a useful tool in regaining function; in postoperative treatment, immediate passive mobilization and prevention of heterotopic ossification are most important. METHODS: Sixty-nine patients were arthrolyzed on average 25.3 months post trauma, with an average of 1.5 previous operations (range 0-8) and a follow-up period of 14.7 mont
