INTRODUCTION: Lateral humeral condyle (LC) fractures are the second most common pediatric elbow fractures. Traditionally, displaced fractures have been treated with open reduction although recent studies have demonstrated successful outcomes of closed reduction for similar injuries. This study investigates the outcomes comparing open and closed reduction in a large cohort of children with moderately displaced (Song classification types 1 to 4) lateral humeral condyle fractures.
METHODS: Retrospective data from patients aged between 1 and 12 years treated for lateral condyle fractures was coll…

BACKGROUND: Fractures of the proximal third of the ulna and radius with associated anterior radial head dislocation are uncommon in children. Early recognition and appropriate management are essential to prevent long-term consequences of loss of forearm rotation, cubitus valgus, elbow instability and chronic pain.
CASE PRESENTATION: We present the case of a 3-year-old Caucasian boy who attended the emergency department following an un-witnessed fall, resulting in right elbow and forearm pain, swelling and deformity. Clinical and radiological examination revealed a Monteggia type IV fracture-d

CASE: A 5-year-old boy sustained a right elbow fracture dislocation with an associated displaced comminuted coronoid fracture. Acute closed reduction as well as an initial attempt at open repair and fixation failed. This case was salvaged 8 weeks after injury with coronoid reconstruction using an iliac crest allograft.
CONCLUSION: This is the first reported case of coronoid reconstruction using iliac crest allograft in a pediatric patient. 2.5 years after reconstruction, the elbow joint is stable, pain free, and demonstrates elbow motion well within the range for full function.

The patient was a 22-year-old man who was currently serving in a military special operations training program. He was referred to a physical therapist for a chief complaint of left elbow pain that currently prevented him from performing routine upper extremity exercise activities. Due to the traumatic nature of the patient's injury, inability to fully extend his elbow, and palpation findings, there was concern for a radial head fracture. Therefore, the physical therapist ordered radiographs of the left elbow, which revealed an intra-articular fracture involving the radial head that extended th

BACKGROUND: Radial head fractures can occur in isolation or in association with elbow and forearm injuries. Treatment options include nonoperative management, fragment or whole-head excision, open reduction and internal fixation (ORIF), and radial head arthroplasty. However, the evidence supporting ORIF for repairable radial head fractures is inconclusive.
QUESTIONS/PURPOSES: We compared patients undergoing ORIF for isolated radial head fractures or for radial head fractures associated with other fractures or elbow dislocations in terms of patient-related disability, presence of posttraumatic

INTRODUCTION: Radial head and neck fractures represent a significant portion of elbow fractures in adults, particularly affecting younger patients due to falls onto outstretched hands. Surgical treatment is often necessary for displaced and comminuted fractures, especially those with associated ligamentous injuries. This study evaluates the long-term clinical, functional, and radiological outcomes of patients with Mason type III and IV radial head fractures managed through open reduction and internal fixation using plate and screw osteosynthesis, due to the unavailability of radial head prosth…

BACKGROUND: This study evaluated the clinical and radiologic results of allarthroscopic treatment of terrible triad of the elbow over a minimum of 5 years.
MATERIALS AND METHODS: We retrospectively reviewed consecutive patients with terrible triad injury who underwent all-arthroscopic surgery between January 2011 and June 2018. We performed all-arthroscopic treatment for these patients, excluding those with a Regan-Morrey type III coronoid process fracture, involvement of the anteromedial facet, or a radial head fracture involving >50% of the articular surface. Elbow stability, range of motio

PURPOSE: Long-term nonunion of the lateral humerus condyle fracture may lead to progressive cubitus valgus, elbow pain and instability, and secondary ulnar neuritis. A number of techniques of osteotomy are available for correction, but each has its disadvantages. The aim of this study was to present a technique of medial trapezoidal osteotomy for correcting nonunion of the lateral humeral condyle with an elbow valgus deformity >20°.
METHODS: Eight patients (mean age, 7.5 years) with cubitus valgus, after neglected nonunion of a lateral humeral condyle fracture of greater than 2 years duration

Legal appraisals of possible treatment errors in childhood and adolescence are rare, often refer to the treatment of trauma of the upper limbs and are frequently recognized. From the activity of the first author as an expert witness 31 expert opinions (25%) referred to the upper limbs. Supracondylar humeral fractures, radius neck fractures, radial condyle fractures and distal radius fractures were predominant. A primary treatment error was determined in 14 patients and a secondary error in 7 patients. This corresponds to a recognition rate of 68%. The aim of this study was the reflection and t

INTRODUCTION: Capitellum fractures are rare, representing only 1% of all elbow injuries. McKee Type 4 capitellum fractures encompass osteochondral shearing fractures of the capitellum, which involve a significant lateral portion of the trochlear ridge and the trochlea. These fractures may be linked with soft tissue and bone injuries surrounding the elbow, such as distal triceps tendon avulsion (DTTA) injuries. To date, only three case reports in the English literature have documented the correlation between Type 4 capitellum and trochlear fractures with DTTA injuries. Orthopedic surgeons shoul

INTRODUCTION: Complex proximal ulna fractures that are associated with radial head dislocation or fracture-dislocation, often referred to as "Monteggia-like" or trans-ulnar fracture-dislocations, constitute a heterogeneous group that poses challenges for classification and treatment. Management often relies on surgeon expertise rather than systematic approaches, leading to poor outcomes, high complication rates, and frequent surgical revisions. We hypothesize that a systematic, coronoid-centric, step-by-step surgical technique could improve outcomes in trans-ulnar fracture-dislocations.

Elbow injuries make up to 3% to 4% of all emergency department presentations and are often difficult to diagnose. These injuries are often missed on radiographs because of the large cartilaginous component of the pediatric elbow resulting in malunion. Fractures around the elbow joint are one of the leading causes of litigation claim, and awareness of the pitfalls in diagnosis of these subtle injuries is necessary for a prompt diagnosis. Fracture of the olecranon epiphysis is rare and often being described around puberty and in association with osteogenesis imperfecta. Management using K-wire t

BACKGROUND: Isolated coronoid fractures without concomitant radial head injury raise suspicion for an anteromedial facet (AMF) fracture as a result of a varus posteromedial rotatory instability (VPMRI) mechanism. However, not all isolated coronoid fractures involve the AMF, nor are they all the result of a VPMRI mechanism. AMF fractures as a result of a VPMRI mechanism have been reported to have specific radiographic features (medially oblique, concave, with extension to the sublime tubercle). We hypothesized that a detailed assessment of the pattern of AMF fracture utilizing three-dimensional

Traumatic distal radioulnar joint (DRUJ) dislocation with or without an associated fracture is a rare injury. When coupled with a radial head fracture this is commonly known as the Essex-Lopresti injury. We report two cases of elbow dislocation with ipsilateral radial neck fractures and associated true DRUJ dislocations. This has not been previously described in the literature. In elbow injuries with wrist involvement, symptoms in the latter may be subtle. Due to inadequate examination of the affected joint, poor initial radiographic views, and general rarity of this injury, distal radioulnar

BACKGROUND: Fracture-dislocations of the elbow, particularly those that involve a fracture through the proximal ulna, are complex and can be difficult to manage. Moreover, current classification systems often cannot discriminate between Monteggia-variant injury patterns and trans-olecranon fracture-dislocations, particularly when the fracture involves the coronoid. The Mayo classification of proximal trans-ulnar fracture-dislocations categorizes these fractures into 3 types according to what the coronoid is still attached to: trans-olecranon fracture-dislocations (the coronoid is still attache
