[Electrocochleography possibilities in the differential diagnosis of hydrops and neural hearing loss].

Enhancement of the click-evoked summating potential causing a pathological summation to compound action potential ratio (SP/CAP) in patients with endolymphatic hydrops has been described by several groups. An increased SP/CAP ratio is found also in some patients with a tumour of the cerebellopontine angle. The study of further parameters of the SP/CAP complex, such as the CAP latency and the width of the complex, permit an electrocochleographic differentiation between both forms of hearing loss. The N2 amplitude showed no diagnostic significance.

[Current aspects in Menière's syndrome].

The authors report, on the basis of the data in the literature and their own experience, the main elements in etiopathology, positive and differential diagnosis and treatment of Ménière's syndrome. The new discoveries of the genesis of endolymphatic hydrops, methods of clinic diagnosis and paraclinical investigations (liminal and supraliminal tonal audiometry, impedance measurement, Luscher's, Metz's audiometric tests, and the balance test--Fowler's) are described. After presenting the affections which have to be left aside in the diagnosis of Ménière's syndrome, the paper reports shortly on t…

Juvenile Menière's disease.

The authors report their experience in the diagnosis and treatment of Menière's disease in childhood. They believe it to be essential to carry out a complete battery of audio-vestibular tests, with particular reference to dehydration procedures, which in this age group are considered to be more significant for a correct diagnosis than in adults. After stressing the importance of eliminating other forms of disease which manifest themselves with vertigo, the authors conclude by asserting the efficacy of medical treatment with diuretics and they support the surgical procedure of endolymphatic-mas

Human inner ear fluid imbalance detected by optical coherence tomography correlates with hearing loss.

Hearing loss and vertigo occur when there is an imbalance between the two inner ear fluids, endolymph and perilymph. The inner ear is a smal … rols, both patient groups demonstrated increased endolymph and reduced perilymph, a disorder termed endolymphatic hydrops. OCT imaging demonstrated good repeatability for measuring the endolymph-to-perilymph ratio. Our data indicate that increased endolymph-to-perilymph ratios correlated with the degree of hearing loss. Thus, small yet meaningful changes in inner ear f …

Cerebral Small Vessel Disease in Elderly Patients With Menière's Disease.

BACKGROUND: Menière's disease (MD) is an inner ear disease characterized by vertigo attacks, progressive hearing loss, tinnitus, and the sensation of aural fullness. Although the exact pathophysiology of MD is unknown, endolymphatic hydrops is considered to be its histopathological hallmark. It has been suggested that endolymphatic hydrops results from lowered perfusion pressure due to cardiovascular comorbidity. Cardiovascular risk factors can cause cerebral small vessel disease (CSVD), visible on MRI. The presence of CSVD in turn raises the risk of developing a stroke. OBJECTIVES: This stud…

The Concept of a Rupture Risk Envelope for the Cochleo-Saccular Membranes.

Introduction  Alterations in endolymphatic pressure have long been suspected of being associated with the development of endolymphatic hydrops and rupture of the membranous labyrinth. More recently, there has been a focus on how membrane mechanics might contribute to membrane rupture. This is suspected to involve the viscoelastoplastic properties of these membranes. Objective  To construct a rupture risk envelope for the cochleo-saccular membranes based on viscoelastoplasticity to provide insight into lesion behavior in Meniere disease. Methods  Reported deformation data from a collagen model …

3D-reconstructions of Bast's Valve and Membranous Labyrinth: Insights for Vestibular Implantation and Meniere's Disease.

HYPOTHESIS/BACKGROUND: Bast's valve is a poorly understood inner ear structure located at the junction between pars superior and inferior in the membranous labyrinth. Anatomically precise three-dimensional reconstructions (3D-reconstructions) of Bast's valve can help illuminate the morphology of the valve, and point toward its role in normal physiology and pathological states such as endolymphatic hydrops. This is of particular relevance to the development of a vestibular implant, a device intended to rehabilitate deficits in the vestibular system. METHODS: Six postmortem human temporal bones…

Electrocochleography and gentamicin therapy for Ménière's disease: a preliminary report.

OBJECTIVE: It is widely held that an enlarged summating potential (SP) relative to the eighth nerve action potential (AP) is a reflection of endolymphatic hydrops. Aminoglycosides are an accepted treatment for incapacitating Ménière's disease and are known to affect both sensory and secretory cells of the inner ear. The intent of this study was to determine whether this effect on secretory cells could be objectively confirmed by virtue of changes in the electrocochleogram (ECoG) of patients receiving gentamicin therapy for Ménière's disease. STUDY DESIGN: This was a prospective longitudinal s…

[Familial deafness showing hearing pattern of low-tone losses].

Clinical and electrocochleographic studies of low tone familial deafness were carried out in 19 patients of 10 families. These hearing impai … that the hearing impairment resulted from a localized abnormality of the cochlear apex, and without endolymphatic hydrops. On the other hand, patients with Ménière's disease and LTSD showed a high -SP amplitude and a satisfactory detection threshold of CM compared with hearing level of the pure tone audiogram. These findings suggest that both Ménière's disease and LTSD …

HSP70 antibodies in 80 patients with "clinically certain" Meniere's disease.

OBJECTIVES: We tested the claim that a significant proportion of patients with Meniere's disease have antibodies to heat shock protein 70 (HSP70) antigen, which may lead to defective endolymphatic sac function and vertigo attacks. METHODS: Serum samples were taken from 80 subjects with a "certain" diagnosis of Meniere's disease (American Academy criteria plus electrocochleographic confirmation of endolymphatic hydrops with tone burst stimuli) and were tested for HSP70 antibodies with the OTOblot (hsp70) Western blot assay. The response was recorded as negative, positive, or equivocal. Samples…

Multiple otopathologic disorders.

Temporal bones (1,383) from 713 patients were studied systematically for multiple pathologic lesions. Eleven percent (152 temporal bones) were found to have more than one pathologic finding. Males (60.5%) had multiple diseases more commonly than did females (37.7%). The most frequently occurring findings were otitis media (71.1%), otosclerosis (43.4%), endolymphatic hydrops (38.8%), labyrinthitis (25.0%), and cancer (24.3%). We conclude that multiple coexisting pathologic conditions can have coincidental or causative relationships. The otolaryngologist should consider multiple pathologic condi…

Assessment of inner ear morphology and function in response to local positive pressure for Ménière's disease: a nonrandomized controlled trial.

Ménière's disease (MD) is an inner ear disorder in which the main pathological feature is endolymphatic hydrops (EH). Positive pressure therapy (PPT) using a portable device is now a second-line therapy for intractable MD when initial medical treatment fails. However, it remains unknown whether PPT causes the morphological and functional changes of inner ear in patients with active MD in accordance with reduction of vertigo attacks. In this nonrandomized controlled trial of 52 patients with MD, the volume of EH significantly decreased with reduction of vertigo attacks during 8 months of PPT co…

Micro-lesions in Reissner's membrane evoked by acute hydrops.

In some pathological conditions excessive amounts of endolymph can accumulate and cause swelling, hydrops, of the membranous labyrinth. Reissner's membrane in the cochlea will distend and may even rupture. We have studied the effects of acute hydrops, followed for up to 5-6 h, in a preparation that allows continuous monitoring of structural alterations in individual cells in Reissner's membrane. This is accomplished by using laser confocal microscopy on the membrane visualized by labeling its cells with fluorescent dyes. In specimens subjected to hydrops it was observed that discrete structura

Automated volumetric analysis of the inner ear fluid space from hydrops magnetic resonance imaging using 3D neural networks.

Due to the development of magnetic resonance (MR) imaging processing technology, image-based identification of endolymphatic hydrops (EH) has played an important role in understanding inner ear illnesses, such as Meniere's disease or fluctuating sensorineural hearing loss. We segmented the inner ear, consisting of the cochlea, vestibule, and semicircular canals, using a 3D-based deep neural network model for accurate and automated EH volume ratio calculations. We built a dataset of MR cisternography (MRC) and HYDROPS-Mi2 stacks labeled with the segmentation of the perilymph fluid space and end…
