Antibiotic Use in Acute Upper Respiratory Tract Infections.

Upper respiratory tract infections are responsible for millions of physician visits in the United States annually. Although viruses cause mo … otic use in most cases of acute otitis media, group A beta-hemolytic streptococcal pharyngitis, and epiglottitis and in a limited percentage of acute rhinosinusitis cases. Several evidence-based strategies have been identified to improve the appropriateness of antibiotic prescribing for acute upper respiratory tract infections.

Section four: laryngitis and dysphonia.

Acute laryngitis is most often caused by viral illnesses through direct inflammation of the vocal cords or from irritation due to postnasal drainage. Bacterial infections, such as acute epiglottitis, also can cause dysphonia but typically have other systemic symptoms as well as respiratory distress. Chronic laryngitis is characterized by symptoms lasting more than 3 weeks. Chronic vocal cord issues can be related to overuse or stress on the vocal cords resulting in nodules or polyps. Individuals in certain occupations, such as singers, school teachers, and chemical workers, are at greater risk…

Multipathogenic necrotising supraglottitis in an immunocompetent patient.

Supraglottitis is a potentially life-threatening condition. It is now uncommon due to the Haemophilus influenzae type B vaccination and is more recently caused by Streptococcus pneumoniae, S. pyogenes, H. influenzae non-type B, H. parainfluenzae, Staphylococcus aureus and Pasteurella multocida Very rarely, it can cause necrotising supraglottitis/epiglottitis, and this has been reported in immunocompromised individuals. We present a unique case of multipathogenic supraglottitis causing laryngeal fibrinoid necrosis in an immunocompetent patient. During his admission, the patient was critically u…

Inhalation of steaming seafood aroma: sudden death in an asthmatic.

A 29-year-old asthmatic with a family history of atopy inhaled a large dose of fumes from sizzling, steaming seafood being served to another client in a restaurant. She immediately developed respiratory distress, loss of consciousness and cyanosis; she also vomited profusely, aspirated and died with 58 min of the onset of symptoms. Autopsy revealed epiglottitis and tracheobronchitis associated with an infiltration by mast cells. The blood IgE was marginally elevated. Death was attributed to upper respiratory airway obstruction due to aspiration of gastric contents, and to bronchospasm followin…

Croup: pathogenesis and management.

Croup syndromes are common in children, most frequently being infectious in origin. Children present with a slow progression of inspiratory and expiratory stridor and a croupy, "barking seal" cough. Children are variably febrile and with progression of disease, exhaustion, agitation, cyanosis and air hunger may develop. The evaluation of the patient must focus on the degree of respiratory distress and associated findings. Epiglottitis and foreign body aspiration must be excluded. Management is primarily dependent upon administration of humidified air. Children with moderate to severe croup ben…

The Effect of Bolus Viscosity and Dysphagia Severity on Laryngeal Closure During Swallowing in Patients with Post-Stroke Dysphagia: A Kinematic Analysis Using Swallowing CT.

Previous research in healthy individuals has demonstrated that fast bolus progression of thin liquid results in earlier true vocal fold (TVF) closure compared to thick liquid. This study investigated whether this timing adjustment occurred in individuals with stroke and whether dysphagia severity affected this adjustment. This retrospective observational study included 54 participants with stroke (37 males, 17 females, mean [standard deviation] age: 63.7 [13.8] years) who underwent CT swallow studies with thick and thin liquids (10 ml each). The onset of TVF closure, laryngeal vestibule (LV) c

Swallowing Safety after Remote sub-total Esophagectomy: How Important is Tongue Pressure?

The factors related to oropharyngeal dysphagia after remote esophagectomy (greater than five months) remain unclear. This study aimed to assess patient perception of dysphagia, maximum anterior isometric pressure (MAIP), maximum posterior isometric pressure (MPIP), lingual swallowing pressure (LSP) and radiographic physiological components of the oral and pharyngeal phases of swallowing in patients who are post remote sub-total esophagectomy (SE). Patient perception of dysphagia was assessed using the Eating Assessment Tool (EAT-10). MAIP, MPIP, and LSP were measured using the Iowa Oral Perfor

Development of a novel technique for esophageal reconstruction via oral-vestibule-enteral anastomosis.

BACKGROUND: Narrowing, trauma, tumors, and systemic diseases can cause esophageal dysfunction. Severe cases resist traditional surgery, leading to long-term gastrostomy or jejunostomy tubes, affecting patients negatively. No established surgery ensures both airway and oral function with proper speech. This article introduces the oral-vestibule-enteral anastomosis (OVEA) technique, targeting patients with compromised epiglottic closure competence and loss of cervical esophagus, where conventional methods fall short. METHODS: Technique description study evaluated in 13 patients in a single tert

Physiological changes to the swallowing mechanism following (chemo)radiotherapy for head and neck cancer: a systematic review.

Emerging research suggests that preventative swallowing rehabilitation, undertaken before or during (chemo)radiotherapy ([C]RT), can significantly improve early swallowing outcomes for head and neck cancer (HNC) patients. However, these treatment protocols are highly variable. Determining specific physiological swallowing parameters that are most likely to be impacted post-(C)RT would assist in refining clear targets for preventative rehabilitation. Therefore, this systematic review (1) examined the frequency and prevalence of physiological swallowing deficits observed post-(C)RT for HNC, and

Within-breath control of genioglossal muscle activation in humans: effect of sleep-wake state.

Pharyngeal dilator muscles are clearly important in the pathogenesis of obstructive sleep apnoea syndrome. Substantial data support the role of a local negative pressure reflex in modifying genioglossal activation across inspiration during wakefulness. Using a model of passive negative pressure ventilation, we have previously reported a tight relationship between varying intrapharyngeal negative pressures and genioglossal muscle activation (GGEMG) during wakefulness. In this study, we used this model to examine the slope of the relationship between epiglottic pressure (Pepi) and GGEMG, during

Endoscopy of the upper respiratory tract during treadmill exercise: a clinical study of 100 horses.

Endoscopy of the upper respiratory tract was performed in 100 horses during high speed treadmill exercise. Reasons for endoscopy were a history of an abnormal noise during exercise in 75 horses, poor performance in 17 horses and to evaluate the results of upper respiratory tract surgery in 8 horses. Of the 75 horses with a history of an abnormal noise during exercise the cause was determined in 67 (89%). Endoscopic abnormalities were detected at rest in 40 of these 75 horses (53%). In these 40 horses, a similar diagnosis as to the cause of the abnormal noise was made at rest and during exercis

Multiple Epidermal Inclusion Cysts of Epiglottis.

Epiglottic cysts are benign laryngeal lesions. Though congenital cysts are often life-threatening, they are usually painless and symptomless in adults, or present with slight, though perceptible, interference with either swallowing or speaking or respiration. Early recognition and diagnosis by routine mirror laryngoscopy is advocated. Prompt removal or draining is the treatment. This is a case report of 42-year-old woman with complaints of feeling of something in throat, sleep apnoea and hoarseness, who on routine mirror laryngoscopy was found to have two large cysts arising from the free bord

[Tracheal wall cyst. A rare cause of stridor in the adult].

A case of tracheal cyst is described in a 43-year-old man who was admitted with nonspecific symptoms of an obstruction in the upper respiratory tract. A CT scan of the thorax showed a solid tumor on the inner tracheal wall. Bronchoscopy revealed a tumor with a broad base and smooth surface that almost completely obstructed the trachea. Intraoperatively, the tumor was found to be a cyst and was successfully removed endoscopically. Tracheal cysts are quite rare and most of the reported cases have been confined to pediatric patients as a congenital disease. More frequently, congenital intralumina

Deglutition after supracricoid laryngectomy: compensatory mechanisms and sequelae.

This study is based on the videofluorographic exploration of deglutition in 14 patients who were treated by supracricoid laryngectomy. The choice of this population rests on two criteria: a 1-year postoperative delay, and absence of residual deglutition disorders elicited by patient history. Asymptomatic aspiration was seen in 6 cases. In the cricohyoidoepiglottopexies (CHEP), aspiration occurred uniquely in patients who did not recuperate satisfactorily from epiglottic dynamics. The deglutition sequelae are less invalidating relative to the cricohyoidopexies (CHP), with a possible recuperatio

Laryngeal findings in short rib polydactyly syndrome: case report and embryological correlations.

Short Rib Polydactyly Syndrome (SRPS) is a group of rare congenital disorders characterized by polydactyly, short limbs, and short ribs. Infants with type I SRPS (Majewski syndrome) may exhibit dysgenesis of the larynx, which is manifested by epiglottic hypoplasia. Photographic documentation of laryngeal findings obtained at autopsy in an affected infant is provided. To our knowledge, such photographic documentation has not been previously published. Normal laryngeal development is reviewed, based on the Carnegie system of staging. The developmental errors leading to SRPS are thought to occur

[Usefulness of rapid antigen detection for the diagnosis of systemic infections due to Haemophilus influenzae].

Twenty-eight cases of systemic infections due to Haemophilus influenzae diagnosed from October 1988 to December 1998 were analyzed retrospectively. The clinical manifestations were 13 meningitis (15 episodes), 9 septic arthritis, 4 acute epiglottitis, 1 septicemia and 1 lung abscess. In the 15 meningitis episodes, 13 had positive CSF culture results, and the other 2 episodes of pretreated with antibiotics were diagnosed by H. influenzae type b (Hib) antigen detection by using concentrated urine specimens. In the 9 septic arthritis cases, 6 had positive synovial fluid culture results. Of the 3 …
