How Clean Is the Linen at My Hospital? The Mucorales on Unclean Linen Discovery Study of Large United States Transplant and Cancer Centers.

Mucormycosis outbreaks have been linked to contaminated linen. We performed fungal cultures on freshly-laundered linens at 15 transplant and cancer hospitals. At 33% of hospitals, the linens were visibly unclean. At 20%, Mucorales were recovered from >10% of linens. Studies are needed to understand the clinical significance of our findings.

Evaluating containment effectiveness of A2 and B2 biological safety cabinets.

PURPOSE: This study investigates the use of a canopy-connected recirculating class II type A2 biological safety cabinet (BSC) as an alternative to the B2 when preparing volatile, sterile compounded preparations. Selection of the appropriate BSC for processes that use subgram levels of volatile chemicals is difficult due to a lack of quantitative containment evidence by cabinet type. There is a perception that hazardous compounding must be done in a B2 cabinet due to the potential for vapors, and this study seeks to challenge that perception. METHODS: In total, 5 tests, 3 prequalification test

Shisha microbiota: the good, the bad and the not so ugly.

OBJECTIVE: Over the last decade, there has been a rapid expansion of the trendy water pipe smoking around the world especially among younger adults. The initial objective of this study was to identify the microbiota of the shisha, which may either be of no harm for the smoker or enhance the threat on his well-being. The total DNA for the metagenomics study was conducted on three different shishas from three different delivery shops in Jounieh, Lebanon. The microbiota in two solid parts of the shisha, shaft and hose, were analysed including the fresh tobacco and the water in the bowl. All sampl

Disinfection of needleless connectors with chlorhexidine-alcohol provides long-lasting residual disinfectant activity.

The optimal disinfection method for needleless connectors (NCs) is unclear. We used an experimental model of microbial NC contamination to test different scrub times (swipe, 5, 15, 30 seconds) of chlorhexidine-alcohol versus alcohol and for residual disinfectant activity. Swipe with alcohol did not adequately disinfect NCs, particularly when contaminated with Staphylococcus aureus or Pseudomonas aeruginosa. With ≥5-second scrub, chlorhexidine-alcohol and alcohol performed similarly, but chlorhexidine-alcohol showed residual disinfectant activity for up to 24 hours.

[Virulence factors in Proteus spp. bacteria isolated from urinary tract infections: their detection and importance].

Nosocomial infections associated with biofilm formation have been a serious problem in recent years. Up to 32 % of them are urinary tract infections in patients with long-dwelling catheters. Catheters represent an ideal surface for bacterial adhesion, facilitating easier colonization of the urinary tract. Important pathogens causing these infections are bacteria of the genus Proteus that colonize catheters not only by biofilm formation but also using other virulence factors. Those were developed for survival in the host organism and are also used by bacteria to infect the host or fight the def

Frequency of biocide resistance genes, antibiotic resistance and the effect of chlorhexidine exposure on clinical methicillin-resistant Staphylococcus aureus isolates.

OBJECTIVES: To detect genes conferring resistance to biguanides, quaternary ammonium compounds, beta-lactams and fluoroquinolones in clinical methicillin-resistant Staphylococcus aureus (MRSA) and to demonstrate whether reduced susceptibility is spread clonally and if the presence of any of the detected genes links to a specific epidemic MRSA. Finally, to identify if exposure to chlorhexidine may cause reduced susceptibility to antibiotics and chlorhexidine. METHODS: In total, 120 clinical MRSA isolates were isolated. qacA/B, qacG, qacH, norA, smr and blaZ genes were amplified by PCR. MICs of

Preparation of the Dräger Fabius anesthesia machine for the malignant-hyperthermia susceptible patient.

BACKGROUND: Anesthesia machines must be flushed of halogenated anesthetics before use in patients susceptible to malignant hyperthermia. We studied the kinetics of sevoflurane clearance in the Dräger Fabius anesthesia machine and compared them to a conventional anesthesia machine (Dräger Narkomed GS). METHODS: Before each trial, the anesthesia machine was contaminated for 2 h with 3% sevoflurane and then prepared by changing the CO(2) absorbent, removing the vaporizer(s), and mounting a clean circuit and artificial lung. The basic flush procedure consisted of oxygen 10 L/min with the ventilat

A sensor to detect the early stages in the development of crystalline Proteus mirabilis biofilm on indwelling bladder catheters.

A simple sensor has been developed to detect the early stages of urinary catheter encrustation and avoid the clinical crises induced by catheter blockage. In laboratory models of colonization by Proteus mirabilis, the sensor signaled encrustation at an average time of 43 h before catheters were blocked with crystalline biofilm.

Recommendations and reports about central venous catheter-related infection.

BACKGROUND: Central venous catheters (CVCs) are used to deliver a variety of therapies, as well as for measurement of hemodynamic parameters. The major associated complication is catheter-related blood stream infection (CRBSI). METHOD: Review of the pertinent English-language literature. RESULTS: The incidence of CRBSI depends on how such infections are defined. Generally, the term includes all BSIs in patients with CVCs when other sources can be excluded, and if a culture of the catheter tip demonstrates a substantial number of colonies of the organism found in the blood stream. Important p

[Indwelling catheters in the bladder are often associated with problems. Complications are difficult to prevent and alleviate].

Long-term indwelling catheters are often associated with complications in the form of pain, concrement formation, outflow obstruction and symptomatic infections. The methods used to alleviate the situation are often ineffective and as for chlorhexidine bladder washouts without any effect documented by scientific studies. The best way to avoid catheter-associated complications is to, as far as possible, avoid long-time use of them.

Bacterial adherence to ofloxacin-blended polylactone-coated self-reinforced L-lactic acid polymer urological stents.

OBJECTIVE: To determine whether ofloxacin coating has any effect on bacterial adherence to bioresorbable self-reinforced L-lactic acid polymer (SR-PLLA) urological stents. MATERIALS AND METHODS: SR-PLLA stents were coated with epsilon-caprolactone/L-lactide copolymer blended with ofloxacin at three different concentrations of ofloxacin (0.5, 2 and 5% w/w). The adherence of five bacterial strains (Pseudomonas aeruginosa, Enterococcus faecalis, Proteus mirabilis and two strains of Escherichia coli) to the coated SR-PLLA stents was analysed. Uncoated stent pieces were used as controls. The effec

Pneumococcal bacteremia associated with an infected central venous catheter.

Pneumococcus (Streptococcus pneumoniae) bacteremia is a serious infection. Pneumococcus has never been implicated as a cause of a central venous catheter-related bacteremia. It has been isolated from the catheter tip only twice before, and in one case caused the infection of an infusion port device. We report case of a 41-year-old woman who developed pneumococcal bacteremia after 6 days of an indwelling central venous catheter. The catheter tip grew > 300 cfu of S pneumoniae by the roll-plate method described by Maki and colleagues. No other focus of infection could be found in this patient. T
